In situ breast carcinoma represents an early localized stage in the development of invasive breast carcinoma, which has an especially favorable prognosis with appropriate management. Its increased frequency of diagnosis in recent years is a tribute to the success of efforts at early detection of this disease. The two histologic forms of this entity are distinct in their pathologic and behavioral characteristics, as well as in their therapeutic implications. Lobular carcinoma in situ may be viewed as a marker of risk for subsequent carcinoma and is generally managed with nonoperative observatio

A total of 150 patients with in situ breast cancer were treated from 1970 through 1976, with a median follow-up of 11.5 years. There were 42 patients with microinvasion and 25 with bilateral disease, analyzed separately. Eight patients who became unavailable for follow-up within five years (but who had no evidence of disease [NED]) were excluded. Eighty-five patients had ductal carcinoma in situ, 43 had lobular carcinoma in situ, and 22 had a mixed type. Modified radical mastectomy was done in 112 cases. One of 128 patients had positive axillary nodes; axillary dissection is not indicated for

In situ carcinoma of the breast is being diagnosed with greater frequency on breast biopsies. Of the two types of CIS, lobular carcinoma in situ is considered a marker for increased risk of developing breast cancer in either breast, and treatment options are based on different philosophies of careful follow-up vs preventive surgery. Ductal carcinoma in situ is a direct precursor to invasive carcinoma and the variety of treatments available reflect the need to completely excise this lesion. Several trials are now in progress to define the roles of surgery, radiation, and hormonal manipulation i

The optimal management of ductal carcinoma in situ of the breast is controversial. With the introduction of the National Mammographic Breast Screening Programme the condition will be encountered more frequently. We have reviewed 76 patients from a 12-year period treated by one surgeon (R.W.B.) at the Nottingham City Hospital. Fifty-nine patients had either ductal carcinoma in situ or lobular carcinoma in situ; 17 patients had Paget's disease. The mean age at diagnosis was 54 years and the commonest mode of presentation was with a palpable breast lump. Pre-operative mammography was performed in

Ductal carcinoma in situ (DCIS) and lobular carcinoma in situ (LCIS) represent a proliferation of malignant epithelial cells within the ducts of the breast without invasion through the basement membrane. It is assumed that all invasive carcinomas of the breast are preceded by DCIS or LCIS. However, it is not known what proportion of DCIS or LCIS--if left untreated--will progress to invasive carcinoma. Standard treatment for DCIS is breast-conserving surgery with free margins and radiation therapy. LCIS is usually treated by observation alone.

A series of 87 cases of carcinoma in situ of the breast was reviewed. IN 49 CASes in which lobular carcinoma in situ was shown on biopsy, three patients were found to have invasive carcinoma in the subsequently done mastectomy. All three of these cases were in a group of 14 patients shown in have bilateral lobular carcinoma in situ on biopsy. In a group of 38 patients with intraductal carcinoma on biopsy, seven were found to have invasive carcinoma in the mastectomy that was subsequently done. Lymph node metastases were found in one patient in the lobular group and four patients in the intradu

Mammographic screening has resulted in an epidemic of diagnoses of ductal carcinoma in situ (DCIS). These are often small, impalpable lesions that do not reflect the natural history of older series of DCIS found by palpation at a larger size. The behavior of DCIS is related to the histological grade and size of the tumor. Therapy is primarily surgical excision. Irradiation has been shown to halve the rate of recurrence after local excision. Therapeutic decisions involving the extent of surgical excision and the addition of irradiation are based on considerations of tumor size and grade and the

BACKGROUND: Breast ductal cancer in situ (DCIS) can recur or progress to invasive ductal cancer (IDC), and the interim stage include DCIS with microinvasion (DCIS-Mi). In this article, we attempt to study the study the differences of clinicopathological features, imaging data, and immunohistochemical-based subtypes among DCIS, DCIS-Mi, and IDC.
METHODS: In this retrospective study, we attempt to compare the clinicopathological features, immunohistochemical results and imaging data of 866 patients (included 73 DCIS, 72 DCIS-Mi, and 721 IDC).
RESULTS: Patients with DCIS and DCIS-Mi were younge

The incidence of breast carcinoma in situ is increasing, yet poorly understood. Diagnosis and management of two types of carcinoma in situ are addressed. Nurses are in key positions to educate and support patients for informed decisions about their treatment.

In reference to a series of 36 in situ breast carcinomas, the current therapeutic possibilities are considered: 8 lobular carcinomas in situ (LCIS) and 28 ductal carcinomas in situ (DCIS) were diagnosed between January 1985 and July 1988. These are infraclinical forms in 80% of the cases, representing 20% of the breast carcinomas treated during the same period of time. The natural history of carcinomas in situ is presented; LCIS rarely evolve toward an invasive carcinoma and they should not be considered as risk factors. DCIS evolve, after biopsy, toward an infiltrating ductal carcinoma in 26

The diagnosis of the lobular carcinoma in situ (LCIS), whose frequency is estimated to range from 0.8 to 3.8 p. cent of breast cancers on the whole, is exclusively anatomopathological since it does not have any specific clinical and/or radiological characteristics. After describing the two main differential diagnoses, the various possible treatments are studied, bearing in mind that the therapeutic strategy must take into account the three characteristics which are typical of LCIS: multicentricity, bilaterality and the possible occurrence of an invasive cancer.

Two cases of carcinoma in situ of the male breast are described: in one patient it arose as a complication of gynecomastia; in the other, it occurred 'de novo' and proceeded to infiltration with widespread metastases. We reviewed 233 cases diagnosed clinically and histopathologically as gynecomastia to see if we could discover other instances of malignant transformation but none was found. Thirty-two cases of infiltrating male breast cancer were also examined. In 11 there was associated intraduct change. In only one of the 32 cases was there evidence of associated microscopic gynecomastia.

Certain preinvasive stages precede invasive carcinoma of the breast. Our knowledge about diagnostics, development and prognosis of such preinvasive forms is poor, for it is not possible to observe them directly and continuously. Analysis of the fine needle aspirates of carcinomata in situ of the breast show suspicious apocrine metaplastic cells with nucleoli in 92% and foam cells in 75%, being in contrast to cytological findings on benign lesions. Those peculiarities are more obvious with CLIS than with intraductal CIS lesions. They can be interpreted as particular signs of increased cell prol

LCIS was first described in 1941 as a distinct pathologic entity by Foote and Stewart who called it a "rare form of mammary carcinoma." It is thought to represent a transitional intra-epithelial, or in situ, stage in the evolution of breast cancer from hyperplastic breast epithelium. With the wide application of mammography, its detection has increased in recent years, being found in approximately 1% of all breast biopsy specimens and 5% of all breast malignancies. Its true incidence is unknown, because the absence of any clinical or radiographic manifestations makes its detection completely a

Lobular carcinoma in situ (LCIS) of the breast is commonly identified as an incidental finding in breast biopsies performed because of either a mammographic abnormality or a palpable mass. Although long recognized as an entity, the significance and optimal treatment of LCIS remains controversial. Initially regarded as a pre-invasive form of breast cancer analogous to ductal carcinoma in situ (DCIS), LCIS was treated by mastectomy. As evidence mounted for an equal risk of invasive carcinoma in both breasts, bilateral mastectomy was advocated by some. More recent studies suggest that LCIS is a m

Lobular carcinoma in situ of the breast is a well defined pathologic entity which is found in about 2.5 per cent of all specimens of the breast taken for biopsy and most commonly occurs in premenopausal females. Its diagnosis is virtually always incidental due to the absence of any clinical indication of its presence. This lesion carries a significant risk for development of subsequent invasive carcinoma which applies equally to both breasts and which appears to increase with time. The appropriate treatment of this disease remains a controversial issue. Various aspects of its epidemiology, pat
