Bretylium tosylate: profile of the only available class III antiarrhythmic agent.

Bretylium tosylate, the only approved class III antiarrhythmic agent, is a unique quaternary ammonium compound with prominent experimental and clinical antifibrillatory effects. Intravenous bretylium causes a biphasic hemodynamic response; initial norepinephrine release is followed by sympathetic ganglionic blockade. Cardiac output is well maintained. Electrocardiographic intervals are unchanged, and global conduction unchanged or facilitated. With long-term experimental use, proportionate lengthening of ventricular action potential and refractory period occurs. Bretylium is largely eliminated

Influence of ion-pair formation on the pharmacokinetic properties of drugs. Pharmacokinetic interactions of bretylium and hexylsalicylic acid in rabbits.

The simultaneous i.v. administration of equimolar doses of bretylium and hexylsalicylic acid results in an increase in plasma area under the curve value of both substances in comparison with their separate administration. The higher plasma levels of both compounds were associated with a reduced renal excretion and an increased biliary elimination. However, the increase in biliary excretion did not compensate for the reduced elimination of bretylium and hexylsalicylic acid via the kidney. The results presented in this paper give further evidence that ion-pairing in-vivo may result in altered ph

Endotracheal bretylium tosylate in a canine model.

This study was conducted to determine whether bretylium tosylate (BT) is effectively and safely absorbed through the endotracheal route in the canine model. Eleven adult mongrel dogs were anesthetized with pentobarbital, were orally intubated, and had continuous blood pressure and electrocardiographic monitoring. Four dogs received 5 mg/kg BT, three dogs received 10 mg/kg BT, two dogs received 20 mg/kg BT, and two control dogs were given volumes of normal saline equal to those given the 5- and 10-mg/kg groups. Each dog received the same dose of BT both endotracheally and intravenously, but in

Porcine malignant hyperthermia. VI: the effects of bilateral adrenalectomy and pretreatment with bretylium on the halothane-induced response.

The effects of bilateral adrenalectomy, together with the i.v. administration of bretylium tosylate 20 mg kg-1 on halothane-induced malignant hyperthermia (MH), were investigated in six Pietrain pigs. All six animals survived the halothane challenge, and failed to show any signs of increased muscle metabolism. Bilateral adrenalectomy alone prevented halothane-induced MH in three out of four Pietrain pigs, whereas the i.v. administration of bretylium alone protected only one pig out of an additional four animals studied. The results show that complete adrenergic blockade inhibits the susceptibi

Efferent limb of the coronary vasoconstrictor reflex elicited by distension of the descending colon in anesthetized dogs.

Distension of the descending colon elicits reflex coronary vasoconstriction. To study the efferent branch of this reflex, experiments were performed on 5 dogs anesthetized with pentobarbitone. A pouch was formed from the distal 15 cm of the descending colon and distended at constant pressure (54 +/- 8.9 mmHg) with warm Ringer solution from a pressurized bottle. During distensions of the colon, arterial blood pressure and heart rate were kept constant by the withdrawal of blood and pacing of the heart, respectively. Experiments were performed before and after intravenous bretylium tosylate (10

THE PHARMACOLOGY OF NEW ADRENERGIC NEURONE BLOCKING AGENTS, N-P-CYCLOHEXYLBENZYLTROPINIUM DERIVATIVES.

The three N-p-cyclohexylbenzyltropinium derivatives studied gave rise to lasting hypotension after transient stimulation and block of sympathetic ganglia in anaesthetized and conscious animals. The drugs inhibited the contractions of the nictitating membrane and spleen evoked by pre- or postganglionic nerve stimulation. In adrenalectomized animals the pressor effect of tetramethylammonium, the hypertensive response to carotid arterial occlusion and the effect of tyramine on the nictitating membrane were also reduced. The drugs have slight or no parasympathomimetic, parasympatholytic or curare-

Sympatho-endocrine and metabolic responses to exercise under post-ganglionic blockade in rats.

The purpose of this study was to further document the role of locally released norepinephrine (NE) in the control of metabolic and endocrine responses to exercise in rats. Post-ganglionic blockade with bretylium (20 mg.kg-1, i.v.) reduced NE release from sympathetic nerve endings and triggered a compensatory increase in epinephrine (E) release from the adrenal medulla, as reflected by plasma NE and E concentrations at rest and exercise (E/NE ratio = 2.92 +/- 0.53 and 2.48 +/- 0.51 vs 0.62 +/- 0.15 and 1.48 +/- 0.18 in control rats; mean +/- SE). Prophylactic treatment of swine with bretylium for experimental cardiac catheterization.

We evaluated a cryothermic ablation catheter for potential use in treating supraventricular tachycardia by cardiac catheterization in miniature swine. Cardiac catheterization was performed on anesthetized miniature swine by using multiple catheters that were inserted bilaterally by either percutaneous or cutdown techniques into the femoral veins. We performed electrophysiologic tests according to standard atrial and ventricular electrical stimulation protocols. Manipulation of catheters in the tricuspid valve area and right ventricle was an essential part of the study. During an initial study

Bretylium, a Class III Antiarrhythmic, Returns to the Market.

Bretylium, with an extensive pharmacologic and medicinal history, was approved by the United States Food and Drug Administration in 1986 for "short-term prevention and treatment of ventricular fibrillation (VF) and treatment of life-threatening ventricular arrhythmias and ventricular tachycardia (VT) unresponsive to adequate doses of a first-line antiarrhythmic agent, such as lidocaine." The NDA sponsor withdrew bretylium from the market in 2011, largely due to unavailability of raw materials required for its production; prior to this, bretylium was removed from the 2000 ACLS Guidelines algori

Influence of ion-pair-formation on the pharmacokinetic properties of drugs. Part 4: Influence of hexylsalicylic acid on the pharmacokinetics of bretylium.

Based on in vitro results it was found that the pharmacokinetic parameters of the hydrophilic drug bretylium (2) can be influenced by an ion-pair-formation with the lipophilic hexylsalicyclic acid (1). After simultaneous i.v. application of 1 and 2 on rabbits a significant increase of the AUC of 2 was observed. Under these conditions a marked increase of the AUC and the MRT of 1 was also obtained, since the blood levels of 2 are high enough for such an influence. A combined rectal application of 1 and 2 causes an increase of the AUC of 2.

Clinical pharmacokinetics of intravenous and oral bretylium tosylate in survivors of ventricular tachycardia or fibrillation: clinical application of a new assay for bretylium.

We studied 12 patients receiving either chronic oral (p.o.) maintenance bretylium and/or acute intravenous (i.v.) bretylium to evaluate drug efficacy and pharmacokinetics. All patients were survivors of ventricular tachycardia or fibrillation. A new assay for bretylium was applied, and it proved sensitive and reliable. After single intravenous dosing, bretylium was eliminated from serum with a mean rate constant of lambda iv1 = 0.0515 hr-1 and a corresponding elimination half-life of tiv1/2 = 13.5 hr (7 studies), similar to previous results in normals. Total body clearance averaged 428 ml/min,

Effect of noradrenaline, bretylium and cocaine on the blood pressure response to tyramine in the rat.

Tyramine is mainly a pressor agent in the rat under urethane, although the hypertensive effect of tyramine in some experiments is followed by a prolonged increase or depression of blood pressure. Bretylium, in doses up to 10 mg/kg, prolonged the response to tyramine, whereas larger doses depressed or blocked its effect. When the hypertensive effect of tyramine was blocked by bretylium, both noradrenaline and dihydroxyphenylalanine, when slowly infused, were found to restore it. The well-known block by cocaine of the hypertensive response to tyramine could also be reversed by intravenous infusi

[Elimination of organic cations from the eye].

Substances injected into the vitreous cavity leave the eye partly by diffusion into the flowing aqueous, but sometimes also by other mechanisms. To look for such other mechanisms removing organic cations from the rabbit eye, 5 labeled quaternary ammonium compounds were injected intravitreally, mixed with labeled sucrose. The latter leaves essentially by way of the aqueous only. After predetermined times had elapsed the rabbits were killed, and the radioactivity remaining in each eye was measured after combustion of the dried eye bulb. The anticholinergic drug Emepronium disappeared faster than

Studies on the mode of action of bretylium and guanethidine in post-ganglionic sympathetic nerve fibres.

1. The effects of bretylium and guanethidine on the nerve terminal impulse and transmitter release from sympathetic postganglionic nerve terminals in the guinea-pig vas deferens have been studied in vitro using focal extracellular recording. Excitatory junction currents (EJCs) were used as a measure of transmitter release. 2. Both bretylium and guanethidine altered the configuration of the nerve terminal impulse in a manner consistent with their being local anaesthetics. 3. Bretylium (1-3 microM) only completely inhibited transmitter release when impulse propagation in the sympathetic nerve te

Mechanism of the effect of bretylium on the ventricular fibrillation threshold in dogs.

Experiments were performed to determine the importance of sympathetic blockade vs a direct myocardial effect as a mechanism for the antifibrillatory action of bretylium. The ventricular fibrillation (VF) threshold was determined in open-chest, anesthetized dogs by scanning the vulnerable period with either a single electrical stimulus (10 ms) or a train of electrical stimuli (14 pulses, 4 ms, 100 Hz). Using the train-of-pulses technique, the VF threshold increased from 6.8 +/- 0.6 mA to 29.7 +/- 6.4 mA 15 minutes after a 10-mg/kg intravenous bolus of bretylium (p less than 0.001, n = 8). There

Class III antiarrhythmics in overdose. Presenting features and management principles.

Class III (Vaughan-Williams classification) antiarrhythmic drugs prolong the cardiac action potential without affecting depolarisation. The 3 class III drugs currently in general use are amiodarone, sotalol and bretylium. The presenting features of acute toxicity are different for each agent and are, therefore, discussed separately. Several new class III antiarrhythmic agents are under development, including dofetilide and d-sotalol, but specific data on overdoses of these potent class III drugs are not yet available. Amiodarone toxicity following acute overdose is rare because poor bioavailab
