The premature neonate is at high risk for childhood airway hyperreactivity and episodes of wheezing. Intermittent hypoxic events are frequently observed during the first weeks and months of life in these infants. Intermittent hypoxemia has been associated with adverse outcomes in extremely premature infants; including the diagnosis of bronchopulmonary dysplasia, reported wheezing, and use of prescription asthma medications. We review the incidence of intermittent hypoxia, their potential role in short and longer term respiratory morbidity, and the translational newborn models now being used to

BACKGROUND: Bronchial hyperreactivity (BHR) is a characteristic of bronchial asthma. Patients with allergic rhinitis who do not report symptoms of bronchial asthma on spirometry show BHR, which could indicate the presence of subclinical inflammation of the lower respiratory airway. The aim of this study was to investigate whether the patients diagnosed with allergic rhinitis in our unit without symptoms of bronchial asthma had bronchial hyperreactivity and to determine which allergens caused these symptoms in our patients.
METHODS AND RESULTS: We performed a retrospective, observational study

In patients with obstructive sleep apnoea (OSA) nCPAP may irritate the mucous membranes of the upper airways. We investigated in this study whether nCPAP can induce bronchial hyperreactivity (BHR). Forty-one patients (33 men, mean age 52.6 years) were treated with nCPAP due to OSA. All of them were tested for BHR with histamine ("pari-provo-Test") before and six weeks after initiation of the nCPAP therapy. Thirty-five of the patients showed BHR neither before nor after the beginning of CPAP. Six patients developed a BHR of moderate degree (PD20: 50-100 micrograms) during the study; four of the

BACKGROUND: Bronchial asthma is a heterogeneous airway disease primarily diagnosed through clinical evaluation and pulmonary function testing. Although imaging plays only a secondary role in the initial diagnostic process, it provides clinically relevant complementary information to support the differential diagnosis, document structural abnormalities, and detect complications.
METHODS/IMAGING TECHNIQUES: The imaging spectrum includes conventional chest radiography, high-resolution computed tomography (CT), and complementary magnetic resonance imaging (MRI) and functional techniques, enabling

OBJECTIVE: To investigate the bronchial hyperreactivity (BHR) and pulmonary function in patients suffering from endobronchial tuberculosis (EBTB), and therefore to provide clues for early and correct diagnosis of EBTB.
METHODS: Twenty-four patients with EBTB, proved by fiberoptic bronchoscopy, chest computerized tomography and biopsy, received measurements of pulmonary ventilatory function, airway resistance (Raw), and bronchial responsiveness test. The results were analyzed by t test and correlations.
RESULTS: As many as 41.7% of the patients with EBTB were found to have BHR, which had neve

Bronchial hyperreactivity is a common report in several respiratory tract diseases such as bronchial asthma, chronic bronchitis and cystic fibrosis. Atopic patients and children who suffered lung diseases in early life (laryngotracheobronchitis, bronchiolitis, bronchopulmonary dysplasia) may have also persistent abnormalities in pulmonary function that frequently include bronchial hyperreactivity. A review of the actual literature, specially concerning with the clinical aspects of bronchial hyperreactivity in children and with the possible etiopathogenetic mechanism is presented.

The airways of asthmatics are hyperreactive, not only to allergens but to a wide range of non-specific physical and chemical stimuli. Four mechanisms which have been proposed to be causes of hyperreactivity are (i) a decrease in baseline airway calibre (ii) an increase in the responsiveness of the bronchial smooth muscle (iii) an imbalance in the autonomic neurophysiological regulation of airway calibre and (iv) epithelial changes which lead to an increase in the accessibility of allergens and non-specific stimuli to the mast cells, sensory nerve endings or bronchial smooth muscle beneath the

Airways of patients with bronchial hyperreactivity (BHR) are characterised by exaggerated bronchoconstriction in response to a variety of stimuli; bronchospasm may be elicited during induction and maintenance of anaesthesia. The prevalence of BHR in normal populations is approximately 10%. BHR is an important feature of clinical asthma, chronic obstructive pulmonary disease (COPD), allergic rhinitis, atopy, upper respiratory tract infections and smoking. This review will outline some important aspects of the pathophysiological basis of BHR, i.e., neural and inflammatory mechanisms. Furthermore

We examined the effect of pyridostigmine (PY) at a dose of 30 mg orally three times a day on nonspecific bronchial hyperreactivity in ten normal nonsmokers (NNS), ten smokers (SM), and ten mild asthmatics (AS). We conducted a double-blind, placebo-controlled, crossover trial, randomly assigning subjects to receive either placebo (PL) or PY before undergoing bronchoprovocation challenge with eucapnic voluntary hyperventilation (EVH) using dry gas. Compliance with PY was confirmed by measuring red blood cell acetylcholinesterase (Achase) levels during both days of testing. While taking PL, the m

Aspecific bronchial hyperreactivity (A.B.H.) is a condition found mostly in cases of bronchial asthma, but it may also present in other pathologies affecting the respiratory tract. This study examines 30 patients presenting hilo-pulmonary sarcoidosis, 27 asthmatic patients and 25 healthy subjects as controls. The three groups were sex and age matched. No significant difference in smoking habits between controls and sarcoidosis patients was found. Patients were tested for aspecific bronchial hyperreactivity by administering increasing doses of methacholine. Methacholine inhalation was performed

The interaction of air pollutants and their effect on bronchial hyperreactivity show that sulfur dioxide and ozone both increase bronchial hyperreactivity. Sulfur dioxide concentrates as low as 0.5 ppm and ozone concentrations as low as 0.2 ppm have demonstrated this effect. Exercise with either sulfur dioxide or ozone exposure can significantly increase bronchial hyperreactivity in susceptible individuals. Other factors including ambient air temperature, time of day, wind velocity and direction, geography, climate, tobacco smoke exposure, and other indoor air pollutants may enhance the impact

Hindrance to gas flow in the bronchi is affected not only by airway smooth muscle tone but also by airway circulation. Congestion and oedema increase airway wall thickness and act in series with airway smooth muscle contraction to reduce airway calibre, an effect which is more marked in small and intermediate bronchi. Many mediators, neuromediators, paracrine mediators produced by resident (epithelium) or migrant (inflammatory cells) cells share bronchomotor and vascular effects. In addition, contraction of airway smooth muscle and vascular phenomena are mechanically coupled. Contraction of ai

In a 37-year-old woman, a heavy smoker and an alcoholic, bronchial hyperreactivity to histamine (PC20 FEV1 0.8 mg/ml) was related to hypomagnesemia (0.55 mmol/l). After acute magnesium repletion (24 h i.v. infusion MgSO4 6 g, serum magnesium 1.05 mmol/l), histamine PC20 FEV1 increased up to 9.8 mg/ml. The role of magnesium in modulating smooth muscle contractility is discussed and the importance of checking serum magnesium in patients with airway obstruction is suggested.

Bronchial hyperresponsiveness (BHR), the exaggerated airway narrowing in response to nonspesific stimuli, is a common characteristic of asthma. One hundred thirty-five children who were diagnosed asthma in the outpatient clinic of Gazi University Hospital, Pediatric Allergy and Asthma Department between January 2007 and January 2008 were retrospectively analysed from the asthma database of the division. BHR was not found to be different according to sex. Younger the age of the patient, more severe was BHR (p = 0.096, r = 0.164). Younger the age at onset of the symptoms, the more severe was the

BACKGROUND: Antibody deficiency comprises a heterogeneous group of disorders characterised by the body's inability to mount an effective antibody response to pathogens. Although it has been reported that asthma and allergic disease are frequent in antibody deficiencies, there are no data that evaluate and compare bronchial hyperreactivity (BHR) in all groups of antibody deficiencies. In this study, we aimed to evaluate and compare the frequency of BHR in patients with different antibody deficiencies.
METHODS: The study was carried out on 113 patients between ages 5 and 18 diagnosed with antib

Prevalence was studied of asthma and wheezing among children residing in Kiev 13 to 14 and 6 to 7 years old, with n = 400 in each of the above age groups using standardized ISAAC protocols. Wheezing was detected in 25.5% of children in the 13-14 group and in 29.3% of those in the 6-7 group. 93.2 percent were children catching frequently recurrent respiratory 30.7% of children ages 6-7 and 31.3% of those 13 to 14 years old had asthma, i.e., one-third of the children with wheezing, with bronchial hyperresponsiveness having been recorded in 56.6 percent of the 6-7 age group and in 70.2 percent of

Marfan syndrome is known to have pulmonary manifestations such as pneumothorax. There have been few previous studies of pulmonary function tests and none of bronchial hyperreactivity. Therefore, pulmonary function tests were performed in 11 children with Marfan syndrome and 11 normal children. Bronchial responsiveness was tested in ten of the Marfan patients by methacholine challenge test and response to bronchodilator. Because of disproportionate length of legs in Marfan patients, an "ideal" standing height was calculated from sitting height. Pulmonary function tests, as absolute values or as

Airway hyperreactivity is a consistent finding in most children with asthma and is responsible for many clinical manifestations of asthma. BH in children has been elicited by bronchial challenge with methacholine and histamine, cold air (isocapnic hyperventilation), and exercise. Pharmacologic agents employed in therapy of childhood asthma, which may modulate both the late asthmatic reaction and bronchial hyperreactivity, include cromolyn sodium, glucocorticoids, and possibly ketotifen. Beta adrenergic agonists, though effectively blocking the early asthmatic response, had no effect on the lat

11 children with bronchial hyperreactivity were tested for Legionella pneumophila serotype 1 and 2-14, including 3 girls, who were treated a year before because of atypical pneumonia, probably caused by Legionella pneumophila, 2 brothers of the girls and 6 children from a different village as well as 5 adults--parents of the girls. In all of them a significant level of antibodies against Legionella pneumophila serotype 2-14 was detected with indirect immunofluorescence. One of previously treated girls presented with increased level of IgG antibodies (ELISA), the remaining two had increased lev
