Hallux valgus deformity is a complex chronic progressive disease primarily characterized by a lateral great toe deviation and deformity of the first metatarsophalageal joint. Numerous etiological factors are related with the expression of this disease, and they are divided into two categories: endogenous and exogenous. Complexity of the hallux valgus deformity is reflected with the progression of the disease that gives rise to numerous forefoot deformities. The diagnosis is first of all affirmed by clinical examination and x-ray of the feet in a standing position. Treatment could be either ope

In seven cases of Lisfranc joint injury after trauma, bunion deformity developed. This "traumatic bunion" occurs over a prolonged period of time after injury. A high index of suspicion is needed to identify the deformity as being traumatic in origin. Injury about the first metatarsophalangeal joint complex may also contribute to this deformity. When recognized, it may need to be treated with a first metatarsal-cuneiform fusion and distal soft tissue realignment.

The management of bunion deformities in adolescent patients is often a source of consternation for orthopaedic surgeons. Reports of recurrence and surgical failure along with a multitude of procedures to choose from create a wariness to manage the problem surgically. The biggest challenge in managing this problem is a lack of understanding by orthopaedic surgeons that adolescent bunions and adult bunions frequently arise from two distinct etiologies. The main difference between the two is that unlike adult bunion deformities, the hallux metatarsophalangeal joint in the adolescent bunion is con

Although dancers put a great deal of stress through the first metatarsophalangeal joint (MTPJ), it is unlikely that dancing causes bunions; however, such forces may produce an environment in which bunions may develop. It is best to employ conservative measures rather than surgical intervention in dancers who have a painful bunion. Any surgery on the first MTPJ will adversely affect dorsiflexion of this joint, which is a critical motion for dancers. Two types of bunions (slowly progressive and rapidly progressive) are commonly seen; arthritic bunions occur in dancers who have mild arthrosis and

The tailor's bunion is a painful bony prominence on the lateral aspect of the fifth metatarsal head that occurs in many individuals, but seldom causes symptoms. This article reviews the current literature regarding the presentation, etiology, and management of the tailor's bunion. The first line of management should be conservative, with advice on shoe wear. Orthotics may be useful if a symptomatic tailor's bunion results from excessive subtalar joint pronation. Operative management, indicated when symptoms are not controlled nonoperatively, aims to decrease foot width and the prominence of th

BACKGROUND: The operative management of the juvenile bunion has not enjoyed the same level of success as its adult counterpart leading to the concern that we do not fully understand what differentiates these 2 deformities. This study aims to (1) characterize the radiographic and pedobarographic features commonly encountered in the juvenile bunion and (2) determine which of these radiographic or pedobarographic parameters correlate with patient-reported outcome (PRO) scores at the time of presentation.
METHODS: An IRB-approved retrospective analysis of prospectively enrolled patients between 1

Hallux valgus is a common condition that results in lateral deviation of the hallux and medial deviation of the metatarsal. When conservative management fails, surgical management is often necessary. More than 150 procedures have been described, and most recommend an open approach. More recently, a minimally invasive approach to bunion correction has gained popularity among surgeons. The authors have been performing a minimally invasive percutaneous method of bunion correction with positive outcomes. This article presents case examples and a systematic approach for correction of this common fo

As theatre practitioners we are on our feet for long periods of time. How many of us can truthfully say that we treat our feet with the respect that they deserve, for example, wearing the correct footwear and generally taking good care of our feet? It is established that theatre clogs, while practical, are not a fashion statement. Most of us probably take our feet for granted--that is until something becomes painful or a deformity occurs. One common and often painful deformity of the foot is a bunion. This article describes how bunions occur and explores the surgical options.

While the surgeon may tend to use one procedure in the repair of a hallux valgus deformity, versatility is most important when treating the juvenile bunion. Using a distal soft-tissue repair when subluxation is solely at the metatarsophalangeal joint is an acceptable approach. A metatarsal or cuneiform osteotomy is necessary if the intermetatarsal angle is abnormally large. It is important not to stretch the indications for a bunion technique in order to correct the hallux valgus deformity. If a more severe deformity is present, a more aggressive technique must be used to correct the abnormali

Iatrogenic dorsal bunions in children, observed mostly after operations for post-poliomyelitic paralysis or club foot, have been successfully treated by an operation in which the tendons of the flexor brevis and the abductor and adductor muscles of the hallux are transferred to the neck of the first metatarsal. This is supplemented by removal of the sesamoids, capsulotomy of the metatarsophalangeal joint, and when necessary by arthrodesis or tenodesis of the metatarsophalangeal joint. In seventeen feet treated in this way only one failure, in a patient with arthrogryposis, was recorded.

The authors present a review of the literature on dorsal bunion and describe its classification, etiology, predisposing factors, and different methods of treatment. The deformity is functionally limiting during ambulation and cosmetically undesirable for the patient. It is a progressively disabling deformity that requires surgical management.

Tailor's bunion or bunionette are terms that describe a pathologic enlargement occurring laterally on the fifth metatarsophalangeal joint. Regardless of the etiology that precipitates the deformity, the resulting abnormal protrusion of soft tissue or bone can result in pain for the patient. Symptoms can range from mild discomfort to severe, debilitating pain. The patient may present with pain dorsolaterally, laterally, or plantarly. The symptoms are mechanically induced, and are often associated with hyperkeratotic lesions and adventitious bursae. Patients complain most often that they cannot

The goal of fine-tuning bunion surgery is to optimize outcomes and prevent complications. This is accomplished through restoring anatomic alignment, imparting first ray stability, meticulous surgical technique, and accounting for other causes that may contribute to first ray instability. Despite various soft tissue and osseous surgical procedures along with anatomic variations of each patient, the principles of anatomic restoration and stability remain consistent. Maintenance of correction is predicated on the treatment of underlying pathology and the establishment of optimal stability and fir

Brachymetatarsia is caused by premature closure of the physis and is characterized by a short metatarsal. Additional foot conditions may exist in patients presenting with brachymetatarsia, such as hallux valgus (HV). A retrospective study was performed to evaluate the prevalence of HV and brachymetatarsia in the ipsilateral foot. Ninety-seven feet with congenital brachymetatarsia were reviewed in a multi-study cohort of 66 patients who underwent surgical correction between January 2005 and August 2020 at a single institution. The group was comprised of 61 females and 5 males, with a mean age o

The results of three different surgical procedures for the correction of bunions on 25 adolescent feet (distal soft-tissue procedure, proximal closing-wedge osteotomy of the first metatarsal with distal soft-tissue procedure, and proximal phalangeal osteotomy) are reviewed. The results with an average follow-up of 3.5 years were excellent or good in 21 cases, and fair or poor in 4. The average correction of the metatarsophalangeal angle, the intermetatarsal angle, and the hallux valgus interphalangeus angle was, respectively, 9.88, 3.11, and 0.0 degrees for the first procedure, 21.0, 6.87, and

Fifty-one cases of moderate to severe bunion deformity with hallux valgus and metatarsus primus varus in 43 patients were treated by bunionectomy, proximal Chevron metatarsal osteotomy, lateral capsulotomy, adductor tenotomy, and lashing of first and second metatarsals together. The hallux valgus angle improved an average of 19 degrees from 33 degrees (mean) preoperatively to 14 degrees (mean) postoperatively. The intermetatarsal angle improved an average of 7.3 degrees from an average of 14 degrees preoperatively to an average of 6 degrees postoperatively. The position of the sesamoids was re
