The effect of calcium sulfate/calcium phosphate composite for the treatment of chronic osteomyelitis compared with calcium sulfate.

BACKGROUND: Treatment of chronic osteomyelitis often requires surgical debridement in combination with bone defect reconstruction and antibiotics administration. We aimed at investigating and evaluating the effect of antibiotic-loaded absorbable calcium sulfate/calcium phosphate (CS/CP) composite as bone substitute in the treatment of chronic osteomyelitis compared with CS. METHODS: A retrospective study of 31 consecutive patients with chronic osteomyelitis from one medical center was conducted. The treatment involved thorough debridement, antibiotic loaded bone substitutes filling (group A:

Calcium sulfates: what is the evidence?

Calcium sulphate has been used extensively as a bioabsorbable bone substitute for 90 years. Its advantages include its low cost, ready availability and unlimited supply, lack of donor site morbidity, use as a delivery vehicle for other compounds (especially antibiotics), inherent osteoconductive properties (based on a structure similar to bone), and its proven safety record. We sought to determine the evidence-based medical studies (prospective and/or randomized clinical trials) that support the use of calcium sulphate as a bioabsorbable bone substitute. At the present time, the majority of re

The kinetics of remodeling of a calcium sulfate/calcium phosphate bioceramic.

In the last years considerable research and development activity have been expended to find new ceramic bone substitutes for the treatment of bone defects. However in many cases the drawback of synthethic bone substitutes are the slow graft incorporation and remodelling into the host bone. The purpose of this study was to analyze the kinetics of resorption and new bone formation of new calcium sulfate (CaSO4)/calcium phosphate (CaPO4) bioceramic engineered to enhance its bone forming properties. We prospectively evaluated the results of a series of 15 hips with osteonecrosis of the femoral hea

Regeneration of bone in the presence of calcium sulfate.

I describe 110 patients in whom diseased and destructed bone was restored to normal healthy bone. The first ten patients were healed by using bismuth-iodoform, and the following 100 patients were treated using the hemihydrate of calcium sulfate (CaSO4.2H3O). Both alloplasts were replaced in all patients by normal healthy cancellous bone within two to four months. It is a safe procedure in which bone regeneration is stimulated to a maximum rate. During June 1974, bismuth-iodoform was used as an allograft. On July 9, 1974, the first three operations in the history of otorhinolaryngology, to my k

Calcium sulfate dihydrate urolithiasis in a pet rabbit.

CASE DESCRIPTION A 3-year-old sexually intact male rabbit (Oryctolagus cuniculus) was evaluated because of a 1-day history of signs of anorexia and depression. CLINICAL FINDINGS Clinical examination revealed signs of depression, hunched posture, low skin elasticity (suggesting dehydration), slightly distended abdomen, and penile and preputial edema. The owner reported that the rabbit had been fed a routine diet, received water via a sipper bottle, and was allowed free movement around the home. It had been observed by the owner to bite and chew gypsum-based plaster from the walls of the home. A

PROLONGED HYPERCALCEMIA FROM ANTIBIOTIC-ELUTING CALCIUM SULFATE BEADS.

OBJECTIVE: Calcium sulfate beads (CSBs) are biocompatible hydrophilic crystals that are used to deliver local antibiotics in periprosthetic joint infections. Hypercalcemia after placement of CSBs is uncommon and poorly understood. METHODS: We present the case of a woman who presented with symptomatic hypercalcemia after placement of antibiotic-eluting CSBs. RESULTS: A 58-year-old, Caucasian woman presented with altered mental status, respiratory failure, and septic shock 2 days after placement of antibiotic-eluting CSBs for a left prosthetic hip infection. Laboratory analysis revealed severe

Availability of calcium from skim milk, calcium sulfate and calcium carbonate for bone mineralization in pigs.

Dairy products provide abundant, accessible calcium for humans, while some calcium sulfate-rich mineral waters could provide appreciable amounts of calcium. But there is little evidence that this calcium is as available as milk calcium for making bone. The availability of calcium was studied by monitoring bone parameters in 2-month-old pigs fed restricted amounts of calcium (70% RDA) for 2.5 months. The 3 main (> or = 50% Ca intake) Ca sources were either CaCO3 or CaSO4 or skim milk powder (29% of the diet). The bones of the pigs fed the "milk" diet had higher (P < 0.01) ash contents, breaking

Acute Hypercalcemia Resulting From Antibiotic-Impregnated Calcium Sulfate Beads.

An 86-year-old man with a history of chronic kidney disease sustained a hip fracture after a mechanical fall, which eventually led to a total hip arthroplasty (THA). During this THA, he was found to have a prosthetic joint infection and underwent debridement, antibiotics, and implant retention, where calcium sulfate beads (CSBs) were placed. He subsequently developed hypercalcemia, requiring intravenous fluids, diuretics, and calcitonin for treatment. This case highlights the development of hypercalcemia after the placement of CSBs, which is a known, although uncommon, side effect.

Calcium sulfate cement in contained traumatic metaphyseal bone defects.

The aim of this study was to evaluate prospective patients with periarticular fractures where a meta physeal bone defect was grafted with high compressive calcium sulfate cement. The calcium sulfate cement MIIG X3, (Wright Medical Technology, Inc, Arlington, TN) was used in 45 patients with periarticular fractures--distal radial, tibial plateau, humeral head, and calcaneal fractures--to fill the metaphyseal defect. All fractures were treated either with open or closed reduction, fracture fixation, and the cement was applied openly or closed. Radiographs were evaluated for fracture reduction, j

Titanium mesh-reinforced calcium sulfate for structural bone grafts.

Calcium sulfate (CS) possesses many of the requirements for an ideal bone graft material: it is biodegradable, biocompatible, and osteoconductive. However, its relatively low strength and brittleness are major obstacles to its use as a structural bone implant. Although the strength of CS can be improved by reducing porosity, its brittleness remains a major obstacle towards its use as bone graft. Here we combine two powerful toughening strategies which are found in advanced ceramics and in natural bone: Multi-layered architectures and ductile reinforcements. We first used stress analysis and mi

Calcium carbonate depresses iron bioavailability in rats more than calcium sulfate or sodium carbonate.

Calcium carbonate supplements depress iron bioavailability when consumed with meals. Our objective was to determine whether this effect is due to the calcium, the carbonate or a combination of the two. A rat hemoglobin repletion assay and an in vitro digestion procedure were used to assess the effects of four salts (calcium carbonate, calcium sulfate, sodium carbonate and sodium sulfate) on iron bioavailability. The salts were added to purified rat diets (for the hemoglobin repletion study) or to iron-fortified infant formula (for the in vitro study) at three different levels. Calcium carbonat

Calcium sulfate matrix as local antibiotic carrier in the mastoid.

We describe the use of calcium sulfate beads as antibiotic carrier in a patient, who suffered from chronic mastoiditis with consecutive otogenic meningitis due to Burkholderia cenocepacia. Our findings suggest a possible role of calcium sulfate matrix as a local antibiotic carrier in the mastoid in complicated mastoiditis cases.

Sulfation and attrition of calcium sorbent in a bubbling fluidized bed.

A bubbling fluidized bed reactor was used as a desulfurization apparatus in this study. The height of the bed was 2.5m, and the inner diameter was 9cm. The bed materials were calcium sorbent and silica sand. The effects of the operating parameters of the flue gas desulfurization including relative humidity, temperature, superficial gas velocity, and the particle size of calcium sorbent on SO2 removal efficiency and calcium sorbent conversion and attrition rate in the fluidized bed were investigated. It was found that the temperature effect in our system was negligible from 40 to 65 degrees C.

Fixation of comminuted distal radius fractures with a mixture of calcium phosphate and calcium sulfate cement.

Distal radius fracture alignment and stabilization can be a surgical challenge in the face of severe comminution and bone loss. We describe a technique using a calcium phosphate/sulfate bone cement, as an adjunct to internal fixation. This bone cement is biocompatible, osteoconductive, and sets quickly with an isothermic reaction. The use of bone cement eliminates the need for primary autologous bone grafting and allows for easier reduction and retention of reduction at the time of surgery. Bone cement is employed for the following purposes in comminuted fractures: (1) to fill a void due to lo

Vancomycin-laden calcium phosphate-calcium sulfate composite allows bone formation in a rat infection model.

OBJECTIVE: Local antibiotic delivery systems with differing chemical and mechanical properties have been developed to assist in the management of osteomyelitis. We investigated the bone conductive and resorptive capabilities of a calcium phosphate-calcium sulfate (CaP/CaS) composite compared with commercially available polymethylmethacrylate (PMMA). In addition, we compared the in vivo preventative and treatment efficacies of both biomaterials in a proven osteomyelitis model. METHODS: Sixty-four, male Sprague-Dawley rats were inoculated with 10 μl of 1.5 x 108 CFU/ml of Staphylococcus aureus

Antibiotic-impregnated calcium sulfate use in combat-related open fractures.

This article presents our experience with the use of antibiotic-impregnated calcium sulfate in the management of comminuted open fractures with a bony defect caused by combat-related blast injuries and high-energy wounds. Calcium sulfate was used 19 times in 15 patients (17 fractures) as a bone graft substitute and a carrier for antibiotics. The anatomic sites of the graft were as follows: 6 calcanei, 1 midfoot, 1 metatarsal, 5 tibiae, 3 femorae, and 1 humerus. The average number of procedures prior to grafting was 6.2 (range, 2-10; median, 6) with grafting performed at an average 28 days afte

Urinary magnesium, calcium, and phosphate excretion during magnesium sulfate infusion.

Urinary excretion of magnesium, calcium, and phosphate was measured in 20 preeclamptic pregnant women treated with intravenous magnesium sulfate. Blood levels of these substances, as well as of parathyroid hormone and calcitonin, were also measured. Urinary magnesium excretion increased 20-fold during magnesium sulfate infusion. Seventy-five percent of the infused magnesium was excreted during the infusion, and 90% within 24 hours after treatment. Urinary calcium concentration increased 4.5-fold during magnesium sulfate infusion, and the urinary calcium excretion rate was 3 times that observed
