[Invasive candidiasis].

Otosclerosis is characterized by pathological remodeling of the bone of the otic capsule. Classically, foci of pathological bone leads to progressive fixation of the stapes, resulting in conductive hearing loss. Involvement can be uni- or bilateral. Otoscopy is normal. -Hypoacusis generally appears between the age of 30 to 50, regularly with tinnitus. Sometimes, the inner ear is also affected, with sensori-neural hearing loss or vestibular impairment (vertigo). Successful hearing rehabilitation can be achieved with hearing aids or stapes surgery (stapedotomy). Cochlear implantation is consider

Invasive Candidiasis.

Invasive candidiasis (IC) is the most frequent health care associated invasive fungal infection. It is also associated with high morbidity, mortality, and cost. The most frequent etiologic agent is Candida albicans, but non-albicans species are increasing and associated with reduced antifungal susceptibility and outbreaks. Candida auris is an emerging multidrug-resistant species recently described. IC presents as a spectrum of disease, going from fungemia to deep-seated candidiasis, and to septic shock with multiorgan failure. Diagnosis of IC is challenging. Several biomarkers and molecular me

Invasive Candidiasis.

Invasive candidiasis (IC) is a term that refers to a group of infectious syndromes caused by a variety of Candida species, 6 of which cause the vast majority of cases globally. Candidemia is probably the most commonly recognized syndrome associated with IC; however, Candida species can cause invasive infection of any organ, especially visceral organs, vasculature, bones and joints, eyes, and central nervous system. The optimal use of these newer diagnostics coupled with a thoughtful clinical assessment of at-risk patients and the judicious use of effective antifungal therapy is a key to achiev

Invasive candidiasis.

Invasive candidiasis is an important fungal disease caused by Candida albicans and, increasingly, non-albicans Candida pathogens. Invasive Candida infections originate most frequently from endogenous human reservoirs and are triggered by impaired host defences. Signs and symptoms of invasive candidiasis are non-specific; candidaemia is the most diagnosed manifestation, with disseminated candidiasis affecting single or multiple organs. Diagnosis poses many challenges, and conventional culture techniques are frequently supplemented by non-culture-based assays. The attributable mortality from can

Invasive candidiasis.

Invasive candidiasis remains an important nosocomial infection that continues to present major diagnostic and therapeutic challenges to the clinician. Changes in the epidemiology of this disorder have occurred for many reasons, and included especially the extensive use of prophylactic antifungal agents, broad-spectrum antibacterial agents, and medical devices (eg, chronic indwelling intravascular catheters). The diagnosis of IC remains elusive in many patients, and there is a critical need for improved diagnostics that will provide clinicians the opportunity to intervene earlier in the course

Neonatal invasive candidiasis.

Over the last two decades, systemic fungal infections have emerged to play a primary role in hospital-acquired infections. C. albicans is involved in 75% of neonatal candidiasis; however, the incidence of infection from C. parapsilosis is also increasing significantly. The higher incidence observed in the high-risk group of very low birth weight (VLBW) infants is linked to their special physical characteristics and the diagnostic and therapeutic invasive procedures they undergo. Colonization is a relevant risk factor depending on the colonized site , the fungal species and the type of coloniza

Epidemiology of Invasive Candidiasis.

Invasive candidiasis (IC) is an increasingly prevalent, costly, and potentially fatal infection brought on by the opportunistic yeast, Candida. Previously, IC has predominantly been caused by C. albicans which is often drug susceptible. There has been a global trend towards decreasing rates of infection secondary to C. albicans and a rise in non-albicans species with a corresponding increase in drug resistance creating treatment challenges. With advances in management of malignancies, there has also been an increase in the population at risk from IC along with a corresponding increase in incid

Invasive candidiasis during granulocytopenia.

Candidiasis remains the most frequently encountered fungal infection in patients with profound granulocytopenia and appears to be increasing in frequency. In addition, Candida infections are occurring earlier during remission induction chemotherapy and can be caused by a variety of species such as C. albicans, C. tropicalis, and C. krusei. The most frequent source of disseminated infection is the gastrointestinal tract, as the integrity of the epithelium is disrupted by chemotherapeutic agents. The spectrum of disseminated candidiasis comprises both an acute and a chronic presentation (also kn

Neonatal Invasive Candidiasis: Current Concepts.

Neonatal invasive candidiasis (NIC) is an important cause of morbidity and mortality, especially in preterm and very low birthweight infants, with the incidence being inversely correlated with gestational age and birth weight. Epidemiological studies in high-income countries show Candida albicans and Candida parapsilosis being the commonest species causing NIC, while in low- and middle-income countries NIC is more often caused by non-albicans Candida species with higher rates of antifungal resistance. The increased incidence of fluconazole-resistant C. parapsilosis and multi-drug-resistant Can

Invasive neonatal candidiasis: an overview.

Until the 1980s, recovery of Candida species from normally sterile body sites in high-risk infants was often dismissed as contamination. Such delay in diagnosis often resulted in death, multifocal disease, or significant morbidity, outcomes that still occur today. Problems establishing the diagnosis of invasive candidiasis persist, while the frequency of this infection in increasingly fragile and smaller premature infants appears to be increasing. Standard culture techniques have limited sensitivity and often take several days for recovery of Candida when positive. Heightened suspicion and imp

Rezafungin: A Review in Invasive Candidiasis.

Rezafungin (Rezzayo®) is a next-generation echinocandin antifungal with improved pharmacokinetic properties over first-generation echinocandins that allows for once-weekly rather than once-daily intravenous administration. It has recently been approved for the treatment of adults with invasive candidiasis in the EU and UK, and is approved for adults who have limited or no alternative options for the treatment of candidaemia and invasive candidiasis in the USA. In the pivotal phase 3 ReSTORE trial, rezafungin was non-inferior to caspofungin (a first-line echinocandin antifungal agent) based bot

Diagnosis and Treatment of Invasive Candidiasis.

Candida species, belonging to commensal microbial communities in humans, cause opportunistic infections in individuals with impaired immunity. Pathogens encountered in more than 90% cases of invasive candidiasis include C. albicans, C. glabrata, C. krusei, C. tropicalis, and C. parapsilosis. The most frequently diagnosed invasive infection is candidemia. About 50% of candidemia cases result in deep-seated infection due to hematogenous spread. The sensitivity of blood cultures in autopsy-proven invasive candidiasis ranges from 21% to 71%. Non-cultural methods (beta-D-glucan, T2Candida assays),

Treatment and prophylaxis of invasive candidiasis.

Invasive candidiasis (IC) is a leading cause of morbidity and mortality in preterm infants. Even if successfully treated, IC can cause significant neurodevelopmental impairment. Preterm infants are at increased risk for hematogenous Candida meningoencephalitis owing to increased permeability of the blood-brain barrier, so antifungal treatment should have adequate central nervous system penetration. Amphotericin B deoxycholate, lipid preparations of amphotericin B, fluconazole, and micafungin are first-line treatments of IC. Fluconazole prophylaxis reduces the incidence of IC in extremely prema

Anidulafungin for the treatment of invasive candidiasis.

Candidaemia/invasive candidiasis (C/IC) is the most frequently occurring invasive fungal infection worldwide, with a particularly strong impact and high incidence in the intensive-care unit, where there is a need for new treatment options and strategies. The echinocandin anidulafungin has broad in vitro activity against a wide range of Candida species, along with favourable pharmacokinetics that allow administration in hepatic and renal impairment and with any comedication without the need for dose adjustments. The efficacy and safety of anidulafungin for the treatment of C/IC were demonstrate

[Invasive candidiasis after hematopoietic cell transplantation].

Candida species are the second most frequent fungal pathogen of invasive fungal disease after hematopoietic cell transplantation (HCT) following Aspergillus species. Prolonged severe neutropenia and mucocutaneous barrier impairment resulting from the conditioning regimen or central venous catheter placement are major risk factors for invasive candidiasis in the early phase after HCT. Graft-versus-host disease (GVHD) and corticosteroid use affect the development of invasive candidiasis in the post-engraftment phase after allogeneic HCT. Breakthrough candidemia mainly caused by non-albicans Cand
