OBJECTIVES: In the middle to late 2000s, many family physicians switched from a Family Health Group (FHG; a blended fee-for-service model) to a Family Health Organization (FHO; a blended capitation model) in Ontario, Canada. The evidence on the link between physician remuneration schemes and quality of diabetes care is mixed in the literature. We examined whether physicians who switched from the FHG to FHO model provided better care for individuals living with diabetes relative to those who remained in the FHG model.
METHODS: Using longitudinal health administrative data from 2006 to 2016, we

BACKGROUND: With an ageing population, there is an increasing demand for home-based palliative care to improve end-of-life care. Funding models can impact service utilisation and patient outcomes.
AIMS: To compare two funding models to assess the effects on service utilisation, hospital admission, home death rates and concordance between preferred and actual place of death.
METHODS: A single-centre, prospective cohort study compared the first 12 months of two funding models. Eligible patients accessing a private health insurance-funded palliative care programme were included. Each funding mo

OBJECTIVE: We compare case mix of Medicare home health patients under HMO and FFS payment.
STUDY DESIGN: A pseudo-experimental design was employed to study case mix using three types of Medicare-certified home health agencies (HHAs): HMO-owned agencies, pure FFS agencies that admit few Medicare HMO patients (less than 5 percent of admissions are Medicare HMO patients), and mixed (or contractual) agencies that admit at least 15 Medicare FFS patients and 15 Medicare HMO patients per month. SAMPLES OF PROVIDERS AND PATIENTS: Random samples of Medicare-aged patients (> or = 65 years) were selecte

PURPOSE: The demand for specialised eye care has quadrupled over the last two decades, driven by an aging population and the advent of new treatments. This surge has led to increased outsourcing by the Helsinki and Uusimaa Hospital District (HUS) Eye Hospital, initially relying on a fee-for-service model. This model, however, has been marked by excessive bureaucracy and transaction costs. Between 2019 and 2021, HUS Eye Hospital embarked on a comparative analysis of outsourcing models to identify the most cost-effective, efficient and suitable approach.
METHODS: After the review of literature

BACKGROUND: Upcoming alternative payment models Primary Care First (PCF) and Kidney Care Choices (KCC) incorporate capitated payments for chronic disease management. Prior research on the effect of capitated payments on chronic disease management has shown mixed results. We assessed the patient, physician, and practice characteristics of practices with capitation as the majority of revenue, and evaluated the association of capitated reimbursement with quality of chronic disease care.

INTRODUCTION: There is increasing interest in using capitation rather than fee for service to promote primary care and population health. The goal of this study was to examine the association between practice reimbursement mix (majority fee for service versus majority capitation versus other) and receipt of common preventive screening examinations and health counseling from 2012 to 2018.
METHODS: Using the National Ambulatory Medical Care Survey, a retrospective cross-sectional study of 24,864 visits with primary care clinicians among patients aged 18-75 years without a cancer diagnosis was c

Due to the challenge of operating within an economically strained healthcare budget, Portuguese health authorities convened with dialysis providers and agreed on a framework to change from a fee-for-service reimbursement modality to a capitation payment system for hemodialysis. This article reviews the components of the agreed capitation package implemented in 2008 as well as the necessary preparatory work undertaken by a for-profit 34-unit dialysis network (approx. 4,200 patients) to cope with the introduction of this system. Furthermore, trends in clinical quality indicators and in resource

The aims of this study were, first, to determine the frequency of dental examination of child patients by general dental practitioners working under a fee-for-service or capitation payment system in eight areas of the United Kingdom and, secondly, to consider how the frequency of examination varied by dentist payment system, by area, by age of the child and by dentist. The number of examinations carried out by 198 general dental practitioners over a one-year period for 17,389 children aged 0-15 years was recorded. The mean number of examinations per year for children treated by dentists workin

BACKGROUND: Ontario's primary care reforms have introduced three blended physician payment models: (i) blended fee-for-service (BFFS), (ii) blended capitation without interprofessional teams, and (iii) blended capitation with teams. Each model includes the same pay-for-performance incentives, yet their impact on cancer screening, including that during the COVID-19 pandemic, remains unclear.
METHODS: We used linked administrative data (2018-23) to examine the associations between these models and colorectal, cervical, and breast cancer screening rates. Fractional probit regression models, adju

OBJECTIVE: To determine primary care physician screening, treatment, and control rates for hypertension and to examine whether type of physician payment model affected these rates.
DESIGN: A cross-sectional chart abstraction study.
SETTING: Community health centres (salary), primary care networks (capitation), or traditional fee-for-service practices in Ontario.
PARTICIPANTS: A total of 135 primary care physicians, 45 from each of the 3 different models of care. Data were abstracted from 28 adult patient charts randomly selected from each physician.

PURPOSE: To examine service use by adults with serious mental illness (SMI) rostered in new primary care models: enhanced fee-for-service (FFS), blended-capitation (CAP) and team-based capitation (TBC) models with and without mental health workers (MHW) in Ontario.
METHODS: This cross-sectional study used administrative health service databases to compare use of mental health and general health services among persons with SMI enrolled in new models (n = 125,233).
RESULTS: Relative to persons rostered in enhanced FFS, those in CAP and TBC had fewer mental health primary care visits (adjusted

The medical model is no longer accepted by many as the best means of achieving optimal health. Financial constraints are pushing more efficient and effective ways to deliver services. In Saskatchewan, greater emphasis is being placed on wellness activities (preventive medical counselling, clinical work with other professionals, training, teaching and research and institutional medical administrative duties). We sought to determine if predicted support for these activities was related to equity of income as perceived by physicians. The study design was a cross-sectional study of all 1462 physic

OBJECTIVES: A pilot NHS dental contract was introduced in Northern Ireland between 2015 and 2016, which involved changing the method for paying general dental practitioners working in the NHS from fee-for-service (FFS) to capitation-based payments, providing an opportunity for a robust evaluation. We investigated the impact of a change in payment methods on clinical activity and the quality of care provided.
DESIGN: A difference-in-difference (DiD) evaluation was applied to clinical activity data from pilot NHS dental practices in Northern Ireland compared to matched control NHS practices and

Improving access to primary care physicians' services may help reduce hospitalizations due to Ambulatory Care Sensitive Conditions (ACSCs). Ontario, Canada's most populous province, introduced blended payment models for primary care physicians in the early- to mid-2000s to increase access to primary care, preventive care, and better chronic disease management. We study the impact of payment models on avoidable hospitalizations due to two incentivized ACSCs (diabetes and congestive heart failure) and two non-incentivized ACSCs (angina and asthma). The data for our study came from health adminis

The United States stands at a crossroads in how to pay for health care. Fee for service, the dominant payment model in the U.S. and many other countries, is now widely recognized as perhaps the single biggest obstacle to improving health care delivery. A battle is currently raging, outside of the public eye, between the advocates of two radically different payment approaches: capitation and bundled payments. The stakes are high, and the outcome will define the shape of the health care system for many years to come, for better or for worse. In this article, the authors argue that although capit

BACKGROUND: Primary care has been reformed in recent years in Ontario, Canada, with a move away from traditional fee-for-service to enhanced fee-for-service and capitation-based models. It is unclear how new models have affected disparities in cancer screening. We evaluated whether Ontario's enhanced fee-for-service model was associated with a change in the gaps in cancer screening for people living with low income and people who are foreign-born.
METHODS: We conducted a population-based longitudinal analysis from 2002 to 2013 of Ontario family physicians who transitioned from traditional fee

Introduction Patient attachment to a primary care medical home has emerged as a key strategy to strengthen population health in high-income countries, including the U.S., Canada, New Zealand, the United Kingdom, and other European countries. In Canada, Ontario introduced primary care medical homes beginning in 2002, with capitation-based and interdisciplinary team-based models becoming increasingly prominent over time. Citing cost concerns, however, various government policies were subsequently introduced to limit physician entry into these types of medical homes. Entry into medical homes, rem

BACKGROUND: Methods of remuneration have been linked with the professional behaviour of primary care physicians. In dentistry, this can be exacerbated as clinicians operate their practices as businesses and take the full financial risk of the provision of services. The main methods for remunerating primary care dentists include fee-for-service, fixed salary and capitation payments. The aim of this review was to determine the impact that these remuneration mechanisms have upon primary care dentists' behaviour.
OBJECTIVES: To evaluate the effects of different methods of remuneration on the leve

OBJECTIVE: Patients with cardiovascular diseases are common in the emergency department (ED), and continuity of care following that visit is needed to ensure that they receive evidence-based diagnostic tests and therapy. We examined the frequency of follow-up care after discharge from an ED with a new diagnosis of one of three cardiovascular diseases.
METHODS: We performed a retrospective cohort study of patients with a new diagnosis of heart failure, atrial fibrillation, or hypertension, who were discharged from 157 non-pediatric EDs in Ontario, Canada, between April 2007 and March 2014. We

BACKGROUND: The prevalence of dental and oral diseases is increasing globally, yet the utilization of dental services has not shown a corresponding rise. Cost remains a major barrier, as dental care ranks among the four most expensive health services globally. These costs are shaped by each country's health payment system, which plays a critical role in determining access to healthcare. This scoping review aims to map the existing evidence on oral health payment systems and examine how these systems influence access, utilization, equity, and financial protection in different settings.
