Depressive Disorder in Older Medical Inpatients on General Medicine and Cardiology Services at a University Teaching Hospital.

The authors examined the prevalence, characteristics, and correlates of depressive disorder among medical inpatients age 60 or over at a private teaching hospital. Seventy-six of 129 patients admitted to the general medicine and cardiology services at Duke University Hospital were screened for depressive disorder using DSM-III-R criteria. Depressive disorders were diagnosed in 34.2% of patients: major depression in 13.2%, adjustment disorder in 11.8%, depression not otherwise specified in 5.3%, organic mood disorder in 1.3%, and uncomplicated bereavement in 2.6%. Of patients with major depress

Stakeholder experiences and expectations of transition of care services to reduce medication-related hospital readmission for cardiology patients.

BACKGROUND: Transition of care (ToC) for patients with cardiovascular diseases is a complex, high-risk period often leading to medication-related harm (MRH) and hospital readmissions. While specialised programs exist, a gap in individualised medication management services exist for specific cardiology patient populations. Pharmacist-led, interdisciplinary ToC services have demonstrated a positive impact on patient outcomes. The REducing hospital re-admission for high-risk CARDiology patients (RECARD) program aims to address this by co-designing, implementing, and evaluating a new pharmacist-le

A Transition of Care Service to Reduce Hospital Readmissions for High-Risk Cardiology Patients (RECARD): Protocol for a Pre-Post Interventional Trial.

BACKGROUND: Transition of care (TOC) is a high-risk period for medication errors as patients move between multiple health care professionals, settings, and changing medication regimens. Ineffective communication systems further increase the likelihood of medication harm and hospital readmission. A TOC service with tailored, patient-specific support may help reduce these risks. Cardiovascular medicines significantly contribute to medication harm, and patients recovering from acute myocardial infarction (AMI) or cardiothoracic surgery (CS) are vulnerable due to complex regimens and frequent medi

[Registry of the Activity of the Section of Hemodynamics and Interventional Cardiology of the Spanish Society of Cardiology for the year 2000].

The results of the Registry of the Working Group on Hemodynamics and Interventional Cardiology of the Spanish Society of Cardiology for 2000 are presented. Date came from 100 centers representing all the cardiac catheterization laboratories in Spain; 93 centers performed mainly adult catheterization and 7 carried out only pediatric procedures. In 2000, 88,339 diagnostic catheterization procedures were performed (73,382 coronary angiograms), representing a total increase of 12.5% over 1999. The population-adjusted rate was 1,825 coronary angiograms per 106 inhabitants. With a total of 26,993 pr

Influence of inpatient service specialty on care processes and outcomes for patients with non ST-segment elevation acute coronary syndromes.

BACKGROUND: Since the broad dissemination of practice guidelines, the association of specialty care with the treatment of patients with acute coronary syndromes has not been studied. METHODS AND RESULTS: We evaluated 55 994 patients with non-ST-segment elevation acute coronary syndromes (ischemic ST-segment changes and/or positive cardiac markers) included in the CRUSADE (Can Rapid Risk Stratification of Unstable Angina Patients Suppress Adverse Outcomes With Early Implementation of the ACC/AHA Guidelines) Quality Improvement Initiative from January 2001 through September 2003 at 301 tertiary

Moving in for Renovations: An Innovative Solution for Replacing End-of-Life Capital Equipment. The Michael Garron Hospital-Sunnybrook Collaborative Catheterization Laboratory Project.

Replacement of an end-of-life cardiac catheterization laboratory ("cath lab") can pose a significant challenge to a hospital, particularly in single-cath-lab institutions. The disruption in patient care requires innovative approaches to minimize the inconvenience and ensure ongoing quality of care. We describe a unique approach whereby Michael Garron Hospital (MGH) "leased" a cath lab within Sunnybrook Health Sciences Centre for a 12-week period during a cath lab replacement project at MGH. The MGH cath lab and patient recovery bay remained a completely separate entity staffed by MGH nurses an

Cardiac Pacing Training in Africa: Endorsed by the Africa Heart Rhythm Association (AFHRA): JACC International.

The field of pacing in Africa has evolved in an uncoordinated way across the continent with significant variation in local expertise, cost, and utilization. There are many countries where pacemaker services do not meet one-hundredth of the national demand. Regional, national, and institutional standards for pacemaker qualification and credentials are lacking. This paper reviews the current needs for bradycardia pacing and evaluates what standards should be set to develop pacemaker services in a resource-constrained continent, including the challenges and opportunities of capacity building and

Contemporary in-hospital management strategies for acute decompensated heart failure.

Acute decompensated heart failure (ADHF) is a common syndrome with diverse etiologies and precipitating factors, which is associated with significant morbidity and mortality. Tremendous resources are used in treating this syndrome, with few prospectively designed clinical trials to guide therapy. Patients suffering from ADHF are at increased risk for readmission to the hospital as well as an increased risk of death. Prompt identification and management of these patients can lead to shorter length of hospital stay, lower likelihood of readmission, and, perhaps, lower mortality. Initial treatmen

Outpatient echocardiography in the evaluation of innocent murmurs in children: utilisation benchmarking.

OBJECTIVES: We sought to benchmark the utilisation of echocardiography in the outpatient evaluation of heart murmurs by evaluating two large paediatric cardiology centres. BACKGROUND: Although criteria exist for appropriate use of echocardiography, there are no benchmarking data demonstrating its utilisation. METHODS: We performed a retrospective cohort study of outpatients aged between 0 and 18 years at the Sibley Heart Center Cardiology and the Children's Hospital of Philadelphia Division of Cardiology, given a sole diagnosis of "innocent murmur" from 1 July, 2007 to 31 October, 2010. How to Develop a Cardio-Oncology Clinic.

Cardiovascular demands to the care of cancer patients are common and important given the implications for morbidity and mortality. As a consequence, interactions with cardiovascular disease specialists have intensified to the point of the development of a new discipline termed cardio-oncology. As an additional consequence, so-called cardio-oncology clinics have emerged, in most cases staffed by cardiologists with an interest in the field. This article addresses this gap and summarizes key points in the development of a cardio-oncology clinic.

A history of cardiology in Jamaica.

The history of cardiology in Jamaica is conveniently considered in decades beginning in the 1950s. The decade of the 1950s was characterized by early descriptions of the pattern of cardiac disease in adults and children in Jamaica, the establishment of a cardiac clinic at the University Hospital of the West Indies and early cardiac surgical landmarks. Extensive preparatory experimental work in the canine laboratory with respect to cardiopulmonary bypass in the early to mid-1960s culminated in the successful completion of the first open heart surgical procedure in April, 1968. Strategies for the improvement of cardiac care services in developing countries: what does the future hold?

Cardiovascular disease is the leading cause of death in developing countries. The atherosclerotic or degenerative diseases are not only on the rise, they are occurring at a younger age compared with those in developed nations. Some of these developing countries continue to have a high prevalence of rheumatic heart disease, with a large number of new cases with acute rheumatic fever. It appears that the prevalence of congenital heart disease has remained the same in both developing and developed countries, although the actual data are missing. Most of the developing countries have an increasing

[Roles of Cardiology Pharmacists].

Inpatients with heart failure take a number of high-risk drugs such as anti-thrombotic and anti-arrhythmic drugs, in addition to other drug types, including diuretics and cardioprotective agents. Pharmacists should support the pharmacological management of these patients by evaluating their clinical condition and laboratory data, due to the frequency of renal failure. Furthermore, it is known that poor adherence to a drug regimen is one of the aggravating factors for heart failure. It is necessary to conduct regular patient education to facilitate early hospital discharge and to prevent the re

[Improved information on drugs to patients. A project with a pharmacist in the therapeutic team at a cardiology department].

Patients with cardiac disease knew more about medicaments after receiving medicament-related counselling by a pharmacist on discharge from hospital. This was demonstrated by a questionnaire answered by 49 patients whose answers were compared with the results of a study conducted in 1991, when patients did not receive information from a pharmacist in hospital. More patients were pleased with the information, and most of them were satisfied with the consultation with the pharmacist. Lack of time was stated to be the main reason why other hospital health care professionals often provided insuffic

Severe bradycardia at acute medical take - Think about myxoedema coma.

We report two cases of myxoedema coma that presented to the acute medical take with severe bradycardia. These patients were initially misdiagnosed as bradyarrhythmia of primary cardiac origin. They were then diverted to the cardiology service at another district general hospital (DGH) for admissions. Both cases were subsequently diagnosed with myxoedema coma having screened thyroid function tests on arrival at the cardiology unit. Despite being treated for myxoedema coma, both patients unfortunately succumbed to the disease and later died in the hospital. These cases highlight that clinical su

Characteristics of paediatric cardiology patients with interrupted outpatient care using a social determinants framework.

BACKGROUND: Disparities in clinical outcomes exist among children with CHD, and social determinants of health are a significant contributor. Follow-up care plays an important role in long-term health and may be impacted by social determinants of health. We aimed to describe the population with lapses in care to outpatient, general paediatric cardiology from a social determinants of health standpoint and compare them to our primary service area. METHODS: A retrospective chart review was completed for children who had lapses in care within outpatient, general cardiology. Data were compared to t

Bifascicular block in unexplained syncope is underrecognized and under-evaluated: A single-center audit of ESC guidelines adherence.

BACKGROUND: The presence of bifascicular block on electrocardiography suggests that otherwise-unexplained syncope may be due to complete heart block. European Society of Cardiology (ESC) recommends investigating it with electrophysiology study (EPS). PPM is indicated if high-degree atrioventricular block is inducible. Long term rhythm monitoring with implantable loop recorder (ILR) is recommended if EPS is negative. We evaluated adherence to these guidelines. METHODS: This is a single-center retrospective audit of adult patients with bifascicular block hospitalized for unexplained syncope bet
