The Caribbean Netherlands: Health System Review.

This analysis of the health system of the Caribbean Netherlands reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance on the islands of Bonaire, St Eustatius and Saba (the BES islands). Since the dissolution of the Netherlands Antilles in 2010, residents of the BES islands live in special municipalities of the Netherlands, each with its own government (public entity). The Ministry of Health, Welfare, and Sport (VWS) in The Hague, through the Department of Care and Youth Caribbean Netherlands (ZJCN), oversees the health sy

Health access reforms in the Caribbean Netherlands from 2010-2023.

BACKGROUND: In 2010, the islands of Bonaire, Sint Eustatius and Saba became part of the Netherlands, one of the four constituent countries within the Kingdom of the Netherlands. Public administration, including the health system, was reformed, with the Dutch Government aiming for a "level of facilities acceptable within the Netherlands, taking into account the specific circumstances on the islands". REFORM CONTENT: The Dutch Health Ministry became responsible for legislating, financing and policy in a health system with limited stakeholders. The health policy agenda focused on building a publ

Sailing between fate and control: Health systems resilience in the Caribbean Netherlands.

Small, non-sovereign islands remain largely absent from the health policy and systems research literature, despite facing distinct governance challenges, rooted in logistical challenges, sociogeographical isolation, and post-colonial entanglements. We therefore studied how health system actors on Bonaire, Sint Eustatius, and Saba - three islands and 'special municipalities' of the Netherlands in the Caribbean - enact health systems resilience in everyday practice. We draw on multi-sited ethnographic research conducted between February and December 2024. We collected 44 interviews, 60 h of obse

HIV status disclosure among HIV-positive African and Afro-Caribbean people in the Netherlands.

HIV status disclosure is often characterized as a dilemma. On the one hand, disclosure can promote health, social support, and psychological well-being. On the other, disclosure can lead to stigmatization, rejection, and other negative social interactions. Previous research has shown that HIV status disclosure is a reasoned process whereby the costs and benefits to oneself and to others are weighed. As such, understanding disclosure requires understanding the reasons for and against disclosure employed by people living with HIV (PLWH). In this study, disclosure among a population disproportion

Uma Tori! Evaluation of an STI/HIV-prevention intervention for Afro-Caribbean women in the Netherlands.

OBJECTIVE: This study describes the effectiveness of 'Uma Tori', an STI/HIV-prevention intervention for women of Afro-Surinamese and Dutch Antillean descent in the Netherlands, aimed at increasing awareness of sexual risk and power in relationships and improving sexual decision-making skills. METHODS: Intervention effects were evaluated in a pre-post-test design, using self-report questionnaires among a sample of 273 women. Data were analysed using intention-to-treat, MANOVA with repeated measures and Bonferroni correction for multiple comparisons. Additionally, a qualitative process evaluati

An analysis of community perceptions of mosquito-borne disease control and prevention in Sint Eustatius, Caribbean Netherlands.

BACKGROUND: In the Caribbean, mosquito-borne diseases are a public health threat. In Sint Eustatius, dengue, Chikungunya and Zika are now endemic. To control and prevent mosquito-borne diseases, the Sint Eustatius Public Health Department relies on the community to assist with the control of Aedes aegypti mosquito. Unfortunately, community based interventions are not always simple, as community perceptions and responses shape actions and influence behavioural responses Objective: The aim of this study was to determine how the Sint Eustatius population perceives the Aedes aegypti mosquito, mosq

Ten-yr results of the trans-Atlantic kidney transplant airlift between the Dutch Caribbean and the Netherlands.

The prevalence of end-stage renal failure in Curaçao (Dutch Caribbean) is one of the highest in the world. In 1998, the St. Elisabeth Hospital started a unique trans-Atlantic collaboration with the Academic Medical Center in Amsterdam, the Netherlands, and the Eurotransplant Foundation. The partnership aimed to achieve a structured transplantation program for patients in the Dutch Caribbean, who otherwise would need lifelong dialysis. This study is an analysis of the 10-yr transplantation results of this trans-Atlantic program. In 41 consecutive transplantations performed between January 1998

The prevalence and manifestations of hereditary hemorrhagic telangiectasia in the Afro-Caribbean population of the Netherlands Antilles: a family screening.

Hereditary hemorrhagic telangiectasia (HHT) has been reported rarely in people of African descent. The prevalence in the Afro-Caribbean population of the Netherlands Antilles is suspected to be high. A family screening in this population was done to arrive at a point prevalence and to identify patients with pulmonary involvement. By clinical history and physical examination, 219 persons over age twelve with a first-degree relative with HHT were screened. The diagnosis was based on the new diagnostic criteria [Shovlin et al., 2000]. Chest-roentgenogram and pulse-oximetry or measurement of arter

Seroepidemiology of Measles, Mumps and Rubella on Bonaire, St. Eustatius and Saba: The First Population-Based Serosurveillance Study in Caribbean Netherlands.

The National Immunization Program (NIP) on Bonaire, St. Eustatius and Saba (i.e., Caribbean Netherlands (CN)) includes the measles-mumps-rubella (MMR) vaccine since 1988/89. Seroepidemiological data is an important tool to evaluate the NIP, hence a cross-sectional representative population-based serosurveillance study was conducted for the first time in CN in mid-2017. Participants (n = 1829, aged 0-90 years) donated a blood sample and completed a health-related questionnaire. MMR-specific IgG antibodies were determined using a bead-based multiplex immunoassay and risk factors were analyzed us

High seroprevalence of multiple high-risk human papillomavirus types among the general population of Bonaire, St. Eustatius and Saba, Caribbean Netherlands.

BACKGROUND: Incidence and mortality of human papillomavirus (HPV)-related cancers differs geographically, with high rates in Caribbean countries. Seroepidemiological data provide information on lifetime cumulative HPV exposure and contributing risk factors, but has not been available yet for Caribbean Netherlands (CN), comprising the islands Bonaire, St. Eustatius and Saba. Therefore, a cross-sectional population-based serosurveillance study was performed in this (recently girls-only HPV-vaccinated) population in 2017. METHODS: Blood samples from participants (n = 1,823, 0-90 years) were test

Factors constraining natural recovery of Diadema antillarum following a mass die-off: a case study near the island of Saba, Caribbean Netherlands.

After the mass mortality of 1983-1984, recovery of the keystone herbivore Diadema antillarum has been limited. Persistently low population densities reduced grazing pressure, leading to algal dominance on many Caribbean reefs. To understand recovery dynamics and support restoration, greater insight into factors constraining sea urchin population recovery is essential. We assessed D. antillarum settlement, predator density, shelter availability, and post-die-off recovery at four locations near Saba, Caribbean Netherlands, following the 2022 D. antillarum die-off. One location, an artificial roc

Obstetrical condition and neonatal neurological outcome in Dominica, the Caribbean. A comparative study.

Risk factors during pregnancy and delivery and neurological morbidity of newborns were assessed in a birth cohort in Dominica, the Caribbean. The data were compared with two reference groups, one from Grenada, the Caribbean, and the other from Groningen, the Netherlands. Despite variations in cultural and socio-economic situation, the similarities in obstetrical conditions, neonatal neurological morbidity and perinatal relationships between the three groups were more striking than the differences. The Dominican group showed a significantly higher rate of preterm births than the two other group

Attempted suicide of ethnic minority girls with a Caribbean and Cape Verdean background: rates and risk factors.

BACKGROUND: WHO data shows that female immigrants in Europe attempt suicide at higher rates than 'native' women and 'native' and immigrant men. Empirical studies addressing attempted suicide of female immigrants of Caribbean (Antillean-Dutch and Creole-Surinamese-Dutch) as well as Cape Verdean descent in Europe are however scarce. We aim to increase knowledge about rates and risk factors of girls of Caribbean and Cape Verdean descent living in the Netherlands. METHODS: We conducted logistic regression on a dataset that consisted of self-reported health and well-being surveys filled out by 561

Schizophrenia among Surinamese in the Netherlands: high admission rates not explained by high emigration rates.

BACKGROUND: A high risk of schizophrenia has been found among Caribbean immigrants in the Netherlands and Great Britain. One hypothesis to explain these findings is that patients with a diagnosis of schizophrenia or patients with symptoms of psychosis not specifically diagnosed, emigrate more than the general population. Such selection might account for high rates of Surinamese patients with a diagnosis of schizophrenia in Netherlands psychiatric hospitals. We examined this hypothesis. METHODS: The files of patients with schizophrenia or patients suffering from unspecified psychosis were sele

Migration and schizophrenia: the challenges for European psychiatry and implications for the future.

BACKGROUND: The last decade of the twentieth century has seen an unprecedented increase in the number of reports in the psychiatric literature documenting increased rates of psychotic illness among migrants in a range of European countries. In countries where high rates of immigration have been long-standing such as Britain and the Netherlands, these increased rates have also been seen in the second generation of migrants. This has impacted on psychiatry significantly with regard to the aetiology, diagnosis, and treatment of schizophrenia. METHOD: We reviewed the literature to summarise the a

Small countries face specific challenges in health workforce sustainability, but policy responses are a testbed for resilience for all countries.

While countries across Europe are facing challenges in building and sustaining a health workforce, small countries (official population of under 2 million people) face specific challenges that go beyond resource constraint. Limited population size constrains training capacity, specialist care provision, and career development opportunities, making these systems highly vulnerable to workforce shortages. Experiences in Cyprus, Luxembourg, North Macedonia, and the Caribbean Netherlands illustrate these dynamics. Cyprus and Luxembourg have expanded in-country medical education and postgraduate tra

Two new species of Varicus from Caribbean deep reefs, with comments on the related genus Pinnichthys (Teleostei, Gobiidae, Gobiosomatini, Nes subgroup).

Tropical deep reefs (~40-300 m) are diverse ecosystems that serve as habitats for diverse communities of reef-associated fishes. Deep-reef fish communities are taxonomically and ecologically distinct from those on shallow reefs, but like those on shallow reefs, they are home to a species-rich assemblage of small, cryptobenthic reef fishes, including many species from the family Gobiidae (gobies). Here we describe two new species of deep-reef gobies, Varicusprometheussp. nov. and V.roatanensissp. nov., that were collected using the submersible Idabel from rariphotic reefs off the island of Roat

[Imported Zika virus infection in the Netherlands].

Since mid-2015, a rapidly expanding outbreak of Zika virus infection is spreading across Latin America and the Caribbean. Although Zika virus infection usually causes only mild disease, the World Health Organization has declared the epidemiological association with the occurrence of congenital microcephaly and neurological complications a 'Public Health Emergency of International Concern' and urged the international community to mount a coordinated international response aimed to protect people at risk, especially pregnant women. In December 2015, the first case of imported Zika virus infectio

Ethnic inequalities in ischemic stroke occurrence are partly mediated by air pollution: a Dutch nationwide follow-up analysis.

BACKGROUND: Ethnic disparities in ischemic stroke are substantial and differences in exposure to air pollution have been demonstrated as well. However, the role of air pollution in mediating ethnic disparities in risk of ischemic stroke remains unclear. Understanding this relationship is crucial, as air pollution is a major modifiable risk factor for ischemic stroke. This study aims to determine whether air pollution exposure mediates ischemic stroke risk differences between Indonesian, Surinamese, Dutch Caribbean, and ethnic Dutch populations. METHODS: We carried out a nationwide cohort study

How patterns of migration can influence the heterosexual transmission of HIV in The Netherlands.

OBJECTIVES: Migrants originating from countries with a high HIV prevalence account for a large number of heterosexually acquired HIV infections in western European countries. These migrants may be infected before migrating and they may engage in unsafe sexual practices both in the country of residence and in their country of origin. This report investigates how patterns of migration may affect the heterosexual HIV epidemic in The Netherlands. METHODS: A mathematical model was used that describes the transmission of HIV in heterosexual partnerships between African migrants, Caribbean migrants
