External carotid dissection and external carotid proatlantal intersegmental artery with subclavian steal prompting external carotid and subclavian artery stenting.

BACKGROUND AND PURPOSE: The authors describe a case of a proatlantal intersegmental artery seen in the setting of external carotid artery dissection and subclavian steal due to proximal subclavian artery stenosis. METHODS: An 83-year-old woman presented with left homonymous hemianopsia and a right posterior cerebral artery distribution stroke. She was found to have severe left subclavian artery stenosis proximal to the vertebral artery (VA) and an occluded contralateral VA. Doppler ultrasonography and angiography both showed a dissection of the proximal left external carotid artery. Left comm

Contralateral external carotid-to-external carotid artery (half-collar) saphenous vein graft for common carotid artery occlusion.

In a patient having common carotid artery occlusion complicated with contralateral internal carotid artery stenosis, a saphenous vein bypass graft was performed from the contralateral external carotid artery to the ipsilateral external carotid artery simultaneously with contralateral carotid endarterectomy. The bypass was patent 1 year later and the patient was free from fainting spells. Among a number of the reconstructive surgical procedures, this "half-collar bypass" can be placed adequately in the subcutaneous soft tissue so that head or mandibular movement does not displace or compress th

External carotid artery stenting in symptomatic internal carotid artery occlusion.

We report the case of a patient with an anterior ischemic stroke due to tandem occlusion of the left M2 segment and ipsilateral internal carotid artery (ICA), with concomitant severe stenosis of the ipsilateral external carotid artery (ECA) and contralateral ICA, and moderate stenosis of the left vertebral artery (VA); as thrombectomy was not possible, stenting of the right ICA was performed. Two days after significant recovery, the patient showed neurological deterioration when in upright position, and brain magnetic resonance imaging confirmed decreased cerebral blood flow on the left hemisp

Spontaneous external carotid artery occlusion.

The case of a 64-year-old man who presented with severe left sided facial pain and cyanosis of the left ear, preauricular region and tongue is described. Digital subtraction angiography confirmed complete occlusion of the left external carotid artery. The patient was anticoagulated and his symptoms resolved over the following month, presumably due to improvement of collateral blood supply to the ischaemic areas. The literature on the clinical manifestations of external carotid ischaemia is reviewed and the relationship between facial pain, internal and external carotid atherosclerosis is empha

Fate of External Carotid Artery Following Carotid Artery Stenting for Internal Carotid Artery near Occlusion.

BACKGROUND: The external carotid artery (ECA) plays a vital role in facial perfusion and acts as a collateral pathway for cerebral blood flow during internal carotid artery (ICA) stenosis. In cases of carotid near-occlusion (CNO), characterized by severe ICA stenosis with hemodynamic changes, carotid artery stenting (CAS) is rising as a potential new treatment. During CAS, the stent is deployed in ICA, covering the ECA orifice. Therefore, this study aims to evaluate the effects of CAS on ECA. MATERIALS AND METHODS: This retrospective study included 159 patients diagnosed with CNO and treated

Clinical Impact of External Carotid Artery Remodeling Following Carotid Artery Stenting.

Background: Carotid artery stenting (CAS) is a common revascularization approach for carotid artery stenosis. While its impact on the internal carotid artery (ICA) has been extensively studied, the effects on the external carotid artery (ECA)-a key collateral pathway for cerebral perfusion-remain insufficiently explored. This study aimed to assess structural changes in the ECA following CAS and their clinical significance. Methods: A retrospective observational cohort study of 963 patients treated with CAS between 2018 and 2024 was conducted. Demographic data, comorbidities, and procedural cha

External carotid artery to ophthalmic artery flow associated with internal carotid artery stenosis.

PURPOSE: One of the most devastating complications of facial filler injection is sudden ischemic blindness. However, its mechanisms and predisposing factors are poorly understood. The purpose of this study was to investigate the prevalence of external carotid artery (ECA) to ophthalmic artery (OA) anterograde flow in patients with internal carotid artery (ICA) stenosis and in a control population without carotid disease. METHODS: In this cross-sectional cohort study, two groups of patients who underwent catheter cerebral angiography over a 5-year period were identified: patients with symptoma

External carotid artery stenting and superficial temporal artery to middle cerebral artery anastomosis for internal carotid artery occlusion with external carotid artery severe stenosis: case report.

Superficial temporal artery (STA) to middle cerebral artery (MCA) anastomosis may have inadequate effects in patients with internal carotid artery (ICA) occlusion and severe stenosis of the ipsilateral external carotid artery (ECA), because poor blood flow in the STA leads to insufficient flow to the MCA. In these patients, dilation of the stenotic ECA is required to improve the blood flow in the STA before STA-MCA anastomosis. A 71-year-old man presented with left hemiparesis and dysarthria. Magnetic resonance imaging revealed an old watershed infarction in the right cerebral hemisphere. Righ

Embolization in the external carotid artery.

The technical skill set of peripheral interventional radiologists is well-suited to the performance of most transcatheter embolization procedures in the external carotid artery (ECA). These procedures center in large part on hypervascular tumors, epistaxis, and trauma. ECA embolization in the trauma patient is well-defined, albeit in small patient series. The transcatheter treatment of epistaxis is still mostly reserved for cases that are intractable to conservative therapy. Preoperative embolotherapy for vascular tumors remains popular, although it is somewhat controversial in terms of its ri

Reconstruction of the external carotid artery.

The external carotid artery (ECA) is an underestimated but important collateral to the cerebral hemisphere and eye in patients with severe disease of the internal carotid artery. Fifteen symptomatic patients with total occlusion of the internal carotid artery underwent ECA reconstruction. Ipsilateral ECA reconstruction was performed upon all patients with no mortality or neurologic deficits. Contralateral disease of the carotid artery was noted in 11 and required correction in seven patients. Follow-up study of the patients ranged from one to 68 months (a mean of 26.8 months) after operation.

Differential effects of carotid artery stenting versus carotid endarterectomy on external carotid artery patency.

PURPOSE: To determine the effect of stent coverage of the external carotid artery (ECA) after carotid artery stenting (CAS) compared to eversion endarterectomy of the ECA after carotid endarterectomy (CEA). METHODS: The records of 101 CAS and 165 CEA procedures performed over 2 years were reviewed. Duplex velocities and history and physical examinations were taken prior to the procedure, at 1 month, and at 6-month intervals subsequently. CAS was performed by extending the stent across the internal carotid artery (ICA) lesion into the common carotid artery (CCA) thereby covering the ECA. CEA w

Fate of the external carotid artery following carotid interventions.

OBJECTIVE: The external carotid artery (ECA) is an important collateral pathway for cerebral blood flow. Carotid artery stenting (CAS) typically crosses the ECA, while carotid endarterectomy (CEA) includes deliberate ECA plaque removal. The purpose of the present study was to compare the long-term patency of the ECA following CAS and CEA as determined by carotid duplex ultrasound. METHODS: Duplex ultrasounds and hospital records were reviewed for consecutive patients undergoing CAS between February 2002 and April 2008, and were compared with those undergoing CEA in the same time period. External carotid artery to internal carotid artery transposition to augment flow for a superficial temporal artery to middle cerebral artery bypass associated with severe external carotid artery stenosis.

BACKGROUND: Donor vessel quality can impact the outcome of extracranial-intracranial (EC-IC) bypass. External carotid artery (ECA) disease may produce embolism into the anastomosis and cerebral territory and possibly reduce flow in the superficial temporal artery (STA). Previously reported remedies to ECA stenosis include ECA endarterectomy, stenting, and angioplasty. Clinical presentation A middle-aged patient with chronic left MCA occlusion, progressive ischemic symptoms on maximal medical therapy, and imaging confirmation of compromised hemodynamic reserve was evaluated for EC-IC bypass. External carotid endarterectomy followed by superficial temporal artery to middle cerebral artery anastomosis for internal carotid artery occlusion with advanced ipsilateral external carotid stenosis.

OBJECTIVE: For patients with internal carotid artery occlusion with advanced narrowing of the ipsilateral external carotid artery (ECA), we performed preventive carotid endarterectomy (CEA) for the ECA stenosis before superficial temporal artery (STA) to middle cerebral artery (MCA) anastomosis for internal carotid artery occlusion. METHODS: Between August 2002 and July 2005, we treated seven patients with such lesions, six men and one woman, ranging in age from 52 to 66 years (median, 60 yr). Before STA-MCA anastomosis, we performed preventive CEA for advanced ECA stenosis (>70%) to ensure s

Carotid artery stenting has increased risk of external carotid artery occlusion compared with carotid endarterectomy.

OBJECTIVE: The external carotid artery (ECA) can be an important source of cerebral blood flow in cases of high-grade internal carotid artery stenosis or occlusion. However, the treatment of the ECA is fundamentally different between carotid endarterectomy (CEA) and carotid artery stenting (CAS). CEA is routinely associated with endarterectomy of the ECA, whereas CAS excludes the ECA from direct flow. We hypothesize that these differences make ECA occlusion more common after CAS. Further, the impact of CAS on blood flow into the ECA is interesting because the flow from the stent into the ECA i

[Internal carotid artery reconstruction with transposition of external carotid artery in carotid body tumor resection].

The current treatment of Shamblin's class III carotid body tumors includes a variety of reconstructive techniques for the internal carotid artery, ranging from angioplasty to the use of various grafts such as autologous saphenous vein or synthetic shunts. We present the case of a 56-year-old female patient with a carotid body tumor. The diagnosis and therapeutic approach is discussed, as well as the surgical technique and postoperative outcome. In those cases where the carotid body tumor involves the internal carotid artery to the point that its sacrifice is imminent and synthetic grafts are n

Critical Procedural Anatomy of High-Risk External Carotid Artery-Internal Carotid Artery Anastomoses.

Transarterial embolization of external carotid artery (ECA) branches is increasingly performed for the management of epistaxis, preoperative embolization of head and neck tumors, treatment of dural arteriovenous fistulas, and, more recently, for the treatment of chronic subdural hematoma. As new indications for ECA embolization in the management of conditions like chronic subdural hematoma continue to be identified, it is imperative that interventionalists understand the presence of intricate anastomoses between the extracranial and intracranial arterial systems, which confer significant proce

External carotid artery stenosis after internal and common carotid stenting.

BACKGROUND: The external carotid artery (ECA) can be an important collateral for cerebral perfusion in the presence of severe internal carotid artery (ICA) disease. ICA stenting that covers the ECA origin may put the ECA at increased risk of stenosis. Our objective was to determine the rate of ECA stenosis secondary to ICA stenting, determine predictive factors, and describe any subsequent associated symptoms. METHODS: We retrospectively reviewed clinical data on all ICA stents crossing the origin of the ECA placed by vascular surgeons at our institution. We analyzed patient demographics, com
