Carotid Artery Pathology in Inflammatory Diseases.

There is considerable evidence that patients with inflammatory conditions are at higher risk of developing cardiovascular (CV) disease including carotid artery stenosis. CV disease accounts for 35-50% of the excess mortality in patients with inflammatory diseases such as rheumatoid arthritis, with cerebrovascular disease being the second leading cause of death. We review current evidence regarding the association of inflammatory conditions and specifically carotid artery disease. Clinical epidemiological observations suggest that mechanisms other than classic risk factors may promote accelerat

Carotid artery disease.

Carotid artery disease is often implicated in primary ischemic stroke secondary to thrombosis or embolism and may indicate concurrent cardiovascular disease. Atherosclerosis is the underlying cause of the majority of strokes. It is highly correlated with carotid artery disease because it occurs most often at the bifurcation of the common carotids. Transient ischemic attacks are an important clinical syndrome in the diagnostic evaluation of patients with carotid insufficiency. Amaurosis fugax is typically the most common ocular symptom. In addition to a careful history and physical examination,

Carotid Artery Tortuosity Is Associated with Connective Tissue Diseases.

BACKGROUND AND PURPOSE: There is a general assumption in the cerebrovascular literature that there is an association between carotid artery tortuosity and connective tissues disease; however, this has not been firmly established. The purpose of this study was to determine the prevalence of carotid artery tortuosity in patients with connective tissue diseases relative to matched controls. MATERIALS AND METHODS: Patients with previous CTA or MRA and a diagnosis of connective tissue diseases were identified and compared with a cohort of age-matched controls. Radiologists blinded to the diagnosis

[Ocular changes in internal carotid artery diseases caused by artheriosclerosis].

The aim of the study was to show ocular changes in internal carotid artery diseases caused by artheriosclerosis and their etiology. Internal carotid artery supply blood to the organ of vision, therefore pathologies of those arteries caused by artheriosclerosis may have direct influence on its functioning. Most common syndromes are temporary (amaurosis fugax) or constant reduction of visual acuity. In fundus examination central retinal artery occlusion and branch retinal artery occlusion are most common diagnosis, retinal vein occlusion, anterior ischemic optic neuropathy, ocular ischemic syndr

Carotid artery occlusive disease.

Carotid disease is a major contributor to stroke, one of the leading causes of death and disability in the United States. Clinically significant stenosis can be detected by duplex ultrasound using well-established criteria. In addition to optimal medical management, surgical and endovascular revascularizations of carotid disease have been demonstrated to be effective interventions that reduce the risk of stroke in properly selected patients.

Nonatheromatous carotid artery disease.

Common and uncommon nonatheromatous diseases affecting the cervical carotid artery are discussed. Specifically, the radiographic and pertinent clinical features of cervical carotid artery congenital variants, dissection, fibromuscular dysplasia, Takayasu's arteritis, and infection are described. Catheter angiography continues to have an important role in the diagnosis of many of these conditions. Magnetic resonance imaging techniques are emerging as a useful adjunct to catheter angiography and occasionally may be the primary imaging modality for diagnosis.

[Magnetic resonance angiography of the carotid artery: artifacts, anatomy, diseases].

Magnetic resonance angiography is now a technique commonly used in neurologic practice. We give a brief overview of the biophysical principles of this technique. Recent refinements and technical innovations are also noted. After some anatomic considerations about the carotid artery, we provide some data about the role of MRA in atherosclerotic and non atherosclerotic diseases (dissections, aneurysms, arteritis, post operative follow-up...) of the extra and intracranial carotid arteries. The purpose of this review is to concentrate on the role of magnetic resonance angiography in patients with

A brief history of carotid artery disease and carotid endarterectomy.

INTRODUCTION: The authors discuss the history of carotid disease and injuries and focus on historical milestones of carotid endarterectomy worldwide and in Bohemia. RESULTS: The first part discusses the carotid artery and ischemic stroke, beginning with Hippocrates' description of apoplexy. It then highlights Willis's discovery of the collateral circulation, followed by the 19th-century thromboembolic theory of ischemic stroke caused by carotid artery occlusion, which is associated with the work of Wirchow and Chiari. Then, in the 1950s, C. M. Fisher visionarily pointed out the possible surgi

[Color-coded Doppler sonography in the diagnosis of carotid artery diseases].

Color-flow Doppler is a new development of duplex sonography of the peripheral vessels. In this study 844 consecutive patients were evaluated (a) to assess the comparative value of these two methods, (b) to see if there is a correlation between the degree of stenosis and the incidence of neurological symptoms and (c) to find a possible relationship between the plaque structure and the incidence of neurological deficits. (a) In 89%, the color-flow assessment was in complete agreement with the duplex assessment. In the remaining 11%, important additional results were discovered in the color flow

[Significance of MR angiography for imaging diagnostics of carotid artery diseases].

During the last decade, magnetic resonance angiography (MRA) evolved to an essential method for radiological diagnostics of extracranial arteries' diseases. Contrast enhanced MRA enables meanwhile the acquisition of angiographic series in high diagnostic quality comparable to that originating from conventional DSA. Due to MRA, conventional DSA anymore plays a crucial role in the diagnostic assessment of carotid artery disease or highly vascularised tumors of the head and neck region. Besides reliable quantification of carotid stenoses, highly resolved MRI sequences provide a promising approach

Asymptomatic Internal Carotid Aneurysm: An Uncommon Disease of the Carotid Arteries.

Despite a low-incidence extracranial carotid artery aneurysm (ECAA) disease has important clinical repercussion that obliges understanding and knowledge of correct treatment. The 2 dominant etiologies are atherosclerotic degeneration and pseudoaneurysm. The natural history of ECAAs is understood. Neck pain, a pulsatile mass and central or peripheral neurological manifestations are the most common symptoms. Recommendations for diagnosis and treatment are not uniform and still under discussion, representing a challenge for clinicians. We discuss a case of 2.5 cm asymptomatic saccular atheroscler

The contribution of the external carotid artery to cerebral perfusion in carotid disease.

PURPOSE: In the presence of carotid occlusion, the external carotid artery (ECA) becomes an important source of cerebral blood flow, especially if the circle of Willis is incomplete. The contribution of the ECA to hemispheric blood flow in patients with severe ipsilateral carotid stenosis has never been previously investigated. METHODS: One hundred eight patients were monitored during sequential cross-clamping of the external (ECA) and then ipsilateral internal carotid artery (ICA) during carotid endarterectomy using transcranial Doppler sonography (TCD) (Neuroguard CDS, Los Angeles, Calif),

The role of carotid endarterectomy in the management of carotid artery disease and stroke.

BACKGROUND: Carotid endarterectomy to surgically clear the carotid artery bifurcation of atherosclerotic material was at one time one of the most common surgical procedures performed in this country. In recent years, however, there has been intense controversy over the appropriateness and efficacy of this procedure for the prevention of ischemic stroke. METHODS: This paper will first review the terminology and pathophysiology of cerebrovascular occlusive disease. The carotid endarterectomy procedure, as well as diagnostic techniques for the evaluation of carotid artery disease, will be descri
