Blunt carotid artery injuries.

BACKGROUND: Blunt carotid artery trauma remains a rare but potentially devastating injury. Early detection and treatment remain the goals of management. Our objective was to identify patients sustaining blunt carotid injuries at a regional trauma center and report on the incidence, demographics, diagnostic workup, management, and outcome. STUDY DESIGN: A retrospective chart review was performed of patients sustaining blunt carotid artery injury between 1990 and 1996. RESULTS: Twenty patients were identified during the 7-year period. All patients suffered blunt trauma, with motor vehicle acci

Internal carotid artery stenting for blunt carotid artery injuries with an associated pseudoaneurysm.

BACKGROUND: Blunt carotid artery injuries (BCI) are being recognized and treated with increasing frequency because of improved screening protocols. Recent advances in endovascular techniques using microcoils, angioplasty, and stenting offer a new treatment strategy for those patients with traumatic pseudoaneurysms (PA) (BCI and PA). Experience with these techniques is limited because of the rarity of these injuries. HYPOTHESIS: Early anticoagulation (AC) or antiplatelet (AP) therapy combined with carotid artery stenting is a safe alternative to AC alone for the treatment of grade III carotid

Carotid artery injuries: experience with 124 cases.

This is a retrospective study of 124 patients with carotid injuries. The common carotid artery was injured in 84% of the patients. Associated trauma to the internal jugular vein was present in 26%. Most patients (56%) were dead on arrival to the hospital and of those who were operated on, the mortality was 22% (overall mortality, 66%). All operative mortalities had severe shock or neurologic deficits on admission. We performed repair on all patients with preoperative neurologic deficits and the mortality was 64%. The use of a shunt did not influence the prognosis. We believe that there is no p

Cranial nerve injuries after carotid artery endarterectomy.

We present a seven-year experience with cranial nerve injuries due to carotid artery endarterectomy. A total of 433 carotid endarterectomies were done on 355 patients by senior surgical residents, with a staff surgeon assisting. Thirteen cranial nerve injuries were identified, five of which were permanent. Knowledge of the anatomic features of the cranial nerves and their branches in the operative field, as well as technical maneuvers during surgery, can reduce such injuries to a minimum.

[Winter sport injuries to the carotid artery].

In 6 patients with a trauma in wintersport we observed carotid artery injuries in the past 13 years. The diagnosis were two occlusions, one dissection, a bilateral stenosis, a traumatic aneurysm and a subclavian steal phenomenon. A spontaneous bilateral thrombosis of the carotid artery was seen during skiing. A carotid artery lesion should be diagnosed as soon as possible followed by an adequate therapy.

[Closed injuries to the extracranial carotid artery].

A series of 26 cases of injury to the extracranial carotid artery included 3 patients with false aneurysms, 21 dissecting aneurysms, and 3 aneurysms following direct trauma. Clinical and angiographic findings, and clinical course in the 26 patients are described, together with changes observed on angiography in the 11 cases where this was repeated. Prognosis in this series appeared to be better than that usually described.

[Injuries of carotid arteries].

Carotid artery injuries reveal as a specific and relatively small group of vascular trauma. Hemorrhage from vessels of this diameter and flow (10 % of cardiac output) has predominantly a fatal end or shows severe neurological sequelae. Cut and stab wounds represent the majority of carotid injuries, often associated with venous damage. Blunt trauma and fractures of the skull basis are uncommon causes. We present 43 cases of carotid injuries operated upon as emergency service (in the majority outside of our hospital). The number of postoperative surgical and neurological complications was modera

Therapeutic endovascular treatments for traumatic carotid artery injuries.

OBJECTIVE: The diagnosis and management of traumatic carotid vessel injuries continue to be controversial, with direct surgical repair with parent artery preservation still presenting difficulties. The purpose of this report is to review the endovascular therapy of patients with traumatic carotid vessel injuries for preservation of the parent artery, and to determine the safety and efficacy of endovascular therapy. METHODS: Ten patients with traumatic carotid lesions were treated using therapeutic endovascular methods. Endovascular therapy was accomplished by implanting balloons, porous or po

Cervical internal carotid artery injuries due to blunt trauma.

Blunt or nonpenetrating trauma to the head and neck occasionally results in damage to the cervical segment of the internal carotid artery. This may produce neurologic signs and symptoms that mimic acute craniocerebral injury. The mortality and morbidity associated with these injuries are alarmingly high. These injuries may be missed if one relies only on computed tomography. Angiography is still the definitive procedure to diagnose these lesions. The clinical and radiographic features as well as the pathogenesis and mechanism of injury are discussed for 21 cases of injury to the cervical inter

Injury of the carotid and vertebral arteries.

A knowledge of the normal anatomy and anatomic relationships of the carotid and vertebral arteries, as well as the biomechanics by which traumatic injury occurs to these vessels, is important both in the interpretation of diagnostic arteriograms and in the clinical assessment of the injured patient.

Management of carotid artery injuries: What you need to know.

Carotid artery injury (CAI), encompassing both penetrating (PCAI) and blunt carotid artery injury (BCAI), occurs infrequently but carries substantial risk of stroke and death. This review summarizes contemporary evidence and best practices in the management of adult PCAI and BCAI. Management of CAI has evolved significantly, driven by the transition from digital subtraction angiography (DSA) to computed tomography angiography (CTA), selective operative exploration of penetrating neck injury, expanded blunt cerebrovascular injury (BCVI) screening, and earlier initiation of antithrombotic therap

Traumatic internal carotid artery dissections caused by blunt softball injuries.

This report describes recently treated patients with carotid artery dissection caused by blunt softball injuries, as well as the results of a study of carotid artery trauma in a community. Data obtained through the medical records linkage system used for epidemiologic studies in Olmsted County, MN were used to identify all cases of traumatic internal carotid artery dissection diagnosed from 1987 through 1994. Four patients with traumatic internal carotid artery dissections were identified during the 8-year period under study. In two patients (50%) the carotid dissection was a result of the dir

Blunt carotid artery dissection: incidence, associated injuries, screening, and treatment.

Blunt carotid dissection (BCD) is a rare injury occurring in less than one in 1,000 victims of blunt injuries. Using a 4-year experience in a trauma system with 14 cases of BCD, we performed a matched blunt trauma patient case-control analysis to determine if there were patterns of injuries that were associated with increased risk of BCD. Patients with combinations of head, facial, and cervical spine injuries with or without extremity fractures proved to be at significantly increased risk for BCD. Duplex scanning appears to be a useful screening tool for these patients. Anticoagulation was the

[Injuries of the common carotid artery due to reanimation measures ].

The status of the large vessels of the neck was determined after emergency intubation in 65 primary atraumatic casualties in the endeavor to provide accident surgeons working under extreme conditions with a check on results to optimize their therapy. Besides the findings in the vertebral artery (shown in 35 cases), which was traumatized in one case, the main attention was dedicated to the injury pattern of the common carotid artery. In this artery, intimal ruptures within the bifurcation were found in three cases, combined in one case with a subintimal hemorrhage on the contralateral side. As

Iatrogenic carotid artery injury in neurosurgery.

Iatrogenic carotid artery injury (CAI) results from various neurosurgical procedures. A review of the literature was conducted to provide an update on the management of this potentially devastating complication. Iatrogenic CAIs are categorized according to each diagnostic or therapeutic procedure responsible for the injury, i.e., anterior cervical spine surgery, central venous catheterization, chemical substances, chiropractic manipulation, diagnostic cerebral angiography, middle-ear surgery, percutaneous procedures for trigeminal neuralgia, radiation therapy, skull-base surgery, tracheostomy,

A Case of Left Carotid Artery Injury.

Invasive pituitary adenomas can infiltrate the dura mater, sphenoid sinus, or cranial bone. Endoscopic transsphenoidal sinus surgery is considered the standard of care; however, several potential complications must be noted. These complications can include cerebrospinal fluid leaks, infection, bleeding, optic nerve damage, and endocrinological complications such as diabetes insipidus. We present a case of a 69-year-old female with multiple recurrent invasive pituitary adenomas who has previously undergone 5 transsphenoidal procedures. Intraoperatively, the patient suffered from a left-sided ca
