A 64-year-old man, with no history of trauma, presented with transient visual loss. He was diagnosed with amaurosis fugax and started on dipyridamole and simvastatin. An inconclusive ultrasound (US) Doppler was followed by CT angiogram (CTA) and MRI, which demonstrated free floating intraluminal thrombus in the distal right common carotid artery. ECG showed sinus rhythm and an echocardiogram showed no cardiac thrombus. Following discussion at the vascular multidisciplinary team the decision was made to treat with intravenous heparin followed by warfarin. He has been regularly followed up with

An autopsy case of a 45-year-old man is presented. The man had sustained a minor head injury in a traffic accident 11 years ago previously and 5 years later had a cerebral infarction due to thrombosis of the carotid artery. There was histological evidence of a traumatic lesion of the vessel wall. Several contradictory expert opinions were rendered regarding the questionable causal relationship between occlusion of the artery and the previous trauma. In the court of appeal, an agreement was arrived at between the widow of the deceased and the third-party insurance of the person responsible for

CASE DESCRIPTION: 37-year-old female with PCR-RT swab for COVID-19 positive, with neurological manifestation as a result of internal carotid artery occlusion.
CLINICAL FINDINGS: Nasal congestion and sneezing of 5 days duration; pulsatile headache in the left hemicranium 3 days prior to admission, with intensity 6/10 according to the visual analogue scale, accompanied by phosphenes, photophobia and diplopia; with subsequent developing right hemiparesis over a 26-hour period.
TREATMENT AND RESULT: She was given medical management with oral antiplatelet agents and anticoagulants (subcutaneous a

A young man with cerebral infarction and circulating lupus anticoagulants was found to have a partially occluding intraluminal thrombosis in the left internal carotid artery. Immediate thrombectomy and medical therapy was followed by gradual partial clinical improvement. Pathologic examination revealed an organized thrombus with diffuse hypersensitivity vasculitis of the small arteries in a skin and muscle biopsy specimen. Increased physician awareness may lead to more prompt diagnosis and treatment of this unusual clinical syndrome.

This is a report of a case of internal carotid artery occlusion associated with rhinocerebral phycomycosis. In 18 autopsied similar cases that have been reported previously, hyphae were described in the walls of the internal carotid artery. Four survivors have been reported even though arteriograms demonstrated internal carotid artery occlusion, and a high index of suspicion for this complication should be maintained in all patients with rhinocerebral phycomycosis.

Thromboangiitis obliterans (TAO, Buerger disease) is a segmental, non-atherosclerotic vasculitis that causes occlusion of the small and medium sized vessels of the distal extremities. In rare cases, it can affect vessels in the gastrointestinal, cerebrovascular, coronary, and renal systems. The etiology of thromboangiitis obliterans is unknown, but there is a strong association with smoking in the development and the progression of the disease. We present the case of a 42-year-old homeless female smoker, who was found dead outdoors. Although originally suspected to be a possible trauma-related

Common carotid artery thrombosis (CCAT) is not common. We studied 4 patients with CCAT to clarify the clinical and radiological features of this disorder. Case 1 had only episodes of TIA. Case 2 was diagnosed by chance as having CCAT at the time of admission due to cerebellar infarction. On the contrary, Cases 3 and 4 were admitted because of disturbance of consciousness and hemiparesis. Case 4 died soon after his stroke because of complications. We evaluated cases 1, 2 and 3 using head CT, head MRI, neck MRI, MR angiography, SPECT and cerebral angiography. Case 4, who was evaluated with head …

Two cases of internal carotid artery thrombosis associated with a maxillary Le Fort III and mandibular angle fractures after maxillofacial blunt injuries are described. Both patients had delayed neurologic deficit and hemiparesis. The diagnosis was made by carotid angiography after clinical evidence of thromboembolism. Clinical aspects and etiology, with special attention to mechanism of injury, are discussed.

Carotid artery dissections are the second leading cause of stroke in young adults. The hemostatic response to a dissection involves exposure of the subendothelium to the intravascular environment. Platelet activation/aggregation superimposed by secondary coagulation cascade activity attempts to heal the injury. Failure of the hemostatic response to heal the injury may lead to further rupture of the intimal and medial layers, which allows for the blood to penetrate these layers to create a false lumen. Continued hemorrhaging into the false lumen may result in dissection progression or obstructi

BACKGROUND AND PURPOSE: The two objectives of this study were to assess the potential of BAY U 3405 to prevent arterial thrombosis in response to vessel wall injury and to determine the ability of BAY U 3405 to prevent thrombotic reocclusion after thrombolysis with anisoylated plasminogen streptokinase activator complex.
METHODS: Dogs were instrumented with a carotid flow probe, stimulating electrode, and a stenosis. Current (150 microA) was applied to the intimal surface of the right carotid artery, and time to occlusive thrombus formation was noted. BAY U 3405 was administered, and the proc

When dense hemiparesis has developed appropriate to the operated vessel after carotid surgery, the patient has been immediately returned to the operating room for reexploration of the vessel and thrombectomy. Over a 5-year period, we identified four patients who developed dense hemiparesis in the recovery room postoperatively. A fifth patient with a similar problem after carotid subclavian bypass was included for analysis. All patients were found to have thrombosis of the operated vessel. Direct closure had been used after endarterectomy. All vessels were thrombectomized and patched with autog

A 75-year-old man who had suffered from right visual disturbance for 10 years suddenly experienced right cavernous sinus syndrome. Magnetic resonance imaging revealed a giant thrombosed aneurysm in the right cavernous sinus extending to the right middle cranial fossa. Digital subtraction angiography disclosed occlusion of the right internal carotid artery at the petrous portion and good cross filling in the right-sided circulation through the anterior communicating artery. There was no filling of the aneurysm. In this case, the mechanism of parent artery occlusion is unclear, but direct compre

The clinical problems posed by a series of partial or total monolateral carotid occlusions complicated by cerebral infarction are discussed. Emphasis is laid on the frequent aspecificity of the symptomatology, the inconsistency of palpatory and auscultatory findings in the cervical region, and the value of certain medical techniques that may precede angiographic diagnosis. Considering the lack of reports in the literature, a case in which carotid stenosis was considered to be of luetic nature is noteworthy. Prophylactic treatment designed to prevent another cerebral attack hinges essentially o
