Carotid Webs.

Carotid webs are increasingly recognized as an underdiagnosed etiology of ischemic stroke, especially in young, otherwise healthy patients. These fibrous intimal protrusions create regions of flow stasis within the internal carotid artery, predisposing to thromboembolism. Diagnosis remains challenging due to their subtle radiographic appearance and underappreciation in clinical practice. While antiplatelet therapy or anticoagulation used to be the cornerstone of management, medical therapy alone has been found to be insufficient for stroke prevention in symptomatic patients. Incidental carotid webs in trauma patients.

INTRODUCTION: Carotid web (CaW) is notably prevalent among young patients with cryptogenic stroke. Data investigating its occurrence in asymptomatic individuals remains scarce. We evaluated the prevalence of CaWs in a cohort of trauma patients. METHODS: Cross sectional retrospective analysis of consecutive patients who underwent thin-cut neck CT angiograms between October 2015 and August 2018 at a Level 1 Trauma Center. The scans were independently reviewed by two fellowship-trained vascular neurologists who were blinded to demographics and clinical information, and all CaW were reviewed by a

Symptomatic carotid webs require aggressive intervention.

OBJECTIVE: Carotid web (CaWeb) is a rare form of fibromuscular dysplasia that can produce embolic stroke. Misdiagnosis of symptomatic CaWeb as "cryptogenic stroke" or "embolic stroke of unknown source" is common and can lead to recurrent, catastrophic neurologic events. Reports of CaWeb in the literature are scarce, and their natural history is poorly understood. Appropriate management remains controversial. METHODS: CaWeb was defined as a single, shelf-like, linear projection in the posterolateral carotid bulb causing a filling defect on computed tomography angiography (CTA) or cerebral angi

Carotid webs management in symptomatic patients.

BACKGROUND: Atypical fibromuscular dysplasia (AFMD), also known as carotid web, is a rare underdiagnosed shelf-like fibrous tissue arising from the posterior carotid artery bulb that is a cause of cryptogenic stroke of the anterior cerebral vascularization. Despite the recurrence and severity of strokes caused by embolization associated with AFMD, there are no recommendations on the best strategy to manage single and bilateral lesions, which have unsatisfactory outcomes when treated with medical treatment exclusively. METHODS: From January 2016 to April 2019, 365 patients were operated on for

Surgical Management of Symptomatic Carotid Artery Webs.

ObjectiveTo demonstrate the demographic profile and postoperative outcomes among a large single-center series of patients undergoing carotid endarterectomy for carotid web.BackgroundCarotid artery web (CaW) is a shelf-like intraluminal lesion at the carotid bulb that produces stagnant flow and can result in cerebrovascular ischemia among young and otherwise healthy individuals. While there is no consensus on the ideal management strategy for CaW with ipsilateral stroke, emerging evidence favors open surgical intervention. We present a large single-center experience of carotid endarterectomy fo

Carotid Webs in Pediatric Acute Ischemic Stroke.

INTRODUCTION: Carotid web is a radiographic entity located in the posterior aspect of the origin of the internal carotid arteries, considered to be a pathologic equivalent to intimal fibromuscular dysplasia, and has been implicated in acute ischemic strokes. The mechanism underlying its development is unknown and it remains unclear if this lesion is congenital or acquired. The aim of our study was to determine the frequency of carotid webs in a pediatric population with acute ischemic stroke (AIS). METHODS: A retrospective review of neck CTA, MRA, or DSA images in a pediatric population with

Morphological and clinical predictors of symptomatic carotid webs: a cross-validated analysis of 124 webs.

PURPOSE: Carotid webs (CWs) are an underrecognized cause of cryptogenic ischemic stroke and transient ischemic attack (TIA). Morphological characteristics are believed to influence their potential to become symptomatic. We aimed to develop and internally validate a reliable predictive model for symptomatic CWs. METHODS: We conducted an institutional retrospective study including patients with CWs identified on Computed Tomography Angiography (CTA). Demographic, clinical, and morphological data were extracted. Symptomatic CWs were defined by a history of stroke or TIA without a more likely cau

Carotid Webs: An Unusual Presentation of Fibromuscular Dysplasia.

Carotid webs are abnormal luminal projections at the carotid bulb associated with blood flow stasis, artery dissection, and subsequent complications. Carotid webs are considered to be a rare variant of fibromuscular dysplasia (FMD). Young individuals with symptomatic carotid webs are found to be associated with ischemic stroke. The incidence of the carotid web is low, and it is rarely reported. Only 150 cases of FMD have been reported so far. FMD is a non-inflammatory and non-atherosclerotic arteriopathy. The most common arterial beds involved are renal and extracranial carotids. Presentation

Atypical Carotid Webs: An Elusive Etiology of Ischemic Stroke.

OBJECTIVE: Typical carotid webs are nonatherosclerotic shelf-like projections of fibromyxoid tissue extending from the posterior wall of the proximal internal carotid artery. Carotid webs may precipitate acute embolic stroke, especially in younger patients. We describe our experience with pathology-proven carotid webs of atypical appearance, or atypical carotid webs (ACWs), a subset of carotid webs exhibiting abnormal location, morphology, or association with atherosclerotic changes. METHODS: Our electronic medical record database was queried for all imaging impressions containing "carotid we

Surgical Treatment of Carotid Webs in Symptomatic Young Adults.

BACKGROUND: Carotid webs are possibly responsible for ischemic stroke in young adults. We aim to report a case series study about surgical treatment of this uncommon condition. METHODS: We performed a retrospective analysis on clinical, imaging, pathological, operative data, and outcomes about consecutive patients affected with carotid ischemic stroke due to carotid webs in a single institution. RESULTS: During a three-year period, a total of 123 patients were treated for carotid stroke, in five cases (4%) (median age 48.4 years, range 44-53, 2 men and 3 women, median NIHSS score 8.4, range

Carotid Web Misdiagnosis.

BACKGROUND: A carotid web (CW), an atypical fibromuscular dysplasia, is rare and may cause ischemic stroke. It is challenging to recognize a CW promptly and treat it accordingly. We report a case of an initially misdiagnosed CW. CASE DESCRIPTION: A 48-year-old man with recurrent cerebral infarction was transferred to our hospital for bypass surgery. Imaging performed at our facility showed multiple old infarct lesions and cerebral tissue hypoperfusion in the occluded left middle cerebral artery territory. Bypass surgery was performed without perioperative complications. While searching for th

Treatment of Symptomatic Carotid Webs: A Single-Center Review.

BACKGROUND: Carotid webs are abnormal thin shelf-like or flap-like tissue in the carotid bulb (proximal internal carotid artery). Rarely are carotid webs detected prior to symptoms since routine carotid artery surveillance is not performed in younger individuals without traditional risk factors for carotid disease. The cause and natural history remain unknown. In general, they are not common but should be considered in the differential diagnosis of a patient who presents with ischemic neurologic symptoms. The web can create a flow disturbance, potentiating local thrombus formation, which can e

Safety and efficacy of staged bilateral carotid artery stenting for bilateral carotid webs.

Carotid webs are endoluminal shelf-like projections caused by fibrous dysplasia observed along the dorsal carotid bulb of the internal carotid artery (ICA). Symptomatic lesions have historically been treated with endarterectomy or stent placement and there is little data to support treating a patient with medical management alone. An asymptomatic contralateral carotid web may be discovered during stroke work-up. In this study, we evaluate the safety and efficacy of staged bilateral carotid artery stenting for bilateral carotid webs. We report three cases: the first patient presented with recur

Carotid Stenting for Treatment of Symptomatic Carotid Webs: A Single-Center Case Series.

BACKGROUND AND PURPOSE: Carotid webs are an increasingly recognized cause of acute is-chemic stroke, particularly in younger adults. The optimal medical and surgical strategies for managing these lesions have not been well established. We report a single-center case series of carotid stenting for treatment of symptomatic carotid webs. MATERIALS AND METHODS: Consecutive patients undergoing stent placement for treatment of symptomatic carotid webs were included. Carotid webs were defined as a thin intraluminal filling defect along the posterior wall of the carotid bulb just beyond the carotid b

Optical Coherence Tomography in the Evaluation of Suspected Carotid Webs.

BACKGROUND: Carotid web (CaW) is a subtype of fibromuscular dysplasia that predominantly involves the intimal layer of the arterial wall and is commonly overlooked as a separate causative entity for recurrent strokes. CaW is defined as a shelf-like lesion at the carotid bulb, although different morphological features have been reported. Optical coherence tomography (OCT) has been described in the literature as a useful microscopic and cross-sectional tomographic imaging tool. This study aimed to evaluate the potential utility of OCT in characterizing the wall structure features of patients wit

Carotid Bulb Webs as a Cause of "Cryptogenic" Ischemic Stroke.

BACKGROUND AND PURPOSE: Carotid webs are intraluminal shelf-like filling defects at the carotid bulb with recently recognized implications in patients with recurrent ischemic stroke. We sought to determine whether carotid webs are an under-recognized cause of "cryptogenic" ischemic stroke and to estimate their prevalence in the general population. MATERIALS AND METHODS: A retrospective review of neck CTA studies in young patients with cryptogenic stroke over the past 6 years (n = 33) was performed to determine the prevalence of carotid webs compared with a control group of patients who receiv

Stent Placement for Carotid Web.

BACKGROUND: The carotid web is an intraluminal shelf-like projection arising from the posterior wall of the carotid bifurcation and an uncommon etiology of ischemic strokes. We describe the feasibility of endovascular stent placement to treat this condition. CASE REPORT: A 47-year-old woman presented with a sudden occlusion of the right middle cerebral artery. Computed tomography angiography and digital subtraction angiography showed a carotid web in the ipsilateral carotid bifurcation. Treatment included mechanical thrombectomy for the middle cerebral artery occlusion and carotid stent place

Intravascular Ultrasound in Carotid Web.

BACKGROUND: Carotid web (CaW) is a shelf-like linear filling defect in the posterior aspect of the internal carotid bulb, representing an intimal variant of fibromuscular dysplasia. The diagnosis of CaW is traditionally restricted to digital subtraction angiography (DSA), CT/MR angiography (CTA/MRA), and Duplex ultrasonography. In this series of patients with acute ischemic stroke, we evaluated the potential utility of intravascular ultrasound (IVUS) in further characterizing suspected CaWs. METHODS: This is a case series of three patients with suspected CaW who underwent DSA for treatment or
