Topical timolol for calciphylaxis: a case report.

BACKGROUND: Calciphylaxis is a rare multifactorial skin disease characterized by thrombosis and calcification of skin subcutaneous vessels, leading to chronic and painful ulcers with skin necrosis. Despite its high morbidity and mortality rates, effective treatments are lacking. CASE REPORT: A 65-year-old female with chronic kidney disease who developed painful bilateral ulcers was successfully treated with topical timolol. Clinical examination and imaging confirmed the diagnosis of calciphylaxis. Treatment with topical timolol resulted in rapid wound healing and pain relief. CONCLUSION: Thi

Topical lignocaine for vaginismus: a case report.

Vaginismus is a sexual dysfunction in which spasm of vaginal musculature precludes penetrative intercourse. In many cases associated pain or fear of pain may contribute to the maintenance of vaginismus. We report a case of primary vaginismus with associated pain that benefited from topical application of lignocaine gel along with systematic desensitization resulting in successful consummation, and suggest that it may be a useful adjunct during finger dilatation in the treatment of vaginismus, specifically in patients who have associated pain or areas of hyperesthesia in the introitus.

Elevated Transaminases with Topical Diclofenac: A Case Report.

Drug-induced liver injury (DILI) has been described with numerous nonsteroidal anti-inflammatory drugs (NSAIDs). Oral diclofenac has been associated with DILI more frequently than other NSAIDs and requires periodic monitoring of liver transaminases and judicious consideration of clinical signs and symptoms of hepatotoxicity. Here we describe a case in which elevated liver transaminases in a 79-year-old female returned to normal following discontinuation of topical diclofenac 1% gel. Using a widely accepted drug reaction causality instrument, a rating of "definite" was assigned given the tempor

[Anterior uveitis and topical brimonidine: a case report].

We report the case of a 66-year-old patient treated for 13 months with brimonidine, who presented granulomatous anterior uveitis associated with an increase in intraocular pressure, allergic conjunctivitis and blepharitis in both eyes. No ocular or general cause for uveitis was found. The uveitis responded rapidly to cessation of brimonidine. This report illustrates that anterior uveitis can occur when brimonidine is used after approximately 1 year of treatment. Physicians must be informed of this side effect even though it is rare, with only four similar cases having been described in the lit

Warfarin--topical salicylate interactions: case reports.

This paper deals with two patients on warfarin in whom the use of topical methylsalicylate preparations led to clinically significant bleeding problems. The first patient required fresh frozen plasma to tide over the crisis while the second patient recovered spontaneously on stopping the warfarin temporarily. The possible mechanisms by which salicylates potentiate the anticoagulant effect of warfarin are briefly outlined.

Methylene Blue, a Unique Topical Analgesic: A Case Report.

Background: Rectal prolapse is a circumferential, full-thickness protrusion of the rectum through the anus, which, if not properly managed, may become incarcerated and pose a risk of strangulation. This pathology is rarely a medical emergency unless a complication is encountered. Such complications include infection, necrosis, perforation, incarceration, and uncontrolled pain. Case Presentation: We report a case of an elderly patient with pain associated with chronic rectal prolapse. Surgical intervention had been ruled out, and there had been no pain relief after using systemic analgesics. Ca

Topical gentamicin-induced acute vestibulopathy: A case report.

Evidence suggests that otologic injury from ototopical aminoglycoside preparations is infrequent when used to treat ear infections with an intact tympanic membrane. Meanwhile, parenteral administration of aminoglycosides, is well known to be associated with a significant incidence of cochlear and vestibular damage. The discrepancy between topical and parenteral ototoxic effects is thought to result from a combination of factors, including the protective function of debris overlying the round window membrane, low antibiotic concentrations of topical antibiotic preparations, length of exposure a

Neurotrophic Keratopathy and Topical Insulin Therapy: A Case Report.

Neurotrophic keratopathy is a rare disorder caused by the loss of corneal sensation. It is characterized by persistent epithelial defects, corneal ulceration, and, ultimately, corneal perforation if not managed in a timely manner. The management includes aggressive lubrication, prophylactic topical antibiotics, therapeutic contact lenses, tarsorrhaphy, and amniotic membrane transplantation. Some novel therapeutic options are also available, one of which is topical insulin therapy. We report the clinical course of a patient with neurotrophic keratopathy that was successfully treated with topica

Topical Gentamicin in the Management of Bart Syndrome: A Case Report.

Bart syndrome is a rare congenital disorder characterized by localized absence of skin, epidermolysis bullosa (EB), and nail abnormalities. We report the case of a female newborn presenting with congenital absence of skin on both feet, associated to multiple blisters in friction-prone areas and paronychia. Despite these findings, she remained clinically stable, was feeding normally, and exhibited no gastrointestinal symptoms. Based on the association of aplasia cutis and epidermolysis bullosa, a diagnosis of Bart syndrome was clinically suspected. The patient was treated with topical gentamici

Topical Sodium Thiosulfate for the Treatment of Calciphylaxis: Case Report.

Calciphylaxis is a rare syndrome of microvascular calcification and skin infarction, occurring most commonly in patients with kidney failure. Calciphylaxis carries a poor prognosis, and current treatment involves mitigating risk factors, wound care, and medical therapies such as intravenous sodium thiosulfate for calcium chelation. We present an innovative treatment approach of topical sodium thiosulfate soaked gauze successfully used to treat calciphylaxis of a 58-year-old female with renal transplant. She presented with a tender bruise over the posterior right calf, in the context of good gr

A Case Report: Utilization of Topical Amphotericin in Postoperative Mucormycosis.

Description Mucormycosis is a rare devastating fungal infection with a high mortality rate often associated with diabetic and immunosuppressed individuals. However, infections found in postoperative states and in immunocompetent patients are rare. Systemic liposomal amphotericin is viewed as a first line treatment for mucormycosis in addition to aggressive debridement to prevent the spread of infection. Literature describing the treatment of mucormycosis with both systemic and topical amphotericin B is scarce. We present a case of an immunocompetent male admitted for perforated diverticulitis

[Crusted scabies induced by topical corticosteroids: A case report].

The frequency of scabies is increasing in France. Crusted (or Norwegian) scabies is a very contagious form of scabies because of the huge number of mites in the skin. It is observed in patients suffering from immunodepression, motor or sensory deficiency, or mental retardation. The clinical presentation, except for the classic manifestation of scabies, is characterized by crusted lesions. Treatment is not easy and requires hospitalization. Topical corticosteroids are frequently used for children's dermatological diseases. Their long-term and inappropriate application in an infested scabies chi

Granuloma faciale treated with topical dapsone: a case report.

Granuloma faciale (GF) is an unusual, treatment-resistant skin disorder that commonly affects the face. Several medical and surgical interventions are available that offer varying degrees of benefit. Both the condition and the treatment modalities can lead to significant disfigurement. The use of oral dapsone in the treatment of GF has been described in the literature, but there are no reports, to our knowledge, of the use of topical dapsone 5% gel (Aczone; Allergan Inc, Irvine, CA). We present a case of a patient with GF on the nasal tip successfully treated with topical dapsone.

First case report of topical tacalcitol for vitiligo repigmentation.

We present a 11-year-old girl with an 8-month history of vitiligo vulgaris on her postcervical, periocular, and periauricular regions. The region topically treated with 0.0002% tacalcitol ointment (vitamin D3 ointment) and sunlight exposure healed completely, while the region treated with the ointment alone did not improve. A combination therapy with tacalcitol ointment and sunlight exposure instead of UV irradiation can be recommended as an alternative therapy for vitiligo vulgaris because the treatment is safe and increases compliance in children.

Methemoglobinemia caused by topical teething preparation: a case report.

Methemoglobinemia (MetHb) remains an uncommon, but potentially fatal disorder. Benzocaine (ethyl aminobenzoate), a topical anesthetic, has been reported to cause acquired MetHb when used during endoscopic or other ambulatory procedures. Reports of severe MetHb following benzocaine-containing preparations in the community, however, are very rare. We discuss this entity by describing an unusual case of severe MetHb in a 5-year-old child, caused by unattended self-use of a benzocaine-containing, pain-relief gel for teething. This case story illustrates the potential lethal risk of over-the-counte
