Tube cecostomy revisited.

A retrospective review of 59 tube cecostomies, performed between 1971 and 1981, was undertaken to evaluate current operative indications, outcome and associated morbidity. Tube cecostomy was performed as a complementary procedure in 81.4% of cases; in the other 18.6%, it represented either the only operative intervention or the initial stage of a two-stage procedure. Complications included local infection in 32% of cases, peri-catheter leak in 25%, skin excoriation in 24% and pain in 12%. Catheters remained in place an average of 14 days, but function was adequate in only 40% of cases. Cecal d

Temporary ileal cecostomy.

Results obtained with temporary ileal cecostomy in 23 patients are reported. Particular attention is given to the incidence of anastomotic leakage, morbidity, mortality, hospitalization and nursing. The results achieved were successful and, although the series presented is small, the authors suggest that this technique should be further considered as an alternative to the more standard diversion techniques of colostomy in colonic resection, particularly where the integrity of the anastomosis is likely to be at risk, as in the anterior resection of the rectum and especially in lower anterior re

Constipation and Outcomes of Cecostomy.

Constipation, defined as delay or difficulty in defecation, present for 2 or more weeks, is a common problem encountered by both primary and specialty medical providers. There are no randomized controlled trials on the use of antegrade enemas in the pediatric population. Most published papers are based on the experience at a particular center. The aim of this article is to describe the pathophysiology of constipation, review the contribution of colonic manometry to the diagnosis of constipation, summarize the advancements in the management of constipation through the use of antegrade enemas, a

Laparoscopic Cecostomy Tube Placement.

Fecal incontinence is a debilitating problem for many children, especially those with anorectal malformations. Historically, surgical options have included the Malone antegrade continence enema, using an appendicostomy for antegrade colonic enemas. Since the development of this procedure, multiple alternatives have been developed, including the Chait cecostomy tube. Here, we present our technique for laparoscopic cecostomy tube placement. We find that this approach has several advantages, including mobilization of the cecum to allow the tube to be placed below the waistline for optimal cosmesi

Successful Colonoscopy-assisted Cecostomy Tube Replacement to Salvage Lost Cecostomy Tract Access in Children.

OBJECTIVES: Cecostomy tubes are commonly used for antegrade enema delivery in children with spinal defects and anorectal malformations to help address chronic constipation and fecal incontinence. Once surgically or radiologically placed, cecostomy tubes require changes by a percutaneous approach, which may be unsuccessful requiring repeat laparoscopy or open surgery to re-establish the cecostomy tract. The role of colonoscopy assistance to salvage lost cecostomy access in children who fail percutaneous replacement is not well described. The primary aim was to describe the safety and effectiven

Cecostomy. Therapeutic indications and results.

PURPOSE: The role and effectiveness of catheter tube cecostomy as a means of colonic decompression are not clearly defined. Our aim was to clarify the clinical indications, functional performance, and concomitant morbidity associated with tube cecostomy. METHOD: This was a retrospective chart review of patients receiving catheter tube cecostomy at the Mayo Clinic over an 11-year period. RESULTS: Sixty-seven patients (median age, 69 years) had catheter tube cecostomy placement. Clinical indications for tube cecostomy were colonic pseudo-obstruction, distal colonic obstruction, cecal perforati

Cecostomy in children with defecation disorders.

Administration of antegrade enemas through a cecostomy is a therapeutic option for children with severe defecation disorders. The purpose of this study is to report our 4-year experience with the cecostomy procedure in 31 children with functional constipation (n = 9), Hirschsprung's disease (n = 2), imperforate anus (n = 5), spinal abnormalities (n = 8), and imperforate anus in combination with tethered spinal cord (n = 7). Data regarding complications, antegrade enemas used, symptoms, and quality of life were retrospectively obtained. Placement of cecostomy tubes was successful in 30 of 31 pa

Percutaneous endoscopic cecostomy: a case series.

BACKGROUND: There are few reports of percutaneous endoscopic cecostomy in adult patients. METHODS: All cases of acute colonic pseudo-obstruction (n = 2) and neurogenic bowel (n = 3) in adults in which percutaneous endoscopic cecostomy was performed were reviewed retrospectively. OBSERVATIONS: Percutaneous endoscopic cecostomy was a definitive treatment. In 1 of the 2 patients with acute colonic pseudo-obstruction, the percutaneous endoscopic cecostomy tube was clamped and subsequently removed 10 weeks after placement; in the other patient with acute colonic pseudo-obstruction, the percutaneo

[Cecostomy in the treatment of large intestine ileus].

Large bowel decompression by simple coecostomy is not generally accepted. As this procedure is our method of choice in all suitable cases, we have checked the results. From 1.11.1971 to 31.3.1979 a coecostomy was performed in 44 patients presenting with uncomplicated colonic obstruction. The operation was done through an ileocoecal muscle split incision without an exploratory laparotomy. The coecum was primarily opened and sutured to the skin. There were 38 carcinomas, one actinic sigmoid stenosis and one pancreatitic stenosis. In four cases further investigations did not show any mechanical b

The button cecostomy for management of fecal incontinence.

The antegrade continence enema (ACE), administered through a continent cutaneous appendicocecostomy, has been used to successfully achieve reliable evacuation and fecal continence in patients with myelomeningocele. However, as originally described, it is not suitable in patients without an appendix or those requiring urologic reconstruction with appendix. We report the use of a standard gastrostomy button in the cecum to create a continent cecostomy in five patients with fecal incontinence. Four underwent simultaneous urologic reconstruction, including two artificial urinary sphincter (AUS) im

Continent cecostomy. An account of 30 patients.

In this paper, an account is given of our experience with continent colostomy in man. In five patients, the end-sigmoidostomy was provided with an intussusception valve. Evacuation of the bowel by irrigation through a catheter was laborious and time-consuming and this method was abandoned. In another group of 30 patients, the cecum was isolated from the rest of the colon and its distal end was provided with an intussusception valve. Of the 30 patients, eight were later given continent ileostomies, two were converted to conventional sigmoidostomies, and one patient with fecal incontinence prefe

Percutaneous cecostomy: 25-year two institution experience.

BACKGROUND: Reports of technical success, adverse events, and long-term outcome of percutaneous cecostomy in children are limited. OBJECTIVE: To characterize technical success, 30-day severe adverse events, and long-term outcome of percutaneous cecostomy at two centers. MATERIALS AND METHODS: A retrospective review of hospital course and long-term follow-up (through May 2022) of percutaneous cecostomy tubes placed May 1997 to August 2011 at two children's hospitals was used. Outcomes assessed included technical success (defined as successful tube placement into the colon allowing antegrade c

Percutaneous endoscopic cecostomy in adults: a case series.

BACKGROUND: Percutaneous cecostomy is used to treat recurrent colonic pseudoobstruction or obstipation in children and adults with multiple medical comorbidities. Percutaneous endoscopic cecostomy is a potentially attractive alternative to surgical or fluoroscopic cecostomy placement. A few reports describe percutaneous endoscopic cecostomy for management of these problems in children, whereas there are no large series of percutaneous endoscopic cecostomy in adult patients describing the indications, complications, and outcomes. OBJECTIVE: Report our experience with percutaneous endoscopic ce

[Anesthetic managements of the patients receiving catheterizable cecostomy].

To improve the quality of life of patients suffering from spinal cord injury, "Catheterizable cecostomy" operation have recently been carried out. In the procedure, cathetelizable stoma is opened and patients are able to control the excretion by injecting enema solution through the stoma. Although operations are usually performed under general anesthesia with tracheal intubation, special considerations sometimes need to be taken for airway management. In such cases, we select spinal anesthesia. Because the surgery is achieved on a fixed charge, efficiency of pre-operative assessment should be
