Cefdinir-Associated Myopericarditis.

Cefdinir is a third-generation cephalosporin with a wide variety of indications and a strong safety profile. A 24-year-old man began taking cefdinir for an uncomplicated urinary tract infection. Two days later, he began experiencing pleuritic, sharp chest pain and diarrhea and was shown to have mildly elevated troponins. Due to the pleuritic nature of his chest pain improving when leaning forward, as well as elevated troponins and a new small pericardial effusion on echocardiogram, he was diagnosed with myopericarditis thought to be secondary to cefdinir. Cefdinir was subsequently held with di

Five-day cefdinir treatment for streptococcal pharyngitis. Cefdinir Pharyngitis Study Group.

A multicenter, randomized, controlled, investigator-blind study was performed to evaluate the safety and efficacy of oral cefdinir versus oral penicillin V for the treatment of pharyngitis due to group A beta-hemolytic streptococci (GABHS). Patients 13 years of age and older were randomized to receive either oral cefdinir (300 mg twice a day) for 5 days followed by placebo for 5 days or oral penicillin V (250 mg four times a day) for 10 days. Throat cultures were obtained, and signs and symptoms of pharyngitis were recorded at study admission and follow-up visits on study days 11 to 15, 16 to

Comparison of cefdinir and penicillin for the treatment of streptococcal pharyngitis. Cefdinir Pharyngitis Study Group.

Cefdinir, an oral cephalosporin active against Streptococcus pyogenes (group A beta-hemolytic streptococci [GABHS]), is also resistant to degradation by most oropharyngeal beta-lactamases. This multicenter, randomized, controlled, double-masked study assessed the tolerability and efficacy of 2 dosing regimens of cefdinir in the treatment of pharyngitis due to GABHS. Adults and adolescents with pharyngitis due to GABHS received cefdinir 600 mg QD, cefdinir 300 mg BID, or penicillin V 250 mg QID each for 10 days. A throat culture and clinical assessment were obtained 4 to 9 days after completion

Interaction of cefdinir with beta-lactamases.

The interactions of cefdinir, a new orally-active third-generation cephalosporin, with cell-free beta-lactamase preparations were studied in comparison with some other beta-lactams. Cefdinir was very resistant to narrow-spectrum Ambler's class A beta-lactamases, as it was for other oximino beta-lactams: cefotaxime, ceftazidime, cefixime and cefuroxime. Cefaclor showed a low but significant hydrolysis by these beta-lactamases. These class-A enzymes include the widespread plasmid mediated TEM-1, TEM-2, SHV-1 and also the enzymes of Gram-positive penicillinases, such as that produced by S. aureus

The efficacy of cefdinir in acute bacterial rhinosinusitis.

Acute bacterial rhinosinusitis is a common infection resulting in substantial morbidity. Cefdinir, an oral cephalosporin, has extended-spectrum, bactericidal activity against common acute bacterial rhinosinusitis pathogens, including Streptococcus pneumoniae, Haemophilus influenzae and Moraxella catarrhalis. Cefdinir shows rapid oral absorption and good respiratory tissue penetration, and may be administered once daily. In randomised clinical trials, cefdinir showed efficacy similar to that of other recommended regimens in the treatment of acute bacterial rhinosinusitis, namely amoxicillin/cla

Cefdinir-Induced Liver Injury: A Case Report.

Cephalosporins are very rarely known to cause drug-induced liver injury (DILI). We present the case of a healthy 40-year-old female who developed DILI after completing a course of cefdinir, a third-generation cephalosporin, to treat a urinary tract infection. A 40-year-old female with no past medical history presented to the emergency department with a chief complaint of epigastric pain and jaundice for the last four days. A workup at an urgent care revealed hyperbilirubinemia and elevated liver enzymes, prompting this visit. She had completed a five-day course of cefdinir for a urinary tract

Cefdinir-induced hepatotoxicity: potential hazards of inappropriate antibiotic use.

Drug-induced hepatotoxicity is well recognized but can cause some diagnostic problems, particularly if not previously reported. The present case involves a 22-year-old male who presented with jaundice and elevated liver enzymes after a course of cefdinir (Omnicef) for streptococcal pharyngitis. A diagnosis of drug-induced liver injury was suspected but a liver biopsy was required after his jaundice worsened despite cessation of the presumed offending agent.
