Gram-negative bacilli are intrinsically resistant to many antibiotics due to the low permeability of their outer membrane. BACKGROUND: During prolonged FDC therapy, the emergence of FDC non-susceptibility in CRAB has been reported. Here, we report a transmission cluster of FDC-non-susceptible CRAB in four patients, all naïve to FDC treatment, characterized by a premature stop codon and amino acid deletion in the PirA protein.
METHODS: CRAB strains obtained from patients admitted in a single medicine ward of the IRCCS Fondazione Ospedale Maggiore Policlinico between March and July 2024 were analyzed by WGS and antimicrobial susceptibility testing. Phylogenetic analysis was used to assess their genetic relatedness.

We evaluated the in vitro activity of cefiderocol in combination with either taniborbactam or xeruborbactam against cefiderocol-resistant, blaNDM-positive Pseudomonas aeruginosa clinical isolates from Vietnam and Nigeria. Cefiderocol is a siderophore cephalosporin designed mainly for treatment of infections caused by β-lactam and multidrug-resistant Gram-negative bacteria. Burkholderia pseudomallei clinical isolates are usually highly cefiderocol susceptible, with in vitro resistance found in a few isolates. Resistance in clinical B. pseudomallei isolates from Australia is caused by a hitherto uncharacterized mechanism. BACKGROUND: Bacterial ventriculitis associated with CSF shunts or drains is a life-threatening infection often caused by MDR pathogens such as Stenotrophomonas maltophilia. Cefiderocol, an IV siderophore cephalosporin approved for drug-resistant Gram-negative infections, lacks extensive clinical data supporting its use in CNS infections.
