Bacterial Infections of the Central Nervous System.

Acute bacterial meningitis and spinal epidural abscess are neurological emergencies. Acute bacterial meningitis may present with symptoms as nonspecific as headache and fever, but rapid progression to an altered level of consciousness is not unusual. Spinal epidural abscess manifests initially as back pain, followed by radicular pain, then weakness, and finally paraplegia. Brain abscess may initially present only with headache, or as a new-onset seizure or with a focal neurological deficit. Bacterial infections of the central nervous system require emergent diagnosis and management. In this ar

Bacterial Infections of the Central Nervous System.

Bacterial infections of the central nervous system (CNS) continue to be an important cause of morbidity and mortality in children. The spectrum of bacterial infection of CNS includes; focal or multifocal infections like brain abscesses or subdural empyema; or more generalized or diffuse infections like pyogenic meningitis or ventriculitis. Focal and generalized infections may co-exist in an individual patient. Prompt and adequate antibiotic therapy and occasionally neurosurgical interventions are the cornerstone of effective management. The recent emergence of several multidrug-resistant bacte

Management of bacterial central nervous system infections.

Bacterial infections of the central nervous system present as a medical emergency, thus requiring rapid diagnosis and immediate treatment. The most prevalent bacterial infections seen in the intensive care unit can be summarized as acute bacterial meningitis, subdural empyema, intracerebral abscess, and ventriculitis, which all commonly involve the brain parenchyma. The infections can either be community-acquired or hospital-acquired, e.g., after neurosurgical intervention, as a complication of severe neurotrauma or related to indwelling cerebrospinal fluid drains. Community-acquired bacterial

Bacterial Infections of the Central Nervous System.

PURPOSE OF REVIEW: Bacterial infections of the central nervous system are neurologic emergencies. Prompt recognition and treatment are essential not only to prevent mortality, but also to decrease neurologic sequelae. This article focuses on the two most common central nervous system bacterial infections, bacterial meningitis and spinal epidural abscess. RECENT FINDINGS: Two outbreaks of serogroup B meningococcal disease have occurred on US college campuses. The meningococcal vaccine given to young adults does not contain serogroup B. SUMMARY: In bacterial meningitis and in bacterial spinal

Nosocomial bacterial meningitis, including central nervous system shunt infections.

Nosocomial bacterial meningitis and CSF shunt infections result in considerable morbidity and mortality, necessitating an organized and thoughtful approach to prevention, diagnosis, and management. Prophylactic antibiotics appear to reduce the rate of postcraniotomy meningitis often caused by S. aureus. On the other hand, prophylactic antibiotics do not appear to reduce the risk of developing a CSF shunt infection. CSF shunt infections usually require shunt removal and antimicrobial chemotherapy to effect a successful outcome.

Treatment of bacterial infections of the central nervous system.

Antimicrobial agents for the treatment of bacterial meningitis and other pyogenic intracranial infections have now been in use for approximately 45 years. The last decade, however, has been a period of tremendous advances in knowledge of the pharmacokinetics and other characteristics of the available drugs. The identification of factors that affect their in vivo metabolism and excretion, the importance of drug interactions, and the development of methods for estimation of serum and cerebrospinal fluid levels have added both complexity and sophistication to the clinical utilization of the antim

Molecular mechanisms of bacterial infections of the central nervous system.

Central nervous system (CNS) infections may involve the meninges, brain and/or spinal cord. The most common etiologic agents are Streptococcus pneumoniae, group B Streptococci, Neisseria meningitidis, Haemophilus influenzae, and Listeria monocytogenes. CNS is characterized by specific structure and function. Despite a unique system of brain barriers and autonomous immune system, CNS is very susceptible to microorganisms which may invade directly, via the blood, or less frequently by reverse axonal transport. The complex process of bacteria and activated polymorphonuclear leukocyte transfer to

Optimal antibiotic therapy for bacterial central nervous system infections in adults.

INTRODUCTION: Central nervous system (CNS) infections are a health concern, leading to high morbidity and mortality. Community-acquired and nosocomial meningitis are distinct entities with potentially different pathogens involved. Prompt antibiotic therapy is crucial. However, challenges arise due to the emergence of multidrug-resistant bacteria and the poor CNS penetration of most antibiotics. AREAS COVERED: This review summarizes the pathogenesis of bacterial CNS infections, the pharmacokinetics, and pharmacodynamics of several classes of antibiotics within the cerebrospinal fluid (CSF) and

Bacterial colonisation and infection in lesions of the central nervous system.

A study was made of the pattern of bacterial colonisation during the first month of life in a group of 116 newborn infants with spina bifida. 55 patients subsequently developed a total of 90 infective episodes and in over three-quarters of these episodes there was a correlation between the infecting and colonising organisms. The principal sites of infection were the lesion, the ventricles and the urinary tract, and infection accounted for 27 of the 35 deaths in the series. Some of the factors governing the pattern of colonisation and the development of infection in newborn patients are discuss

Early diagnostic strategy for central nervous system bacterial infections after neurosurgery: A retrospective study.

Accurate diagnosis of post-neurosurgical bacterial infection of central nervous system is challenging due to the nonspecific nature of clinical signs and cerebrospinal fluid (CSF) parameters, which often overlap with sterile postoperative inflammation. This study aimed to develop and validate a stepwise diagnostic strategy integrating readily available clinical and basic laboratory indicators to improve the early identification of post-neurosurgical bacterial infection of the central nervous system. A retrospective cohort study was conducted at Tianjin Huanhu Hospital, a tertiary neurosurgical

Kynurenine Is a Cerebrospinal Fluid Biomarker for Bacterial and Viral Central Nervous System Infections.

BACKGROUND: The tryptophan-kynurenine-nicotinamide adenine dinucleotide (oxidized; NAD+) pathway is closely associated with regulation of immune cells toward less inflammatory phenotypes and may exert neuroprotective effects. Investigating its regulation in central nervous system (CNS) infections would improve our understanding of pathophysiology and end-organ damage, and, furthermore, open doors to its evaluation as a source of diagnostic and/or prognostic biomarkers. METHODS: We measured concentrations of kynurenine (Kyn) and tryptophan (Trp) in 221 cerebrospinal fluid samples from patients

[Autochthonous acute viral and bacterial infections of the central nervous system (meningitis and encephalitis)].

Rapid diagnosis of acute viral and bacterial infections of the central nervous system (meningitis and encephalitis) is highly important for the clinical management of the patient and helps to establish early therapy that may solve life-threatening situations, to avoid unnecessary empirical treatments, to reduce hospital stay, and to facilitate appropriate interventions in the context of public health. Molecular techniques, especially real-time polymerase chain reaction, have become the fastest and most sensitive diagnostic procedures for autochthonous viral meningitis and encephalitis, and the

The uselessness of procalcitonin in the diagnosis of focal bacterial central nervous system infection.

We investigated the utility of procalcitonin in early diagnosis of bacterial central nervous system (CNS) infection. Serum procalcitonin level was markedly elevated in the patients with systemic meningitis but not in the patients with brain abscess and subdural empyema. Procalcitonin may be useless to diagnose focal bacterial CNS infection.

Typhus group Rickettsia community-acquired bacterial central nervous system infections: We must think outside the box!

Typhus group rickettsiosis (TGR), caused by Rickettsia typhi and Rickettsia prowazekii, are globally distributed vector-borne diseases with increasing cases. Diagnosis is usually clinical, confirmed by seroconversion of IgG antibodies. Human infection occurs in diverse geographic areas with some developing CNS infection characterized by fever, headache, meningismus, and/or focal signs - usually beyond the first week of initial symptomatology. Seizures and other CNS manifestations have been observed. When untreated, infection may result in neurologic sequelae and even death. This study presents
