Central Nervous System Infections.

Infections of the nervous system are potential life-threatening and are caused by pathogens such as bacteria, viruses, and fungi. Prompt recognition and treatment of a central nervous system (CNS) infection is crucial for patient survival, as these infections have a high morbidity and mortality. CNS infections include meningitis, encephalitis, and brain abscesses. This article seeks to detail the etiology, clinical course, diagnostic challenges, and treatment of CNS infections organized by infectious agent.

Central Nervous System Opportunistic Infections.

Opportunistic infections of the central nervous system are classically associated with immunosuppression arising from infection with human immunodeficiency virus and with various hematologic malignancies. However, over the past few years, they are increasingly associated with transplantation and various immunosuppressive treatments used to treat autoimmune diseases. They cause significant morbidity and mortality and remain a diagnostic challenge due to the absence of typical signs and symptoms of infection and mimicry by various noninfectious causes. The pathogens associated with these infecti

[Central nervous system mycotic infections].

INTRODUCTION: There has been an increased number of cases of fungal diseases of the central nervous system (CNS) during the past few years. This paper reviews current literature about these conditions, with emphasis on recent advances on diagnosis and therapy. DEVELOPMENT: While some fungi may cause disease in normal hosts, most of these microorganisms are opportunistics and affect immunocompromised hosts. With the exception of Candida albicans, that is a normal inhabitant of the intestinal tract, most fungi enter the body by inhalation or through skin abrasions. Common fungal diseases includ

Central nervous system opportunistic infections.

The spectrum of opportunistic infections occurring in association with human-immunodeficiency virus, type 1, is very broad. These infections develop most frequently in the setting of advanced immunosuppression. There is no part of the neuraxis that is immune to these complications and the concurrence of more than one infectious illness may always be considered. The neuroimaging features, when coupled with the clinical and laboratory findings, often suggest the correct diagnosis and enable the physician to initiate therapy.

Healthcare-associated central nervous system infections.

PURPOSE OF REVIEW: The aim of this review is to provide a practical and updated summary on healthcare-associated central nervous system infections and their management. RECENT FINDINGS: The term 'healthcare-associated ventriculitis and meningitis' has recently been coined and clinical practical guidelines have been published on the management of these nosocomial infections. Many aspects have still to be further investigated (e.g. cerebrospinal fluid biomarkers, indications for novel antibiotics, intrathecal antimicrobial regimens). SUMMARY: Clinicians should maintain a high index of suspicio

Stroke and central nervous system infections.

Stroke is the first cause of disability in adults worldwide. Central nervous system infections as a cause of stroke are not uncommon. The common central nervous system infections associated with stroke can be categorised into viral, bacterial, parasitic and of fungal origin. In HIV infection, central nervous system involvement is a major reason for hospitalisation and morbidity. Varicella-zoster virus encephalitis is uncommon and neurological features are protean. Japanese encephalitis is the most common cause of endemic encephalitis in humans. Viral haemorrhagic diseases are common in the tro

Infections of central nervous system shunts.

Approximately 1 out of every 10 ventricular shunts for hydrocephalus will become infected. This represents a three- to fivefold increase in infection rate compared with other neurosurgical procedures. Most often, the infection results from colonization of the shunt device by normally nonpathogenic skin flora at the time of surgery. Properties of the foreign body itself may contribute to this increased risk of infection, and most infections are clinically apparent within the first 6 months following surgery. Meticulous surgical technique remains one of the most important variables in reducing s

[Immunodeficiency and Central Nervous System Infections].

Central nervous system (CNS) infections are serious and potentially life-threatening complications in patients with immunodeficiency. Predisposing conditions include organ transplant, hematopoietic stem cell transplantation, Human Immunodeficiency Virus (HIV) infection, and treatment with steroids, immunosuppressive agents, anti-complement therapy, or anti-B cell therapy. Neurologists must anticipate opportunistic and severe infection in these settings and closely monitor the clinical course. This review focuses on the clinical information and management of CNS infections in organ transplant a

Neuroimaging of Central Nervous System Infections.

Neuroimaging plays a key role in the diagnosis of central nervous system (CNS) infections, as well as common infectious mimics. Standard imaging techniques such as computed tomography (CT) and magnetic resonance imaging (MRI), as well as advanced techniques such as vessel wall imaging and MR spectroscopy, are becoming widely used in many areas of the world and are valuable tools to aid neurologists in clinical practice. This review seeks to elucidate patterns of infectious pathogen tropism in the brain and spine, detail key imaging features of specific neuroinfectious pathogens such as pattern

Stroke in central nervous system infections.

BACKGROUND: Stroke subtypes and etiology may differ between developing and developed countries. Infections are a relatively common cause of stroke in tropical regions. OBJECTIVE: To review the main infectious diseases associated with stroke. DISCUSSION: Prevalence of stroke in HIV patients is around 1%. Pathogenic mechanisms include HIV vasculopathy, vasculitis, cardioembolism, acquired hypercoagulability, and the effect of opportunistic infections. Treatment with protease inhibitors has been associated with premature atherosclerotic vascular disease. Emerging viral infections that are assoc

Central nervous system infections in transplantation.

Central nervous system (CNS) infections are a rare but serious complication of transplantation. An appropriate diagnostic workup should be streamlined with stratification of infection risk by length and degree of immune suppression, lesion localization, and timing. Polymerase chain reaction has high sensitivity and specificity for rapid identification of viral infections and should be used when available. Early diagnosis is imperative, and biopsy should be pursued if a diagnosis is not readily obtained with noninvasive testing. Treatment is pathogen specific. Combination antifungal therapy sho

Infections of the central nervous system.

Several discussions of the neuroimaging of inflammatory diseases with emphasis on computerized tomography (CT) and magnetic resonance imaging (MRI) findings and on rare diseases, such as sparganosis, neuroborreliosis and various types of encephalitis, have been published during the past year. Infectious processes in immunosuppressed patients have been described by many authors, especially in the field of acquired immune deficiency syndrome (AIDS). The importance of gadolinium-diethylenetriamine-penta-acetic acid (Gd-DTPA) in MRI for delineation, and differential diagnosis of the inflammatory p

Infections of the central nervous system.

Infections of the central nervous system are common, serious medical conditions. One hundred consecutive adult cases with purulent meningitis of known etiology encountered by the Medical Service at Parkland Memorial Hospital were reviewed. Streptococcus pneumoniae was the most common pathogen (56 cases), followed by Neisseria meningitidis (16 cases) and Listeria monocytogenes (seven cases). Hemophilus influenzae, Staphylococcus aureus, and streptococci each accounted for five cases. An additional 15 patients had purulent meningitis with a pathogen being isolated. Twenty five purulent meningiti

Infections of the central nervous system.

Acquired immunodeficiency syndrome has had a major impact on all aspects of clinical neuroscience in the past few years. Thus understanding acquired immunodeficiency syndrome and its associated complications within the nervous system was a major component of this review of the past year's radiologic literature of infection in the central nervous system. Particular topics discussed include the appearance of primary human immunodeficiency virus infection of the brain as well as toxoplasmosis, cryptococcal infection, and progressive multifocal leukoencephalopathy. In addition, the association of

Corticosteroids for viral central nervous system infections.

PURPOSE OF REVIEW: Viruses are frequent causes of central nervous system (CNS) infection. Lacking specific antiviral treatment or inadequate clinical response may lead to treatment with corticosteroids. This review describes the rationale for and clinical experience with the use of adjunctive corticosteroids for viral CNS infections. RECENT FINDINGS: Corticosteroids display anti-inflammatory, immunosuppressive, antiproliferative, and vasoconstrictive effects by genomic and nongenomic regulation of human cells. Recent population-based studies consistently show that empiric dexamethasone during
