Maternal height and age: risk factors for cephalopelvic disproportion in Zimbabwe.

Maternal age and stature are among several factors used to screen pregnant women for potential risk of labour complications. In a population-based case-control study in Harare, Zimbabwe, multivariate analysis was carried out to evaluate the importance of maternal age and height as risk factors for cephalopelvic disproportion (CPD). Using data abstracted from the medical records of 203 women with operative deliveries due to CPD and 299 women with normal unassisted vaginal deliveries, multiple logistic regression models were developed. Although maternal age < 18 years was not a significant risk

Childbirth simulation to assess cephalopelvic disproportion and chances for failed labor in a French population.

Reducing failed labor and emergency cesarean section (CS) rates is an important goal. A childbirth simulation tool (PREDIBIRTH software and SIM37 platform) that evaluates a 5-min magnetic resonance imaging (MRI) assessment performed at 37 weeks of gestation was developed to enhance the consulting obstetrician's ability to predict the optimal delivery mode. We aimed to determine the potential value of this childbirth simulation tool in facilitating the selection of an optimal delivery mode for both mother and infant. A retrospective cohort study was performed on all patients referred by their o

Risk factors and perinatal outcome of pregnancies complicated with cephalopelvic disproportion: a population-based study.

OBJECTIVES: To characterize risk factors and perinatal outcome following cephalopelvic disproportion (CPD). METHODS: A retrospective population-based study comparing all singleton deliveries of women with and without CPD, between 1988 and 2010, was conducted. A multiple logistic regression model was used to control for confounders. RESULTS: Out of 242,520 patients, 0.3% (n = 673) were diagnosed with CPD. Using a multivariable analysis, the following obstetric risk factors were significantly associated with CPD: fetal macrosomia (birth weight above 4 kg, OR = 3.3, 95% CI 2.7-4.1, P < 0.001),

Evaluation of the natural birth prediction system for assessing cephalopelvic disproportion: a multicentre prospective study.

OBJECTIVES: To assess the potential value of the Natural birth prediction system (NBPS) on predicting cephalopelvic disproportion (CPD) and compare the value with a pelvimetry model and the foetal-pelvic index. METHODS: A multicentre prospective study was conducted between November 2023 and September 2024 in three hospitals in China. Singleton nulliparas suspected with CPD and willing to attempt vaginal delivery were included. Patients were categorised according to NBPS results (absence CPD, borderline CPD, and presence CPD). The software results were compared with actual delivery modes. Rece

Prelabor rupture of membranes at term requiring labor induction - a feature of occult fetal cephalopelvic disproportion?

AIM: To examine the maternal and neonatal characteristics related to labor outcome following induction for prelabor rupture of membranes (PROM) after 37 weeks' gestation. METHOD: All singleton pregnancies delivered following spontaneous or induced labor after 37 weeks in four years were analyzed. Patients with labor induction for PROM comprised the study group. Outcome of labor was analyzed according to gestational age at delivery. RESULTS: Among the 13,505 patients in the entire cohort, 1710 (12.7%) had labor induction for PROM. These women were older and heavier, were more nulliparas and h

An Evolutionary Quantitative Genetic Analysis of the Impact of Cephalopelvic Disproportion on Cranial and Pelvic Co-Evolution in Anthropoids.

OBJECTIVES: Evolutionary quantitative genetics methods are increasingly applied to studies of human skeletal evolution, with a growing emphasis on investigating postcranial evolution and the evolution of multiple skeletal elements. Here, we apply a commonly used method from evolutionary quantitative genetics, the drift-rate test, to test whether broad patterns of cranial and pelvic co-evolution within male, female, and pooled-sex samples of anthropoid primates follow those expected under the long-standing hypotheses of obstetric selection via cephalopelvic disproportion. Head circumference, as predictor of cephalopelvic disproportion: A prospective analysis of cases of spontaneous vaginal delivery and caesarean section in Ekiti State, Nigeria.

Cephalopelvic disproportion (CPD) is a previously undiagnosed anatomical misfit between maternal pelvis and the fetal head. It is one of the major indications for cesarean section (CS), especially in sub-Saharan Africa. Early diagnosis, could avert events that can increase maternal and perinatal morbidity and mortality associated with this condition. This study was designed to determine the mean head circumference of the fetus in relation to CPD as an indicator for caesarean section. A total of 350 parturients who had spontaneous vaginal deliveries (group A) were compared with another 350 part

Indications for primary cesarean delivery relative to body mass index.

BACKGROUND: Obesity is a known risk factor for cesarean delivery. Limited data are available regarding the reasons for the increased rate of primary cesarean in obese women. It is important to identify the factors leading to an increased risk of cesarean to identify opportunities to reduce the primary cesarean rate. OBJECTIVE: We evaluated indications for primary cesarean across body mass index (kg/m(2)) classes to identify the factors contributing to the increased rate of cesarean among obese women. STUDY DESIGN: In the Consortium of Safe Labor study from 2002 through 2008, we calculated in

Indications for Caesarean sections in a rural hospital in the Highlands of Papua New Guinea.

We retrospectively documented indications for Caesarean sections in a rural district level hospital in the highlands of Papua New Guinea. Over a 53-month study period, 745 Caesarean sections were performed. Prolonged labour, previous history of Caesarean section, cephalopelvic disproportion, malpresentation and fetal distress accounted for over 88% of Caesarean sections performed. In older mothers (aged >30 years), antepartum haemorrhage (Fisher exact test, P = 0.05) and multiple indications (P = 0.001) were leading reasons for Caesarean sections while cephalopelvic disproportion (P = 0.005) w

Cesarean section deliveries among adolescent mothers enrolled in a comprehensive prenatal care program.

A comparison was made of the incidences of cesarean section deliveries between mothers 18 years of age or younger and those 19 to 21 years of age, all of whom were enrolled in a comprehensive program of prenatal care. Also compared were the indications for surgical delivery between the two groups. The rate of cesarean section deliveries was 8.7% among mothers 18 years of age or younger, compared to 14.5% among mothers 19 to 21 years of age. The general-ward cesarean section rate at the same hospital was 19.4%. Since maternal height and weight, and infant's birth weight may affect the diagnosis

Evolution of the human pelvis and obstructed labor: new explanations of an old obstetrical dilemma.

Without cesarean delivery, obstructed labor can result in maternal and fetal injuries or even death given a disproportion in size between the fetus and the maternal birth canal. The precise frequency of obstructed labor is difficult to estimate because of the widespread use of cesarean delivery for indications other than proven cephalopelvic disproportion, but it has been estimated that at least 1 million mothers per year are affected by this disorder worldwide. Why is the fit between the fetus and the maternal pelvis so tight? Why did evolution not lead to a greater safety margin, as in other

Principle of biological adaptation: Evolutionary adaptations of the female pelvis to bipedalism as the main cause for fetopelvic disproportion.

Throughout human evolution, changes in the size and shape of the pelvis have occurred in connection with bipedalism. These changes have become genetically fixed in the ontogeny of modern female Homo sapiens. The typological classification of pelvic shapes and sizes in women was examined, demonstrating the significance of pelvic structural features in influencing the course of labor. The application of modern pelvic examination methods for planning management strategies and predicting the outcome of natural childbirth was also discussed. Several recent studies on pelvic mobility during pregnanc

Maternal and neonatal outcomes in pregestational and gestational diabetes mellitus, and the influence of maternal obesity and weight gain: the DEPOSIT study. Diabetes Endocrine Pregnancy Outcome Study in Toronto.

We prospectively studied pregnancy outcome in 428 women with gestational diabetes mellitus (DM) and 196 women with pregestational DM, with particular reference to the influence of maternal obesity and excessive weight gain. These were consecutive singleton pregnancies delivered in our institution over 5 years. After controlling for multiple risk factors, including maternal BMI and pregnancy weight gain, women with pregestational DM were at increased risk (compared to those with gestational DM) for Caesarean delivery (OR 3.6, 95%CI 2.3-5.6), shoulder dystocia or cephalopelvic disproportion (OR

Abnormal pelvic morphology and high cervical length are responsible for high-risk pregnancies in women displaying achondroplasia.

Pregnancies of women displaying achondroplasia are at high risk of adverse events. Early sonographic assessment of affected women can indicate an unusually long cervical length. It is the consequence of pathological anatomy of the pelvis. Thus, there is a foreseeable dystocia owing to cephalopelvic disproportion. Furthermore, this situation could also complicate cervical ripening prior to fetal extraction.
