Cerebellar cognitive affective syndrome.

There has been persistent uncertainty as to whether lesions of the cerebellum are associated with clinically significant disturbances of behavior and cognition. To address this question, 20 patients with diseases confined to the cerebellum were studied prospectively over a 7-year period and the nature and severity of the changes in neurological and mental function were evaluated. Neurological examination, bedside mental state testing, neuropsychological studies, and anatomic neuroimaging were administered at the time of presentation and during follow-up assessments. Behavioral changes were cli

The cerebellar cognitive affective syndrome.

Anatomical, physiological and functional neuroimaging studies suggest that the cerebellum participates in the organization of higher order function, but there are very few descriptions of clinically relevant cases that address this possibility. We performed neurological examinations, bedside mental state tests, neuropsychological studies and anatomical neuroimaging on 20 patients with diseases confined to the cerebellum, and evaluated the nature and severity of the changes in neurological and mental function. Behavioural changes were clinically prominent in patients with lesions involving the

Cerebellar cognitive affective syndrome after acute cerebellar stroke.

INTRODUCTION: The cerebellum modulates both motor and cognitive behaviors, and a cerebellar cognitive affective syndrome (CCAS) was described after a cerebellar stroke in 1998. Yet, a CCAS is seldom sought for, due to a lack of practical screening scales. Therefore, we aimed at assessing both the prevalence of CCAS after cerebellar acute vascular lesion and the yield of the CCAS-Scale (CCAS-S) in an acute stroke setting. MATERIALS AND METHODS: All patients admitted between January 2020 and January 2022 with acute onset of a cerebellar ischemic or haemorrhagic first stroke at the CUB-Hôpital E

Cerebellar cognitive affective syndrome (Schmahmann syndrome) following isolated cerebellar infarction.

The cerebellum plays an important role in higher-order cognition through cerebrocerebellar networks. Lesions affecting this structure may result in cerebellar cognitive affective syndrome (CCAS), which is frequently underrecognized by routine cognitive screening tools. We reported a 69-year-old right-handed man who presented with 18 months of forgetfulness, impaired concentration, and irritability following a documented left cerebellar infarction. Neurological examination revealed cerebellar signs without cortical deficits. Brain magnetic resonance imaging (MRI) demonstrated a left cerebellar

The cerebellar cognitive affective/Schmahmann syndrome scale.

Cerebellar cognitive affective syndrome (CCAS; Schmahmann's syndrome) is characterized by deficits in executive function, linguistic processing, spatial cognition, and affect regulation. Diagnosis currently relies on detailed neuropsychological testing. The aim of this study was to develop an office or bedside cognitive screen to help identify CCAS in cerebellar patients. Secondary objectives were to evaluate whether available brief tests of mental function detect cognitive impairment in cerebellar patients, whether cognitive performance is different in patients with isolated cerebellar lesion

Cerebellar cognitive-affective syndrome in Friedreich Ataxia.

BACKGROUND: Increasing evidence suggests that cognition and affect may be affected in Friedreich Ataxia (FA). OBJECTIVES: To investigate the Cerebellar Cognitive Affective Syndrome (CCAS) in patients with FA and evaluate its correlations with speech and clinical parameters. METHODS: Patients were recruited from the PROFA study (NCT05943002), a prospective observational cohort in France, Germany, and Austria. Assessments included clinical scales (Friedreich Ataxia Rating Scale-Activities of Daily Living, FARS-ADL, Scale for the Assessment and Rating of Ataxia), the CCAS scale, and patient-rep

The Cerebellar Cognitive Affective Syndrome in Ataxia-Telangiectasia.

Ataxia-telangiectasia (AT) is an autosomal recessive, multisystem disease causing cerebellar ataxia, mucocutaneous telangiectasias, immunodeficiency, and malignancies. A pilot study reported cognitive and behavioral manifestations characteristic of the cerebellar cognitive affective / Schmahmann syndrome (CCAS). We set out to test and further define these observations because a more comprehensive understanding of the spectrum of impairments in AT is essential for optimal management. Twenty patients (12 males; 9.86 ± 5.5 years, range 4.3 to 23.2) were grouped by age: AT-I (toddlers and preschoo

Treatment of cerebellar cognitive affective syndrome with aripiprazole.

This report describes a patient who developed agitation, disorientation, visual hallucinations, inappropriate verbal outbursts, and impaired memory following resection of a choroid plexus papilloma. No medical, neurologic, or metabolic disorders unrelated to the surgery were identified. Five weeks following surgery, treatment with aripiprazole, a partial dopamine agonist, was started to address the delirious state. Improvements in agitation, orientation, memory, and executive functions, as well as a decrease in emotional lability, began within twenty-four hours and continued over the remainder

The cerebellar mutism syndrome and its relation to cerebellar cognitive function and the cerebellar cognitive affective disorder.

The postoperative cerebellar mutism syndrome (CMS), consisting of diminished speech output, hypotonia, ataxia, and emotional lability, occurs after surgery in up to 25% of patients with medulloblastoma and occasionally after removal of other posterior fossa tumors. Although the mutism is transient, speech rarely normalizes and the syndrome is associated with long-term adverse neurological, cognitive, and psychological sequelae. The clinical, neuroradiographic, and neuropsychological findings associated with CMS as well as possible mechanisms of injury are reviewed. Theories about the pathophys

Cerebellar Cognitive Affective Syndrome in Spinocerebellar Ataxia Type 6.

BACKGROUND: Spinocerebellar ataxia type 6 (SCA6) has traditionally been classified as a "pure" cerebellar syndrome; however, converging evidence points to non-motor involvement. Characterization of its cognitive-affective profile remains limited, constraining the understanding of its full clinical spectrum. OBJECTIVES: To investigate non-motor manifestations in SCA6 and contribute to a more comprehensive characterization of its cognitive-affective profile through standardized neuropsychological assessment. METHODS: A monocentric, cross-sectional case-control study included nine patients with

Bipolar Camouflage: A Cerebellar Cognitive Affective Syndrome Case Report.

The cerebellar cognitive affective syndrome is a neuropsychiatric syndrome composed of affective (anxiety, depression, euphoria, and emotional lability) and cognitive symptoms (executive, attentional, and visuospatial deficits) that was described in the 1990s. We present the case of a 49-year-old woman with a history of an acute neurological episode at the age of 28, after which she reported a change in personality, brief and alternating periods of depression, hypomania, and mixed episodes, and cognitive impairment that had a major impact on her personal and occupational level of functioning.

Post-infarct cerebellar cognitive affective syndrome: a case report.

BACKGROUND: EPB41L1 gene (erythrocyte membrane protein band 4.1 like 1) encodes the protein 4.1N, a member of 4.1 family, playing a vital role in cell adhesion and migration, which is associated with the malignant progression of various human cancers. However, the expression and prognostic significance of EPB41L1 in kidney renal clear cell carcinoma (KIRC) remain to be investigated. METHODS: In this study, we collected the mRNA expression of EPB41L1 in KIRC through the Oncomine platform, and used the HPA database to perform the pathological tissue immunohistochemistry in patients. Then, the s

The Cerebellar Cognitive Affective Syndrome in Essential Tremor Plus.

The latest tremor classification suggested to stratify patients with Essential Tremor (ET) into pure (pET) and plus (ET-plus) forms. Cognitive dysfunction in ET might reflect the Cerebellar Cognitive Affective Syndrome (CCAS), but there is no evidence in ET-plus. We aimed to evaluate the CCAS in ET-plus, further attempting to explore possible motor-cognitive associations.Thirty-nine patients with ET-plus and 19 matched healthy controls (HC) performed the CCAS-scale (CCAS-S). Patients were also assessed using the Tremor Research Group Essential Tremor Rating Scale and the Scale for the Assessme

Mapping the Cerebellar Cognitive Affective Syndrome in Patients with Chronic Cerebellar Strokes.

The cerebellar cognitive affective syndrome (CCAS) has been consistently described in patients with acute/subacute cerebellar injuries. However, studies with chronic patients have had controversial findings that have not been explored with new cerebellar-target tests, such as the CCAS scale (CCAS-S). The objective of this research is to prove and contrast the usefulness of the CCAS-S and the Montreal Cognitive Assessment (MoCA) test to evaluate cognitive/affective impairments in patients with chronic acquired cerebellar lesions, and to map the cerebellar areas whose lesions correlated with dys

Developmental cerebellar cognitive affective syndrome in ex-preterm survivors following cerebellar injury.

Cerebellar injury is increasingly recognized as an important complication of very preterm birth. However, the neurodevelopmental consequences of early life cerebellar injury in prematurely born infants have not been well elucidated. We performed a literature search of studies published between 1997 and 2014 describing neurodevelopmental outcomes of preterm infants following direct cerebellar injury or indirect cerebellar injury/underdevelopment. Available data suggests that both direct and indirect mechanisms of cerebellar injury appear to stunt cerebellar growth and adversely affect neurodeve

Perioperative Assessment of Cerebellar Masses and the Potential for Cerebellar Cognitive Affective Syndrome.

The cerebellum was long perceived to be a region of limited importance with primary functions in the regulation of motor control. A degree of its functional topography in motor modulation has been traditionally appreciated. However, an evolving body of evidence supports its role in a range of cognitive processes, including executive decision making, language, emotional processing, and working memory. To this end, numerous studies of cerebellar stroke syndromes as well as investigations with functional magnetic resonance imaging and diffusion tensor imaging have given clinicians a better model

The cerebellar cognitive affective syndrome scale in early multiple sclerosis.

BACKGROUND: Cerebellar posterior lobes regulate various cognitive functions, and their damage can result in Cerebellar Cognitive Affective Syndrome (CCAS). The validity of the CCAS Scale (CCAS-S), a reliable tool to diagnose CCAS, remains unexplored in multiple sclerosis (MS). OBJECTIVES: To evaluate the performance of CCAS-S in patients with MS (pwMS) at clinical onset, and to assess MRI characteristics of pwMS with CCAS (CCAS+) compared with healthy controls (HC) and with pwMS classified by standard cognitive assessments as cognitively impaired (CI+) or cognitively normal (CI- CCAS-).
