OBJECTIVES: Dysplastic gangliocytoma (Lhermitte-Duclos disease) is a rare disorder, characterized by a slowly progressive unilateral tumour mass of the cerebellar cortex. The fundamental nature of this apparently benign entity and in particular its pathogenesis remain unknown. The debate, whether it represents a neoplastic, malformative or hamartomatous lesion, is still in progress. Lhermitte-Duclos disease was recently encountered to be part of a multiple hamartoma-neoplasia complex (Cowden's syndrome).
METHODS: The present account gives a review of the pertinent literature with emphasize on

A Developmental Gerstmann syndrome in a 7 years-old-boy with hyperactivity, short attention span, acalculia, agraphia, right-left confusion, finger agnosia and constructional apraxia is reported. An initial trial with methylphenidate was done with good improvement regarding hyperactivity, attention span and constructional apraxia. In the clinical course developed a cerebellar syndrome and intracranial hypertension. Surgical exploration of the cerebellum discovered a medulloblastoma. The author believes that this is the first described association of Developmental Gerstmann syndrome and cerebel

Association of developmental venous anomaly (DVA) and cavernous malformation (CM) of the posterior fossa is rare. If bleeding occurs, it must be assigned to the CM. Developmental venous anomalies, considered as normal vascular variation, are rarely symptomatic. Indeed, the CMs are true malformations and have a high bleeding potential.

BACKGROUND: Thrombosed intracranial aneurysms are rare in pediatric populations and pose significant diagnostic challenges due to their atypical imaging features, which often mimic neoplasms. These lesions can present with mass effect, contrast enhancement, and calcifications, leading to potential misdiagnosis and inappropriate management. Accurate differentiation through advanced imaging is essential to prevent surgical complications and ensure optimal outcomes.
OBSERVATIONS: The authors describe the case of a 15-year-old boy with severe acute headaches and an MRI finding of a contrast-enhan

OBJECT: Pediatric cerebellar astrocytomas with pilomyxoid features include classic pilomyxoid astrocytomas (PMAs) and intermediate pilomyxoid tumors (IPTs). Since the original description of PMA in 1999, most reports in the literature have described PMAs arising from the hypothalamic/chiasmatic region. To the authors' knowledge, PMAs arising from the posterior fossa have not been discussed in the neurosurgical literature. Intermediate pilomyxoid tumors, or tumors with pathological features of both pilocytic astrocytoma (PA) and PMA, have only recently been described. In this article, the autho

BACKGROUND: L-2-hydroxyglutaric aciduria (L2HGA) is a rare neurometabolic disorder characterized by a slowly progressive clinical course, psychomotor and mental retardation, macrocephaly, dysarthria, seizures, and cerebellar and extrapyramidal findings. The diagnosis depends on the presentation of increased levels of L-2-hydroxyglutaric acid in the urine, plasma, and cerebrospinal fluids. Patients with L2HGA have an increased risk for the development of cerebral neoplasms which, though rarely, can be the initial presentation of the disease. Moreover, patients with L2HGA have an increased risk

Cerebellar liponeurocytoma is a rare neoplasm with distinctive morphologic features. It typically involves the cerebellar hemispheres of middle-aged to older adults. The tumor is composed of a uniform population of neurocytic cells possessing round to oval nuclei and pale to clear cytoplasm. A variable degree of lipidization of the tumor cells is present, lending a resemblance to mature adipose tissue. Immunohistochemistry serves to confirm the neurocytic differentiation of the tumor cells. In the 2007 revision of the World Health Organization classification of central nervous system tumors, c

Several groups of investigators have confirmed the occurrence of antibodies to Purkinje and other cerebellar neuronal populations in the serum and spinal fluid of patients with paraneoplastic cerebellar degeneration. Although this antibody response suggests that paraneoplastic cerebellar degeneration may have an autoimmune basis, it is not known what role anticerebellar antibodies play in the pathogenesis of this disorder or whether the presence of antibodies invariably results in cerebellar injury. We identified 3 patients with ovarian malignancies in whom high titers of circulating anticereb

Von Hippel-Lindau (VHL) is hereditary syndrome characterized with benign and malignant neoplasms resulting from dysregulation of hypoxia-sensitive pathway. Fifty-year-old man with VHL underwent multimodal imaging for remaining CNS disease. ⁶⁸Ga-DOTATATE PET/CT showed single cerebellar lesion, while ¹⁸F-FDG PET/CT remained negative despite active disease; [⁶⁸Ga]Ga-DPI-4452 CAIX-targeted PET/CT revealed multisystem tumor burden; cerebellar and spinal hemangioblastomas, recurrent and contralateral ccRCC lesions and epididymal involvement. This case demonstrates the ability of CAIX-targeted imagin

Germ cell tumors of the basal ganglia are rare neoplasms that usually present with hemiparesis or other neurological symptoms. MR and PET/CT with different tracers have demonstrated their usefulness for the correct assessment of disease. Crossed cerebellar diaschisis refers to a depression in metabolism of a cerebellar hemisphere as a result of contralateral supratentorial lesions. We present a case in which MR and PET/CT were crucial for the assessment of left basal ganglia germ cell tumors with the presence of crossed cerebellar diaschisis.

Results of recent studies have led investigators to suggest that the retinoblastoma tumor-suppressor (rb) gene plays an underappreciated role in the genesis of brain tumors. Such tumors cause significant rates of mortality in children suffering from hereditary retinoblastoma. It has been assumed that the pineal gland, which is ontogenetically related to the retina, accounts for the intracranial origin of these trilateral neoplasms. To address this issue, the authors describe an unusual trilateral retinoblastoma variant. The authors provide a detailed clinicopathological correlation by describi

BACKGROUND: Movement disorders associated with hematologic malignancies remain incompletely defined. This review synthesized published articles to clarify clinical patterns, mechanisms, and outcomes across hematologic malignancies.
METHODS: A PRISMA-compliant systematic search of PubMed, Embase, Scopus, and Google Scholar was conducted to identify reports describing movement disorders associated with hematologic malignancies. Eligible reports were screened according to predefined criteria, and data were extracted on demographics, hematologic diagnosis, central nervous system (CNS) involvement

Hemangioblastomas are regarded as benign neoplasms. They may recur in 3 to 10% of cases after apparently total excision. The opposite site recurrence is not found in the reported literature. A case is described in which, after successful removal of a cerebellar hemangioblastoma followed by two years of freedom from symptoms, developed a new growth of cerebellar hemangioblastoma contralaterally again. The histology of the tumor tissue is in no way different from that of a conventional hemangioblastoma. This case gives us some insights into the biological multipotentiality of cerebellar hemangio

A 70-year-old man with cerebellar syndromes was clinically diagnosed as paraneoplastic cerebellar degeneration and underwent whole-body F-FDG PET/CT imaging for screening primary tumor. Intensely elevated tracer uptake in both cerebellar hemispheres was revealed, whereas no abnormality was found in MRI. Increased tracer uptake and swelling of the left tonsil and a cervical lymph node were found at the same time, suggesting neoplasm in tonsil with lymph node metastasis. Pathological examination demonstrated lymphoepithelial carcinoma of the left tonsil.

UNLABELLED: We present the case of a 57-year-old man with ataxia and clinical and radiological features of cerebellar degeneration. Computed tomography showed a mediastinal mass and the patient was diagnosed with thymic carcinoma. Paraneoplastic cerebellar degeneration is aninfrequent disorder and its association with thymic carcinoma very rare.
LEARNING POINTS: Unexplained subacute neurological symptoms in an adult patient should beconsidered in the possibility of a paraneoplastic syndrome. In patients over 50 years of age, acute or subacute cerebellar degeneration is paraneoplastic in 50% o

The cerebellum is one of the main targets in the central nervous system for autoimmunity. Immune-mediated cerebellar ataxias include gluten ataxia, GAD antibody-associated cerebellar ataxia, Hashimoto's encephalopathy, and paraneoplastic cerebellar degeneration. Autoimmune cerebellar ataxia may be of either insidious or subacute onset, and vertigo or transient neurological symptoms occur in some patients before the onset of the disease, in contrast to spinocerebellar degeneration. If autoimmune cerebellar ataxia is suspected, early diagnosis and introduction of treatment are very important. Fo

Gangliogliomas (GGs) are rare central nervous system neoplasms composed of neuronal and glial cells. This report details a rare case of a 32-year-old male patient with an incidentally discovered cerebellar GG following a motor vehicle accident. The literature review highlights the variability in symptoms, age, and imaging findings, emphasizing the importance of considering GG in differential diagnoses for infratentorial tumors. The patient's treatment involved subtotal resection due to the risk of significant neurological deficits, with ongoing monitoring for recurrence. This case demonstrates
