[Cerebral blood volume, endarterectomy and SPECT].

Cerebral blood flow and cerebral blood volume were measured and quantified using single photon emission computed tomography before and after unilateral endarterectomy in 3 patients with bilateral severe lesions of the internal carotid artery. These parameters were measured using an intravenous injection of 133 Xenon and 99m Technetium respectively. Before endarterectomy cerebral blood volume was high in all patients suggesting a focal vasodilatation in response to a reduced cerebral perfusion pressure. After endarterectomy a decrease of cerebral blood volume and an increase of cerebral blood f

[Cerebral and blood flow and cerebral blood volume in communicating hydrocephalus (author's transl)].

Regional cerebral blood flow (rCBF) and regional cerebral blood volume (rCBV) were estimated in patients with communicating hydrocephalus, arteriosclerotic and presenile dementia. In all groups ischemic patterns were detected which did not allow any differential diagnostic separation between the patients. In some patients with communicating hydrocephalus reduction of cerebrospinal fluid pressure by lumbar puncture or ventriculoperipheral shunt respectively resulted in increase of rCBF and rCBV. This was not achieved in the other patients with dementia. Predominantly areas with ischemic flow va

Cerebral blood flow, cerebral blood volume, and cerebrovascular reactivity after severe head injury.

Traumatic brain injury (TBI) often causes disturbances of the cerebrovascular circulation, which contribute to the infliction of secondary injury, although the complex nature of the mechanisms involved is not fully understood. First, the role of ischemia in TBI is still controversial. Despite experimental and pathologic data suggesting important interactions between ischemia and trauma, evidence for posttraumatic ischemia with CBF measurements in patients so far had eluded most investigators. Recent data, however, indicate that low CBF and ischemia probably only occur within the first few hour

Cerebral blood volume alterations during fractional pneumoencephalography.

Simultaneous and continuous measurements of the cerebral blood volume (CBV), cerebrospinal fluid (CSF) and blood pressure were carried out in six patients during fractional pneumoencephalography in order to examine intracranial volumetric interactions. Three patients (Group A) showed normal encephalographic findings, and in three patients (Group B) communicating hydrocephalus with convexity block was found encephalographically. In all patients the injection of air was followed by an immediate increase of CSF pressure and blood pressure and a concomitant decrease of CBV. The initial CSF pressur

Evaluation of total cerebral blood flow volume in cerebral atrophy.

The goal of our study is to determine total cerebral blood flow volume (tCBFV) via extracranial Doppler ultrasound and to investigate its value in the differential diagnosis of dementia. Twenty-eight outpatients with cerebral atrophy confirmed by cranial computed tomography were enrolled into the study. Nine patients with Alzheimer's disease (AD) and nine patients with vascular dementia (VaD) based on neuropsychologic test and imaging findings, as well as 10 neurologically normal elderly subjects underwent Doppler sonographic examination of extracranial internal carotid artery (ICA) and verteb

Cerebral blood flow, oxygen utilization, and blood volume in dementia.

Patients with dementia had significant decreases in cerebral blood flow and cerebral oxygen utilization and a mild, but not significant, increase in cerebral blood volume. These studies were not useful in distinguishing patients with cerebral atrophy from patients with normal pressure hydrocephalus, as similar changes in cerebral circulation and metabolism were seen in both groups. Changes in cerebral blood flow after acute decrease in the intracranial pressure also were not helpful differentiating patients with normal pressure hydrocephalus from patients with cerebral atrophy.

The influence of intravascular volume expansion on cerebral blood flow and blood volume in normal rats.

BACKGROUND: Intravascular volume expansion can increase cerebral blood flow (CBF) in patients with cerebral ischemia. However, the changes in CBF produced by volume expansion in the normal brain remain the subject of debate, and the changes on cerebral blood volume (CBV; one determinant of intracranial pressure) have not been described. The effects of acute nondilutional volume expansion on cardiac output (CO), CBF, and CBV in normal rats were examined. METHODS: Normocarbic, normothermic halothane/N2O-anesthetized rats were given 0, 6, 12, 24, or 36 ml of a blood-hetastarch mixture (n = 6 for

Apnoea of prematurity and changes in cerebral oxygenation and cerebral blood volume.

The aim of this study was to analyse the behaviour of cerebral oxygenation and cerebral blood volume (CBV) in preterm infants during apnoea by means of near-infrared spectroscopy (NIRS). The sum of oxygenated and deoxygenated haemoglobin, the total cerebral haemoglobin (Hbtot) corresponds to CBV, whereas the difference of oxygenated minus deoxygenated haemoglobin (HbD) represents a value for cerebral oxygenation. During 2 hours of daytime sleep, 72 polygraphic tracings (including NIRS) were done in 58 premature infants. The main criteria for study entry were clinically evident episodes of apno

Increased cerebral blood volume in benign intracranial hypertension.

In two patients with benign intracranial hypertension, the regional cerebral blood volume was markedly elevated (mean of 85 percent) while regional cerebral blood flow was slightly reduced (mean of 10 percent). Reduction of cerebrospinal fluid pressure by removal of cerebrospinal fluid reduced the mean values of regional cerebral blood volume by 13 percent without significant change in regional cerebral blood flow. The abnormal regional volume and regional flow returned to normal concurrent with the clinical improvement. Vinous engorgement and increased intracranial blood volume appear to play

Cerebrospinal compensation of pulsating cerebral blood volume in hydrocephalus.

OBJECTIVE: The aim of the study was to develop a computational method for the assessment of brain pressure compensation of cerebrospinal arterial blood inflow. The method was verified using clinical recordings performed during infusion studies in a group of patients diagnosed with hydrocephalus. MATERIALS AND METHODS: We studied 27 patients suspected of having normal pressure hydrocephalus. The infusion test was used to measure the resistance to cerebrospinal fluid outflow, and the elastance coefficient was performed together with recording of the blood flow velocity in the middle cerebral ar

Elevated arteriolar cerebral blood volume in prodromal Huntington's disease.

BACKGROUND: Neurovascular alterations have been implicated in the pathophysiology of Huntington's disease (HD). Because arterioles are most responsive to metabolic alterations, arteriolar cerebral blood volume (CBVa) is an important indicator of cerebrovascular regulation. The objective of this pilot study was to investigate potential neurovascular (CBVa ) abnormality in prodromal-HD patients and compare it with the widely used imaging marker: brain atrophy. METHODS: CBVa and brain volumes were measured with ultra-high-field (7.0-Telsa) magnetic resonance imaging in seven prodromal-HD patient

Alterations in intracranial pressure and cerebral blood volume in endotoxemia.

Marked deterioration of neurologic function accompanies organ dysfunction in systemic sepsis. Although previous hypotheses have suggested that cerebral hypoperfusion, anoxia or progressive edema of the brain may be causative, the pathogenesis remains unknown. Patients with sepsis with stable or supported hemodynamics and adequate oxygenation may manifest confusion, stupor or coma. Recent evidence has demonstrated that the brain is the source of many classical mediators of inflammation after various forms of injury. These mediators, including the leukotrienes, have pronounced effect on cerebrov

Predicting Delayed Cerebral Ischemia with Quantified Aneurysmal Subarachnoid Blood Volume.

BACKGROUND: The amount of blood detected on brain computed tomography scan is frequently used in prediction models for delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (aSAH). These models, which include coarse grading scales to assess the amount of blood, have only moderate predictive value. Therefore, we aimed to develop a predictive model for DCI including automatically quantified total blood volume (TBV). METHODS: We included patients from a prospective aSAH registry. TBV was assessed with an automatic hemorrhage quantification algorithm. The outcome mea

Ultrasonographic assessment of global cerebral blood volume in healthy adults.

The authors describe a new ultrasonographic method for analysis of global cerebral blood volume (CBV) and its application under controlled hyperventilation. CBV was determined as the product of global cerebral blood flow volume (CBF) and global cerebral circulation time. CBF was measured by duplex sonography and calculated as the sum of flow volumes in both internal carotid arteries and vertebral arteries. Extracranial Doppler assessed cerebral circulation time by determining the time interval of echo-contrast bolus arrival between internal carotid artery and contralateral internal jugular vei

Local cerebral blood volume response to carbon dioxide in man.

We used an emission tomographic brain scanner to investigate the relationship between local cerebral blood volume (LCBV) and arterial blood carbon dioxide tension (PaCO2) in normal awake man. Measurements were made separately in three dimensions in various regions of grey and white matter, and the resting LCBV as well as the difference in sensitivity among these regions were compared. Over the range of PaCO2 studied (20-42 torr), the response of both the grey matter and the white matter to carbon dioxide was linear. The LCBV sensitivity of the grey matter to changes in PaCO2 was 0.053 ml/100 g

MRI techniques to measure arterial and venous cerebral blood volume.

The measurement of cerebral blood volume (CBV) has been the topic of numerous neuroimaging studies. To date, however, most in vivo imaging approaches can only measure CBV summed over all types of blood vessels, including arterial, capillary and venous vessels in the microvasculature (i.e. total CBV or CBVtot). As different types of blood vessels have intrinsically different anatomy, function and physiology, the ability to quantify CBV in different segments of the microvascular tree may furnish information that is not obtainable from CBVtot, and may provide a more sensitive and specific measure

[The effect of glycerol on regional cerebral blood flow, blood volume and oxygen metabolism].

Using positron emission tomography with 15O-labelled CO2 O2 and CO gases, the effects of glycerol on regional cerebral blood flow (CBF), blood volume (CBV) and oxygen metabolism (CMRO2) were investigated in 6 patients with meningioma accompanying peritumoral brain edema. The same study was done in 5 normal volunteers. The changes of blood gases, hematocrit and hemoglobin were also examined. After a drip infusion of glycerol, the regional CBF increased not only in the peritumoral cortex and white matter but also in the intact cortex and white matter on the contralateral side.
