Cerebral vasospasm associated with intraventricular hemorrhage.

BACKGROUND AND PURPOSE: Cerebral vasospasm remains a major complication associated with aneurysmal subarachnoid hemorrhage. Although several case reports have demonstrated that intraventricular hemorrhage (IVH) related to a ruptured arteriovenous malformation can result in vasospasm in the absence of subarachnoid hemorrhage, to our knowledge, this is the first case report of cerebral vasospasm associated with primary IVH. CASE REPORT: A 44-year-old female was admitted with cerebellar infarction secondary to left posteroinferior cerebellar artery occlusion. Her hospital stay was complicated by

[Fibrinolytic treatment of cerebral intraventricular hemorrhage].

INTRODUCTION: Spontaneous cerebral intraventricular hemorrhage (SCIVH) is associated with hydrocephaly which requires ventricular drainage as treatment. Maintenance of adequate debit through the drainage system contributes to reduce morbid mortality. When the drain becomes obstructed, intermittent irrigation is necessary. This causes increased intracranial pressure and risk of infection. We propose the use of recombinant plasminogen tissue activator (r-tPA) in the treatment of SCIVH and associated hydrocephaly. CLINICAL CASES: We gave two patients with SCIVH 4 cc/day of r-tPA intraventricular

Cerebral blood flow and experimental intraventricular hemorrhage.

We evaluated the effects of acute hypovolemic hypotension followed by rapid, corrective blood infusion upon sequential regional cerebral blood flows (rCBF) in nine anesthetized 24-48-h-old beagle pups using the radioactive microsphere technique. After 5 min of hemorrhagic hypotension, during which time systemic arterial blood pressures were reduced 35-45%, cerebral blood flow to all regions increased by 31-42% over steady-state values. After blood reinfusion, rCBF increased by 39-88% over control values. Highest blood flows during hypovolemia were in the medulla and pons; slightly lower values

Idiopathic primary intraventricular hemorrhage and cerebral small vessel disease.

BACKGROUND: Although primary intraventricular hemorrhage is frequently due to trauma or vascular lesions, the etiology of idiopathic primary intraventricular hemorrhage (IP-IVH) is not defined. AIMS: Herein, we test the hypothesis that cerebral small vessel diseases (cSVD) including hypertensive cSVD (HTN-cSVD) and cerebral amyloid angiopathy are associated with IP-IVH. METHODS: Brain magnetic resonance imaging from consecutive patients (January 2011 to September 2019) with non-traumatic intracerebral hemorrhage from a single referral center were reviewed for the presence of HTN-cSVD (define

Symptomatic cerebral vasospasm after intraventricular hemorrhage from ruptured arteriovenous malformation.

We present a rare case showing symptomatic vasospasm after intraventricular hemorrhage from a ruptured arteriovenous malformation. No significant subarachnoid hemorrhage in the basal cisterns was observed on computed tomography in the entire course of illness. Although most cases showing vasospasm are associated with the deposition of blood in the basal cisterns, this case suggests the possibility of some other important factor in causing the subsequent development of vasospasm.

Concomitant cerebral infarction and intraventricular hemorrhage in moyamoya disease. Case report.

Moyamoya disease is categorized as either ischemic or hemorrhagic type, and the pathogenesis of this disease is unknown. In this paper, the authors report on a patient with moyamoya disease who suffered concomitant cerebral infarction and intraventricular hemorrhage (IVH). Endoscopic removal of the intraventricular hematoma and ventricular drainage were both performed. The patient did not experience further ischemic complications. Patients with moyamoya disease have intracranial hemodynamic insufficiency. Adequate control of intracranial pressure and removal of the intraventricular hematoma is

[Real-time sonography in the evaluation of peri- and intraventricular cerebral hemorrhage].

Of 1776 sonograms done on 1312 patients with sector real time equipment, 923 done on 670 patients were selected because of suspected peri and intraventricular cerebral hemorrhage. The studies demonstrated hemorrhage in only 117 patients. It was possible to follow 107 patients and in 6.8 the hemorrhage was grade I, in 14 children it was grade II, in 19 it was grade II and in 6 it was grade IV. Most of the children (82 of 107) were pre-term (the average gestational age was less than 32 weeks). Twenty-three were term and two were post term. In most instances, the grade I hemorrhage resolved witho

Cerebral intraventricular hemorrhage caused by a large cerebral arteriovenous malformation at 31 years after diagnosis.

Cerebral arteriovenous malformation (AVM) has an overall 2% to 4% annual risk of hemorrhage. The annual risk of hemorrhage does not decrease with age. However, the natural history of non-operative AVM is difficult to follow up consistently for more than 30 years. In this report, we present a case with a large cerebral AVM which developed a major bleeding causing intraventricular hemorrhage (IVH) 31 years after the diagnosis. A male patient was proved to have a cerebral AVM in the right corpus callosum by cerebral angiography at ages 20 and 28. He was found losing consciousness due to rupture o

Intraventricular hemorrhage in the preterm neonate: timing and cerebral blood flow changes.

Serial cranial ultrasound studies, 133xenon inhalation cerebral blood flow determinations, and risk factor analyses were performed in 31 preterm neonates. Contrast echocardiographic studies were additionally performed in 16 of these 31 infants. Sixty-one percent were found to have germinal matrix or intraventricular hemorrhage. Seventy-four percent of all hemorrhages were detected by the thirtieth postnatal hour. The patients were divided into three groups: early GMH/IVH by the sixth postnatal hour (eight infants) interval GMH/IVH from 6 hours through 5 days (10), and no GMH/IVH (12). Cerebral

Rapid progression of cerebral infarction after intraventricular hemorrhage in adult moyamoya disease.

The authors present a rare case of adult moyamoya disease in which a patient experienced rapid progression of cerebral infarction after intraventricular hemorrhage (IVH). A healthy 39-year-old woman was admitted to our hospital with sudden headache, a decreased level of consciousness and mild tetraparesis. Initial magnetic resonance imaging revealed small cerebral infarction and IVH. Although the patient underwent conservative therapy including hypervolemia, hemodilution, keeping moderate hypertension and administration of a free radical scavenger, she showed a fulminant clinical course of cer

Drug therapy in hypoxic-ischemic cerebral insults and intraventricular hemorrhage of the newborn.

Recent progress in understanding the pathophysiology of hypoxic-ischemic encephalopathies and intraventricular hemorrhages have enabled us to propose new therapeutic modalities in preventing the development and severity of these disorders in the newborn. Because neuronal damage may be reversible under certain conditions, appropriate evaluation of these drugs (single or in combination) is very important in the continuous attempt to ameliorate the neurodevelopmental outcome of these infants. An interventionist attitude should prove to be a better alternative than a supportive one.

Fatal cerebral infarction after intraventricular hemorrhage in a pregnant patient with moyamoya disease.

Moyamoya disease is a progressive occlusive cerebrovascular disorder of unknown etiology. We describe a case of a fatal cerebral infarction after intraventricular hemorrhage in a 29-year-old primipara with moyamoya disease. The patient had been stable for about 10 days after the initial hemorrhage, but she showed a fulminant clinical course of cerebral infarction, and died on the 16th day after the initial hemorrhage. Abnormality in coagulation system and/or cerebral vasospasm may have resulted in such a fulminant clinical course. A possible pathogenesis of this rare condition is discussed.

Fibrinolysis Treatment for Cerebral Intraventricular Hemorrhage: A Temporal and Spatial Voxel-Based Analysis.

OBJECTIVE: A voxel-based statistical approach on computer tomographic data in patients with intracerebral hemorrhage (ICH) and acute intraventricular hemorrhage (IVH) was used to evaluate spatial and temporal patterns of intraventricular blood in patients treated with intraventricular fibrinolysis (IVF) or without. METHODS: IVH shapes were systematically assessed three dimensionally in patients with supratentorial ICH at three intervals of time (day of admission, day 4 ± 1, day 7+). The boundaries of the intraventricular blood clot were delineated on computed tomography (CT) scans using dedic

Cerebral Oxygenation is Associated with Neurodevelopmental Outcome in Infants Born Preterm with Intraventricular Hemorrhage.

OBJECTIVE: To examine how cerebral oxygenation levels monitored by near-infrared spectroscopy (NIRS) are associated with neurodevelopmental trajectories in neonates with intraventricular hemorrhage (IVH). STUDY DESIGN: In this prospective study, 95 neonates born at <34 weeks of gestation with IVH (63 males, median gestational age: 25+6 weeks) between 2013-2023 were included. Regional cerebral oxygen saturation (rScO2) was monitored from IVH diagnosis until term-equivalent age. Analysis included rScO2 levels, duration outside the normal range (<55%; >85%), fluctuations, and cerebral fractional

Cold-Induced Reversible Cerebral Vasoconstriction Syndrome Presenting With Intraventricular Hemorrhage in a Normotensive Patient.

Reversible cerebral vasoconstriction syndrome (RCVS) is a transient neurovascular disorder characterized by sudden, severe headaches and reversible multifocal cerebral artery narrowing. While it is often associated with vasoactive substances, emotional stress, or postpartum status, environmental triggers such as cold exposure are rarely reported. We describe the case of a 61-year-old normotensive female who developed a thunderclap headache and vomiting shortly after brief exposure to cold weather. Initial evaluation revealed hypothermia and metabolic acidosis; she was discharged without neuroi

Assessment of intraventricular hemorrhage risk in preterm infants using mathematically simulated cerebral blood flow.

Intraventricular hemorrhage (IVH)4 is one of the most threatening neurological complications associated with preterm birth which can lead to long-term sequela such as cerebral palsy. Early recognition of IVH risk may prevent its occurrence and/or reduce its severity. Using multivariate logistic regression analysis, risk factors significantly associated with IVH were identified and integrated into risk scales. A special aspect of this study was the inclusion of mathematically calculated cerebral blood flow (CBF) as an independent predictive variable in the risk score. Statistical analysis was b

Uncomplicated intraventricular hemorrhage is not associated with lower estimated cerebral volume at term age.

BACKGROUND AND AIMS: Cerebral lesions detected using cerebral ultrasound (cUS) in very preterm infants are associated with increased risk for neurodevelopmental problems. However, uncomplicated intraventricular hemorrhage (IVH) has no consistent association with poor outcome. In this study we evaluate the effect of uncomplicated IVH on estimated brain volume at term-equivalent age (TEA), using a model based on measurements made from cUS. METHODS: We studied 2 groups of preterm infants (<32 weeks' gestational age (GA)) up to and at TEA: (1) infants with uncomplicated grades 2 or 3 IVH, (2) inf
