[Hematoma of the cerebral peduncle].

The paper presents a rare case of surgical treatment for hematoma of the cerebral peduncle in a 30-year-old woman found to have progressive neurological focal disorders due to mesaticephalic and thalamic lesions. Computed tomography and magnetic resonance imaging (MRI) revealed a round mass (26 x 3 x 25 mm) in the left cerebral peduncle and thalamus without surrounding edema. The focus showed characteristic signs of subacute parenchymatous hematoma. The interpeduncular cistern was explored via transsylvian approach and hematoma was removed by making a small incision of the left cerebral pedunc

Superior cerebellar peduncle deep brain stimulation for cerebral palsy.

OBJECTIVE: Patients with coexisting spastic cerebral palsy (CP) and dystonia have limited treatment options. In this study, the authors aimed to evaluate the efficacy of deep brain stimulation (DBS) targeting the superior cerebellar peduncles (SCPs) in adults with CP. METHODS: Five patients with CP and medically refractory dystonia and spasticity underwent SCP DBS. Assessments included the Burke-Fahn-Marsden Dystonia Rating Scale (BFMDRS), modified Ashworth scale (mAS), and tests of cognition, mental status, and quality of life preoperatively and at 3, 6, and 12 months postoperatively (in bot

"Cortical" Wrist Drop due to a Cerebral Peduncle Infarct.

Wrist drops are usually due to a peripheral neuropathy affecting the radial nerve. Cortical wrist drops may occur, usually due to a lesion in the contralateral hand area of the motor cortex. We report a unique case of "cortical" wrist drop due to an acute infarction of the contralateral cerebral peduncle. The patient is a 70-year-old gentleman with the vascular factors of hypertension, hyperlipidaemia, and prior cerebellar strokes, who developed sudden onset of right wrist weakness without numbness. Neurological examination showed a right wrist and finger drop. Nerve conduction study of the ra

Cerebral peduncle tumor ablated by novel 3-mm laser tip.

BACKGROUND/OBJECTIVE: Decisions to use open surgery or radiotherapy in pediatric patients with familial neoplastic syndromes must consider not only the symptomatic benefits of treatment, but also future limitations these treatments may impose. Specifically, open surgical resection of noncurable tumors may preclude or encumber future lesion resections, while radiotherapy has detrimental effects on pediatric cognitive development and increases the risk of future malignancy development. We provide the first report of using a novel 3.0-mm diffusing laser tip with laser-induced thermal therapy (LiT

Bilateral cerebral peduncle infarct in a paediatric intensive care unit patient.

UNLABELLED: Bilateral isolated cerebral peduncle infarction is a rare stroke, previously reported only in adults. This report describes a paediatric burns patient with preceding cardiac arrest who developed subtle neurology. Imaging revealed bilateral isolated peduncle infarcts with hypoplasia of the left posterior communicating and vertebral arteries. Despite supportive care, the patient deteriorated and demised. This case highlights the vulnerability of peduncular perforators to haemodynamic instability and emphasises the need for early imaging in the detection of paediatric stroke. CONTRIB

Post-coronavirus inflammatory cerebral peduncle lesion presenting as painful tonic spasms.

Painful tonic spasms (PTS) are paroxysmal, stereotyped, painful episodes of involuntary posturing typically associated with demyelinating or structural lesions within the corticospinal tract. We report a case of PTS in temporal association with a COVID-19 infection, most likely attributable to post-infectious neuroinflammation. A woman in her 40s developed frequent, disabling, painful spasms of the left extremities 2 weeks after a COVID-19 infection. An MRI of the brain revealed a solitary contrast-enhancing T2/fluid-attenuated inversion recovery hyperintense ovoid lesion in the right cerebral

The usefulness of DTI for estimating the state of cerebellar peduncles in cerebral infarct.

Elucidation of the state of cerebellar peduncles (CPs) is difficult because CPs are not completely isolated from adjacent structures and long structures. We attempted to investigate the usefulness of diffusion tensor tractography (DTT) in patients with CP lesions resulting from cerebral infarct. We recruited 6 patients with CP infarct and 6 age-and sex-matched control subjects. Diffusion tensor images (DTIs) were acquired using a sensitivity-encoding head coil at 1.5 T. Three cerebellar peduncles, namely, the superior cerebellar peduncle (SCP), the middle cerebellar peduncle (MCP), and the inf

Localization of arm representation in the cerebral peduncle of the non-human primate.

Motor deficit severity and the potential for recovery in patients with brain injury depend on the integrity of descending corticofugal projections. Clinical assessment of these conditions following subtotal brain trauma requires a comprehensive understanding of the anatomical structures involved in the lesion as well as those structures that are spared. To assist in this endeavor, we investigated motor fiber organization in the crus cerebri of the cerebral peduncle (ccCP) in the rhesus monkey. Fibers originating from the arm representations of the primary (M1), supplementary (M2), rostral cing

The somatotopic localisation of the descending cortical tract in the cerebral peduncle: a study using MRI of changes following Wallerian degeneration in the cerebral peduncle after a supratentorial vascular lesion.

We studied the effects of Wallerian degeneration in the cerebral peduncle shown by magnetic resonance imaging (MRI) following a supratentorial vascular lesion, to identify the somatotopic localisation of the descending cortical tracts. Patients with a lesion involving a large area of a cerebral hemisphere had an area of abnormal signal intensity in the whole cerebral peduncle, suggesting Wallerian degeneration of all the whole descending cortical tracts. With a small lesion confined to the precentral gyrus, corona radiata, or posterior limb of the internal capsule there was an abnormal signal

Anisotropy of water diffusion in corona radiata and cerebral peduncle in patients with hemiparesis.

Diffusion tensor imaging is a magnetic resonance method which provides quantitative measurements of the directionality (anisotropy) of diffusion. Anisotropy measurements can be used to obtain quantitative information about the microstructural integrity of white matter tracts. In intact tracts diffusion is restricted and directional because water molecules move predominantly longitudinally to tracts. The aim of this study was to measure the anisotropy of diffusion in patients with chronic hemiparesis. We measured in the corona radiata and the cerebral peduncle in 10 patients with a chronic hemi

Relationship between ataxia and inferior cerebellar peduncle injury in patients with cerebral infarct.

INTRODUCTION: The inferior cerebellar peduncle (ICP) is a major neural tract in the cerebellum and is involved in coordination of movement and proprioceptive; therefore, ICP injury can be accompanied by poor coordination of movement, including ataxia. In this study, using diffusion tensor tractography (DTT), we investigated the relationship between ataxia and ICP injury in patients with cerebral infarct. METHODS: We recruited 14 stroke patients with ataxia after the onset of stroke and 12 normal subjects. The Score of Assessment and Rating of Ataxia (SARA) was used to evaluate ataxia. The val

Microsurgical Resection of a Left Cerebral Peduncle Cavernous Malformation: 3-Dimensional Operative Video.

Brainstem cavernous malformations are challenging lesions considering the numerous eloquent structures frequently related. Surgical resection is the preferred treatment if the patient is symptomatic and the lesion can be safely resected. We present the case of a right-handed 38-yr-old female, presenting with progressive impairment of her handwriting. Physical examination showed a right-sided grade 4/5 hemiparesis. Preoperative imaging was suggestive of a left cerebral peduncle cavernous malformation with a recent area of hemorrhage. The most superficial portion of the lesion was on the surface

Cerebral peduncle angle: Unreliable in differentiating progressive supranuclear palsy from other neurodegenerative diseases.

INTRODUCTION: The significant symptom overlap between progressive supranuclear palsy (PSP) and other parkinsonian neurodegenerative diseases frequently results in misdiagnosis. However, neuroimaging can be used to quantify disease-related morphological changes and specific markers. The cerebral peduncle angle (CPA) has been shown to differentiate clinically diagnosed PSP from other parkinsonian diseases but this result has yet to be confirmed in autopsy-proven disease. METHODS: Magnetic resonance imaging (MRI) scans were obtained for 168 patients representing 69 medical facilities. Following

Cerebral Peduncle Angle: An Objective Criterion for Assessing Progressive Supranuclear Palsy Richardson Syndrome.

OBJECTIVE: Several criteria for time-consuming volumetric measurements of progressive supranuclear palsy Richardson syndrome subtype (PSP-RS) have been proposed. These often require image reconstruction in different planes for proper assessment. The purpose of this study was to evaluate the cerebral peduncle angle as a simple and reproducible measure of midbrain atrophy in patients with PSP-RS. MATERIALS AND METHODS: The records of 15 patients with PSP-RS were retrospectively identified. The records of 31 age-matched healthy control subjects, 15 patients with multiple-system atrophy, and 22 p
