Cerebrospinal fluid eosinophilia associated with intraventricular shunts.

CSF eosinophilia (CSF-eo) is uncommon and is usually caused by helminthic infections. However, it has also been found in ∼30% of patients experiencing intraventricular shunt malfunctions. We present a case report and review the conditions associated with CSF-eo and their prophylaxis. An 8 year-old boy with tetraventricular hydrocephalus has had several shunt malfunctions over the last three years. During hospitalization in January 2009 for shunt revision, a transient 30% eosinophilia was detected in his cerebral spinal fluid (CSF) concomitant with Staphylococcus epidermidis infection and long

Imaging of Cerebrospinal Fluid Shunts and Their Complications.

Hydrocephalus, characterized by excessive intracranial accumulation of cerebrospinal fluid (CSF) in the ventricular system as a result of excessive production, decreased absorption, or impaired circulation of CSF, is a common condition affecting 1%-2% of the U.S. population. More than 40 000 CSF shunts are placed every year in the United States, the majority of which are for treatment of hydrocephalus. Temporary options for CSF diversion include serial lumbar or ventricular punctures, ventriculosubgaleal shunt, external ventricular drain, and ventricular access device. Permanent CSF diversion

Management of pregnant women with cerebrospinal fluid shunts.

As more women with cerebrospinal fluid shunts reach child-bearing age, neurosurgeons, obstetricians and other health care providers will increasingly be called upon to care for them once they become pregnant. A review of the literature reveals that these patients may develop symptoms of shunt malfunction as uterine size increases. In most cases, symptoms can be managed conservatively during pregnancy and usually resolve following delivery. The presence of a CSF shunt per se, is not a contraindication to pregnancy and eventual fetal and maternal outcome has been excellent in the majority of cas

[Cerebrospinal fluid shunting and pregnancy].

AIMS: The aim of this study is to survey the literature and update the steps to be taken in managing a pregnant patient in whom cerebrospinal fluid (CSF) shunting has been performed. DEVELOPMENT: The risk of valve malfunction (VMF) during pregnancy is approximately 27.5% and is mainly produced by a functional obstruction due to an increase in intra abdominal pressure during gestation. The risk of VMF and complications rises until 6 months after childbirth. The study of the functioning of the shunt and genetic counselling prior to conception are of vital importance. We discuss the steps to be

Cerebrospinal fluid shunt infections.

Cerebrospinal fluid (CSF) shunt infection is one of the most frequent and disabling complications. We reviewed the records of 306 patients who underwent CSF shunt surgery from 1983 through 1992. Six hundred and twelve procedures were performed in these 306 patients. Infection occurred following 46 of the procedures for an infection rate of 7.5% per procedure. The 46 infections involved 39 patients. There were 8 recurrent infections. The infection rate per child was 12.7%. Staphylococcal species were isolated in 50% of all infections. Patients younger than 1 year old and children with multiple

Sports and pediatric cerebrospinal fluid shunts: who can play?

OBJECTIVE: The purpose of this study was to collect and review available data on the incidence of sport-related complications in children with cerebrospinal fluid (CSF) shunts. METHODS: After review of medical and legal literature revealed no reports on sports and shunts, an Internet-based survey was conducted of pediatric neurosurgeons. Observed complications, recommendations for sport participation, and shunt volume data were collected. RESULTS: Ninety-two providers (55% sample) responded. Seventy-seven percent had never observed a sport-related shunt complication in their practice. The re

Cerebrospinal fluid shunt infections in children.

Cerebrospinal fluid (CSF) shunts are commonly used for the long-term management of hydrocephalus in children. Shunt infection remains a common complication, occurring in about 5%-15% of CSF shunts. This narrative review summarises key evidence from recent literature on the epidemiology, pathogenesis, clinical presentation, diagnosis, management, outcomes and prevention of CSF shunt infections in children. The majority of shunt infections occur due to contamination at the time of surgery, with coagulase-negative staphylococci and Staphylococcus aureus being the most common infecting organisms.

[Cerebrospinal fluid shunting: anesthetic particularities].

The symptomatic treatment of hydrocephalus remains cerebrospinal fluid (CSF) drainage to an external reservoir (external CSF drainage) or to an internal cavity mainly the peritoneum or the right atrium via a unidirectional valve (internal CSF drainage) and finally by endoscopic ventriculocisternostomy. Local anaesthesia is adequate for external CSF drainage in adults and children above 10 years while general anaesthesia is required in all other cases. The main problems encountered in these patients are difficult intubation and full stomach associated with increased intracranial pressure. The a

Cerebrospinal fluid shunt infections in infants.

Infection remains a major cause of morbidity and mortality following CSF shunt procedures. In this study 191 shunt procedures carried out from January 1981 to December 1992 in a series of 81 infants (less than 6 months old) were retrospectively analyzed for possible risk factors. The overall surgical infection rate was 7.8%, with 15 infections occurring in 14 patients (17.2%). No significant difference in the rate of infections was found in relation to sex, birth weight, gestational age, and type of shunt procedure (primary insertion/revision). The occurrence of other infections during the per

[Diagnosis and therapy of intraperitoneal cerebrospinal fluid pseudocyst in ventriculoperitoneal cerebrospinal fluid shunts in patients with hydrocephalus].

Problems of an intraabdominal CSF cyst following ventriculoperitoneal shunt for hydrocephalus are demonstrated in an 8 1/2-month-old female infant. Whereas previously tentative diagnosis was made by plain abdominal x-ray and could be verified by laparotomy only, repeated ultrasound investigations now allow for early recognition of this complication. As it happened in this case, laparotomy was not necessary and mere aspiration of cyst contents via distal catheter followed by insertion of the distal catheter into the right atrium was sufficient. This method is an alternative to transabdominal cy

Multimodality imaging of cerebrospinal fluid shunts: system evaluation and complications.

Cerebrospinal fluid (CSF) shunts are critical for managing hydrocephalus by diverting excess CSF from the craniospinal axis to absorptive sites. Imaging plays a pivotal role in the assessment of shunt function, early postoperative changes, and a wide array of complications. Understanding the imaging characteristics of both normal postoperative appearances and potential complications with different imaging modalities enhances diagnostic accuracy and enables timely clinical management. This comprehensive review outlines the anatomical and functional aspects of various shunt systems.

A new method for measuring cerebrospinal fluid flow in shunts.

An implantable device for measurement of cerebrospinal fluid (CSF) flow in a ventriculoperitoneal shunt tube has been developed. The unit is energized by an extracorporeal high-frequency generator (200 KHz), and electrolysis creates bubbles in the shunt tube. Velocity of bubble flow is detected by a pair of ultrasonic Doppler probes placed a certain distance apart on the skin surface and in parallel with the implanted tube. The CSF flow rate is calculated taking into account velocity and tube diameter, and is expressed in ml/min. The unit consists of a coil with a capacitor, a silicon diode to

Observations regarding failure of cerebrospinal fluid shunts early after implantation.

Despite advances in cerebrospinal fluid (CSF) diversionary techniques, shunt failure due to infection or malfunction remains a persistent problem in hydrocephalus care. The aim of this study was to evaluate the independent predictors of early shunt survival after implantation in a large cohort of patients. The authors retrospectively reviewed the records of all patients who had undergone shunt implantation procedures at their institution during an 8-year period. They analyzed the independent predictors of shunt survival in 116 failed shunt placement procedures (infection or malfunction) by per

Haemophilus influenzae meningitis in the presence of cerebrospinal fluid shunts.

Haemophilus influenzae is an uncommon pathogen in shunted patients and there is uncertainty about optimal management. We report here two cases which were managed differently, with different outcomes. The first case was treated with chloramphenicol and the shunt was not removed. Although there were subsequent episodes of respiratory infection, the outcome was satisfactory. The second case was treated with cefuroxime and the shunt was exteriorized. Re-shunting was followed by relapse and further shunt removal. This and other case reports suggest that in Haemophilus meningitis in shunted patients

Clinical outcome of cerebrospinal fluid shunts in patients with leptomeningeal carcinomatosis.

BACKGROUND: Leptomeningeal carcinomatosis (LMC) is frequently associated with hydrocephalus, which quickly devastates the performance of the patient. Cerebrospinal fluid (CSF) shunt is a widely accepted treatment of choice, but the clinical outcomes in patients with LMC are not well studied. This study aimed to examine the efficacy of a CSF shunt in patients with LMC. METHODS: Seventy patients with LMC confirmed by cytology or magnetic resonance imaging (MRI) underwent ventriculoperitoneal (VP) or lumboperitoneal (LP) shunt surgery. We retrospectively analyzed the clinical characteristics of
