Incomplete ossification of the atlas in dogs with cervical signs.

Osseous defects affecting the atlas were identified in computed tomography and magnetic resonance images of five dogs with cervical signs including pain, ataxia, tetraparesis, or tetraplegia. Osseous defects corresponded to normal positions of sutures between the halves of the neural arch and the intercentrum, and were compatible with incomplete ossification. Alignment between the portions of the atlas appeared relatively normal in four dogs. In these dogs the bone edges were smooth and rounded with a superficial layer of relatively compact cortical bone. Displacement compatible with unstable

Hypertrophic posterior arch of atlas causing cervical myelopathy.

Cervical stenosis, especially of the upper cervical spine, is quite rare which can be developmental or acquired. Clefts or aplasias of anterior and posterior arches of atlas, ossification of the transverse atlantal ligament, hypertrophy of the dens and os odontoideum are rare conditions causing cervical myelopathy reported either singly or in combination. Hypertrophy of the posterior arch of atlas in the absence of any ring hypoplasia as a cause of cervical myelopathy has not been reported earlier. The authors report a case of cervical myelopathy in a 26-year-old female due to hypertrophied po

Magnetic resonance imaging atlas of the cervical spine musculature.

The anatomy of the cervical spine musculature visible on magnetic resonance (MR) images is poorly described in the literature. However, the correct identification of individual muscles is clinically important because certain conditions of the cervical spine, for example whiplash associated disorders, idiopathic neck pain, cervical nerve root avulsion and cervical spondylotic myelopathy, are associated with different morphological changes in specific muscles visible on MR images. Knowledge of the precise structure of different cervical spine muscles is crucial when comparisons with the contrala

[Unstable Jefferson variant atlas fractures: an unrecognized cervical injury].

Nine cases of unstable Jefferson variant atlas fractures were treated with nonoperative external immobilization between 1989 and 1993. All of them were studied by plain films and CT scans. Seven cases had three breaks of the atlas ring. One case had unilateral anterior arch fracture, associated transverse ligament tear and quadriplegia. The other case had Jefferson, Hangman, C3pedicle burst fractures and C3,4dislocation. All of the fractures were unstable or potentially unstable. Despite the abnormal open month view in all cases, the plain films showed minimal abnormalities, requiring CT for d

Cervical spinal stenosis and myelopathy due to atlas hypoplasia.

This paper describes a patient who presented at our hospital with neurologic symptoms due to congenital cervical spinal stenosis at the atlas level. Congenital atlantal stenosis is usually due to hypoplasia of the posterior arch of the atlas. It is a rare cause of spinal stenosis, and only 12 symptomatic patients with isolated atlantal stenosis have been reported. Current treatment is surgical decompression, and all reported patients receiving surgical treatment improved to some degree.

Analytical static stress analysis of first cervical vertebra (atlas).

INTRODUCTION: Fracture of the atlas was first described by Jefferson (1920). He theorised a bursting mechanism of fracture as the occipital condyles were driven into the atlas. Experimental studies by Hays and Alker (1988) and Panjabi et al (1991) were also conducted to explain the injury mechanisms. Injury mechanisms and fracture patterns are important in the clinical evaluation of spinal injuries. Recognition and interpretation of the fracture patterns help to determine the spinal instability and consequently the choice of treatment. Although the fracture mechanics of the atlas have received

Unstable Jefferson variant atlas fractures: an unrecognized cervical injury.

We retrospectively reviewed the plain films and CT scans of 11 unstable atlas fractures from a series of 72 atlas fractures to better characterize these injuries and to determine if the correct diagnosis could have been made. These 11 atlas fractures were unstable because either the anterior bony ring was disrupted (six cases), the posterior longitudinal ligament was torn (one case), or both the anterior bony ring and the posterior longitudinal ligament were disrupted (four cases), allowing C1-C2 subluxation to occur. Although all the fractures were potentially unstable, only eight demonstrate

Congenital hypoplasia of the arch of the atlas with abnormal segmentation of the cervical spine.

We describe a 53-year-old Japanese woman with absence of the posterior arch of the atlas. Computed tomography revealed that the residual posterior fragment which was displaced to the right side does not correspond to a rare ossification center in the midline and that there was no clear mass of the posterior arch except for a small fragment. Moreover, the midline center was not demonstrated as a soft tissue mass. Easy tilting of the posterior fragment on flexion-extension movement suggests that the cartilaginous arch was not present between the lateral masses and the posterior fragment of the a

Spontaneous fracture of atlas of cervical spine affected by rheumatoid arthritis.

A case of spontaneous fracture of the atlas in a 67-year-old woman who had suffered from rheumatoid arthritis for more than 20 years is described. This fracture is unique and can present diagnostic problems. Lying in the coronar plane makes the atlas difficult to visualize radiographically. Computer-guided tomography is a helpful diagnostic technique. The brittleness of the cervical spine in long-term rheumatoid patients should also be kept in mind when performing endotracheal intubation.

Cervical myelopathy secondary to atlas hypoplasia - reports of 3 adult cases -.

There have been paucity of reports on atlas hypoplasia, and as a result this condition is not clearly defined, nor well understood. The authors reported three cases of atlas hypoplasia that were found in adults who presented with myelopathic symptoms. On radiographic examination, it was found that the anterior-posterior diameter of the atlas was remarkably narrower in all three cases in comparison with normal persons. The MRI in all three cases also revealed intramedullary high signal lesions at the levels where severe spinal cord compression was present. This led to our diagnosis of atlas hyp

First cervical vertebra (atlas) fracture mechanism studies using finite element method.

Injury mechanisms and stress distribution patterns are important in the clinical evaluation of spinal injuries. Recognition and interpretation of the failure patterns help to determine spinal instability and consequently the choice of treatment. Although, the biomechanics responses of the atlas have received much attention, it has not been investigated using theoretical modeling. Mathematical techniques such as finite element model will provide further understanding to the injury mechanisms of the atlas, which is important for the prevention, diagnosis, and treatment of spinal injuries. Magnetic Resonance Imaging Atlas-Based Volumetric Mapping of the Cervical Cord Gray Matter in Cervical Canal Stenosis.

BACKGROUND: White matter volume loss may be an anatomic driver in the development of clinical symptoms in cervical spondylotic myelopathy (CSM). Considerably less attention has been devoted to gray matter (GM) injury. Newly developed atlas-based mapping techniques may allow evaluation of GM cord volume alterations in CSM. METHODS: There were 29 subjects evaluated: 15 patients with CSM (61.1 ± 8.7 years old) and 14 age-matched control subjects (56.1 ± 5.3 years old). All subjects underwent 3T magnetic resonance imaging of the cervical spine. Post-processing with the Spinal Cord Toolbox (v3.0)

Preserving Cervical Mobility: A Novel Robot-Assisted Approach for Atlas Fracture Fixation.

BACKGROUND The management of unstable atlas fractures remains a subject of ongoing debate and controversy. The conservative surgical treatment commonly involves fusion, resulting in severe loss of cervical spine mobility, and a large incisions and extensive tissue dissection are required. We aim to introduce a novel concept and surgical approach for treating atlas fracture, one that involves minimizing trauma while maintaining mobility of the upper cervical spine without resorting to fusion. CASE REPORT We present the case of a 48-year-old woman with an atlas fracture who underwent robot-assis

Atlas-Free Cervical Spinal Cord Segmentation on Midsagittal T2-Weighted Magnetic Resonance Images.

An automatic atlas-free method for segmenting the cervical spinal cord on midsagittal T2-weighted magnetic resonance images (MRI) is presented. Pertinent anatomical knowledge is transformed into constraints employed at different stages of the algorithm. After picking up the midsagittal image, the spinal cord is detected using expectation maximization and dynamic programming (DP). Using DP, the anterior and posterior edges of the spinal canal and the vertebral column are detected. The vertebral bodies and the intervertebral disks are then segmented using region growing. Then, the anterior and p

Minimally invasive diagnosis of precancerous cervical lesions using single-cell peripheral immune atlas.

Cervical cancer remains a major global health concern for women. Current screening methods are either invasive or lead to low participation and over-referral for colposcopy, particularly among high-risk human papillomavirus (HPV)-positive women. This study analyzes 613 participants with varying cervical lesions using mass cytometry by time-of-flight (CyTOF) to identify disease-specific peripheral immune signatures. A diagnostic model based on 23 immune features achieves ∼91% sensitivity and specificity for detecting precancerous and cancerous lesions. A separate model for HPV-positive women sh
