Hospital chaplaincy is an important component of holistic care, but its role may not be fully understood by foundation doctors during early postgraduate training. This cross-sectional survey explored foundation doctors' awareness, perceptions, and use of hospital chaplaincy services at a district general hospital in the United Kingdom. Of 83 eligible foundation doctors, 44 completed the anonymous questionnaire (53% response rate, 95% CI: 42.4-63.4%). Although 88.6% (95% CI: 76.0-95.0%) of respondents were aware that the hospital had a chaplaincy service, 63.6% (95% CI: 48.9-76.2%) were unfamil

By welcoming laypersons into the pastoral ministry, the Church is participating in transformation--clearing away the familiar patterns of ministry so that the laity can respond more fully to its baptismal calling. Three basic questions should be asked in meeting the challenges of this calling: 1. Who should be chosen for lay pastoral care? Those selected should not be overly passive, but assertive and mature in their faith. 2. What should be expected of pastoral laypersons? Catholic hospitals should be careful not to exploit lay ministers by expecting extraordinary sacrifices (e.g., being on c

This reflection challenges the revenue-centric model of healthcare administration within the United States that treats professional chaplains as financial liabilities due to a lack of direct fee-for-service insurance billing codes-a structural condition reflected in compensation disparities between chaplains and their clinical peers. Operating within the contemporary landscape of value-based care, this paper synthesizes health economics and clinical literature to establish a data-informed case for chaplaincy as an institutional asset for cost mitigation and quality improvement. Early spiritual

Canadian federal prison chaplaincy underwent a major shift in 2013 when the provision of its services was privatized and outsourced to a single for-profit company. This article presents a summary of the experiences and concerns expressed by minority faith chaplains serving in federal correctional institutions following privatization. It is based on ten in-depth, semi-structured interviews with minority faith prison chaplains. The results show that minority faith federal prison chaplains are concerned about increased levels of bureaucratization that have compromised the quality of spiritual car

The COVID-19 pandemic brought about a scenario of physical, social, and emotional illness that significantly increased the need for spiritual care, particularly in hospital environments, where individuals were more vulnerable, fragile, and anxious. In this context, the hospital chaplain played a strategic role within the multidisciplinary care team for hospitalized patients, serving as the primary provider of spiritual care. This article presents an experiential report of the role of hospital chaplains in providing spiritual support to the multidisciplinary team, patients, and their families i

Healthcare chaplains recognize the importance of informed referrals for spiritual care; they are essential for spiritual care in outpatient settings. Research about the prevalence and harmful effects of religious/spiritual distress underscores the importance of effective methods for identifying patients who would benefit from spiritual care. Spiritual screening is a valuable way to help healthcare colleagues identify patients who would benefit from further assessment and spiritual care. To help spiritual care programs implement spiritual screening, in this article chaplains from six organizati

AIDS is unique in that it is terminal, transmissible, and surrounded by stigma. AIDS patients are often ostracized by those close to them and frequently they are alienated from religion. The pastoral care giver must provide a special ministry to AIDS patients, a ministry in which he or she accompanies the patient on a journey inward. This journey involves a broad range of feelings related to such concerns as social alienation, emotional isolation, and preparation for death. Its final destination, however, lies in the ultimate issues such as self-identity, the meaning of life, and individual de

As an interfaith hospital chaplain and a Christian, navigating the profound emotional terrain of grief and loss is both a professional duty and a deeply personal journey. Attending the funerals of two close friends has brought out the interplay between my role as a spiritual caregiver and my own vulnerability. In reflection, I realize the importance of self-care and the reminder that in offering comfort and empathy, I must first find it within myself.

This clinical pastoral reflection explores how historical and artistic narratives may support meaning-making in chaplaincy encounters. During a pastoral visit with a patient experiencing advanced cancer and depression, the lives of Vincent van Gogh and William Cowper became interpretive resources through which suffering was reframed as spiritually continuous rather than spiritually disqualifying. The encounter illustrates how narrative engagement can foster renewed hope while suffering remains unresolved within clinical pastoral care.

Spiritual Care is a recognised dimension of Palliative Care, yet its provision is often inadequate. This study aimed to improve the identification of patients' spiritual needs using a standardised screening, address these more effectively in everyday Palliative Care, and evaluate potential benefits of this intervention. We conducted a monocentric, mixed-methods, quasi-experimental, pre-post study. For a six-month period (12/2023-05/2024), patients attended by an inpatient Palliative Care consultation service in a tertiary hospital were screened for spiritual needs using the Spiritual Needs Scr

Various "arbitrary factors" shaped healthcare chaplains' experiences during the early phase of the COVID-19 pandemic. The author's reflection provides personal recollection, uniquely situated in the particular local organizational dynamics and inter-personal relationships among the professionals, supplementing the qualitative survey articles by the field's on-going effort to collect, organize, and interpret the pandemic's impact on the spiritual care practitioners.

The COVID-19 pandemic impacted healthcare leadership, but much of the academic focus on leadership research has been on public health professionals and policymakers. Less attention has been given to the leadership roles of interfaith hospital chaplains. This study explores how interfaith hospital chaplains enacted leadership throughout the pandemic, focusing on their personal experiences, practices, and challenges. Utilizing Interpretative Phenomenological Analysis (IPA), the study involved in-depth interviews with ten interfaith chaplains from U.S. hospitals, all with at least six months of s

This paper describes the development and initial chaplaincy user evaluation of 'Pastoral Narrative Disclosure' (PND) as a rehabilitation strategy developed for chaplains to address moral injury among veterans. PND is an empirically informed and integrated intervention comprising eight stages of pastoral counselling, guidance and education that was developed by combining two previously existing therapeutic techniques, namely Litz et al's (2017) 'Adaptive Disclosure' and 'Confessional Practice' (Joob & Kettunen, 2013). The development and results of PND can be categorized into five phases. Phase

PURPOSE: This quality improvement study evaluated a novel, electronic medical record (EMR)-based order set designed to standardize interdisciplinary care coordination for patients undergoing lower extremity amputation at a large academic medical center. When activated, the order set automatically placed consultations for an interdisciplinary team consisting of physical therapy, occupational therapy, physical medicine and rehabilitation (PM&R), chaplaincy, and orthotics/prosthetics.
METHODS: Patients (n=637) who underwent lower extremity amputation were divided into 2 cohorts: those with and w

PURPOSE: Pain is a universal experience for hospitalized patients, with physical, psychological, spiritual, and cognitive implications. As hospitals seek to identify nonpharmaceutical options for managing acute pain, the role of chaplains has been overlooked. The purpose of this study was to evaluate the perceptions of nurses regarding chaplain involvement in pain management.
METHODS: A survey was distributed to nurses to determine if they would request spiritual care services in various patient and family scenarios. Respondents were dichotomized into two groups based on self-report of whethe

BACKGROUND: The death of a baby is recognised as one of the most difficult bereavements with life-long impact for parents. How bereaved parents are cared for influences their grief journey. Optimal holistic care is provided when the physical, emotional, spiritual and social needs of parents are attended to. This study reviewed how spiritual care is provided to bereaved parents following stillbirth in maternity units in Ireland and the impact of stillbirth on healthcare chaplains.
METHODS: This was a mixed methods study using semistructured qualitative interviews with hospital chaplains in Iri

The death of a baby is one of the most challenging bereavements for parents and healthcare professionals. This study explores the spiritual and theological issues raised for healthcare chaplains as they minister with parents following perinatal bereavement. Chaplains from 85% of maternity units in the Republic of Ireland participated in this study. Suffering, doubt and presence were the main theological themes raised for chaplains following perinatal death. The process of theological reflection is recommended as a sustaining and necessary tool in perinatal healthcare ministry.

Studies published in the last few years have suggested that the current practice in hospital obstetric units of encouraging parents to spend time with, hold, and even care for their stillborn fetus or baby may be deleterious to them. Rather than helping to allay grieving and successfully bring mourning to closure, mothers who had increasing levels of contact with the body of their stillborn baby were incrementally more likely to suffer depression and symptoms of post-traumatic stress disorder in their next pregnancy and to have difficulty with attachment to their next child. These findings par

BACKGROUND: Spiritual health is a vital aspect of holistic health linked to improved outcomes, yet little is known about approaches used in primary care to support this dimension of wellbeing.
OBJECTIVES: To identify which interventions have been used to improve patients' spiritual health in primary care, and what works, for whom, and in which contexts.
METHODS: A qualitative realist review was registered with PROSPERO and conducted in line with RAMESES standards. Systematic database searches were completed in May 2024 and June 2025. Iterative analysis and synthesis were informed by discussi
