Trichloroacetic acid chemexfoliation (chemical peel) for extensive premalignant actinic damage of the face and scalp.

Trichloroacetic acid chemexfoliation is an effective, versatile, and safe therapeutic option for patients with extensive actinic keratoses of the face and scalp. With an experienced and skilled operator, the associated complications are rare and tend to be mild. Cosmetically, the patient not only benefits from removal of the erythematous, scaly actinic keratoses but also obtains cosmetically pleasing rejuvenation of facial skin and diminution of fine facial wrinkles. Two representative cases are presented. In comparison with other available treatment modalities, trichloroacetic acid chemexfoli

Basic chemical peeling: Superficial and medium-depth peels.

Chemical peeling, or chemexfoliation, has been used for centuries to improve signs of ultraviolet light-induced sun damage. Over the last 30 years, the science behind chemical peeling has evolved, increasing our understanding of the role of peeling ingredients and treatment indications. The depth of peels is directly related to improved results and to the number of complications that can occur. Key principles for superficial and medium depth peeling are discussed, as well as appropriate indications for these treatments.

Chemical peel as an adjuvant treatment in pigmented contact dermatitis: a case series.

A chemical peel is chemexfoliation, a process of application of a chemical substance to the skin that causes controlled chemical destruction of the epidermis with or without part of the dermis leading to skin regeneration and remodeling. It can be classified depending upon the depth of penetration into superficial, medium, and deep peels. Among various indications, peels can be used to enhance treatment within a variety of conditions including skin- rejuvenation, inflammatory disorders like acne, rosacea, acne scar, and pigmentary disorders like melasma, freckles, lentigens, dyschromia, and po

A primer of facial chemical peel.

Facial chemical peel encompasses a plethora of procedures or techniques and so may be confusing. This should not blunt one's enthusiasm for what chemexfoliation can achieve. With the use of TCA, chemical peel is a safe, effective, elegant, and simple method for treating some common problems of photoaged, fair skin (Table 7). When a standardized technique is used, it is possible to quantify the therapeutic effects and to predict the outcome reliably. Variations of chemical peel will be used as clinicians try to achieve better results. If possible, variations from standard techniques should be s

[Chemical peeling for treatment of Melasma, pigmentary disorders and hyperpigmentation : Indications, effectivity and risks].

Pigmentary disorders and hyperpigmentation are widespread. Dark skin types in particular show a tendency to formation of melasmas and to hyperpigmentation. Light skin types have a tendency to ephelides and solar lentigines. In addition to topical treatment with lightening substances, superficial chemical peeling as well as combined procedures of topical treatment with chemical peeling play an important role in the treatment of hyperpigmentation. A strict avoidance of UV light and consequent daily application of sun protectíon factor 50+ are mandatory for successful treatment.

Chemical Peels in Skin Cancer: A Review.

Background: Chemexfoliation is widely used to reverse signs of photodamage. Although photodamage can eventually lead to skin cancer, it remains unclear whether chemical peels also affect photocarcinogenesis. Moreover, concerns about the systemic and/or cutaneous toxicity of peeling agents have already arisen. Objective: This review sought primarily to summarize the data available on the effects of chemical peels on ultraviolet-induced skin carcinogenesis, focusing particular attention on actinic keratoses and cutaneous field cancerization. In addition, considerations about the systemic and/or

Trichloroacetic acid and other non-phenol peels.

Medium peels are an excellent modality for treating actinically damaged skin. Because they create wounds that extend only down into the upper dermis, they have less risk of complications than deeper phenol peels. Conversely, because of their more superficial nature, they also are not as effective as phenol peels in improving deep scars and wrinkles. Cosmetic surgeons should consider medium-depth peels a useful addition to their current armamentarium against aging skin, rather than a replacement for phenol peels.

The art of the trichloroacetic acid chemical peel.

Despite escalating interest in laser resurfacing techniques, chemical peel procedures remain an integral part of the armamentarium for resurfacing aging, sun-damaged, and diseased skin. Like other chemicals, the advantage of the technique of tricholoracetic (TCA) acid application is that the degree and depth of penetration within the skin can be controlled accurately according to specific requirements. This article reviews the preoperative evaluation of skin and the techniques for preconditioning, intraoperative peel, and postoperative management of TCA-treated skin.

Trichloroacetic acid peels revisited.

Trichloroacetic acid (TCA) is a time-honored peeling agent that has no known systemic toxicity. Recent interest in reversing the effects of actinic damage has led to the utilization of combination therapies. Retinoic acid, solid CO2, pyruvic acid, Jessner's formula, and other agents have been used to increase the effectiveness of TCA and enhance peeling results. Repetitive peels with low concentrations of TCA can be effective for areas prone to scarring.

Treatment of Periorbital Vascularity, Erythema, and Hyperpigmentation.

Periorbital hyperpigmentation (POH) is a common aesthetic concern that impacts patients' emotional well-being and quality of life. POH can be difficult to manage as the etiology is often multifactorial or difficult to elucidate. An understanding of different contributing factors and ability to classify hyperpigmentation can aid in the management of POH. Classification of POH is divided into pigmented, vascular, structural, and mixed subtypes. A wide array of treatment options has been proposed belying the challenges inherent to improving POH. Modalities vary from topical therapies, chemical pe

Getting to the Core of Contemporary Therapies for Post-Inflammatory Hyperpigmentation.

Post-inflammatory hyperpigmentation (PIH) is a typical complication of inflammatory dermatoses that more frequently and severely affects people with darker skin. External insults to the skin, such as burn injuries, dermatologic treatments, and intrinsic skin disorders (eg, eczema and acne), are common causes of PIH. Individuals with darker skin are prone to develop PIH, which can cause substantial psychological suffering. PIH can be prevented or alleviated. When this happens, it is essential to point out what is causing it and treat it as soon as possible to prevent inflammation and PIH from p

Microdermabrasion assisted delivery of glycolic acid 70% peel for the treatment of melasma in dark-skinned patients.

Melasma is a disfiguring dermatologic condition and its treatment is still considered a challenge. To evaluate the efficacy and safety of microdermabrasion (MDA) combined with glycolic acid 70% (GA70%) peel versus GA70% alone in treating melasma in dark-skinned patients. This study included 30 female patients (skin type IV and V) with melasma. After cleansing the face, 3 passes of MDA were done on one side of the face. Then, GA70% was applied to the whole face in 1-2 uniform passes. Melasma area and severity index (MASI), modified MASI and hemi-MASI scores were used to assess the outcome. A si

[Chemical peel--current possibilities and limits].

The increased exposure to UV-radiation as well as the longer life expectation are the main reasons for photo-aging of the skin, which becomes more and more important in dermatology. Chemical peels represent potent agents, which meets not only the aesthetic aspect of skin rejuvenation, but also the medical aspects of skin aging. It allows to remove skin pigmentations, to smooth fine wrinkles and to improve the skin texture, and it can successfully be used in the therapy of actinic keratosis as well. The selection of the most suitable peeling-procedure is crucial for a successful treatment. It m
