BACKGROUND: Please see Appendix 4 for a glossary of terms. The outcome of patients with esophageal cancer is generally poor. Although multimodal therapy is standard, there is conflicting evidence regarding the addition of esophagectomy to chemoradiotherapy.
OBJECTIVES: To compare the effectiveness and safety of chemoradiotherapy plus surgery with that of chemoradiotherapy alone in people with nonmetastatic esophageal carcinoma.
SEARCH METHODS: We performed a computerized search for relevant studies, up to Feburary 2017, on the CENTRAL, MEDLINE, and Embase databases using MeSH headings and key

Over the past 30 years, significant advances have been made in the integration of radiation therapy and chemotherapy in the treatment of patients with localised gastrointestinal malignancies. The therapeutic goal of chemoradiotherapy is to enhance local control resulting in improved survival and outcome of these patients. To define the optimal sequence, agents and efficacy of these modalities, an array of randomised studies have been conducted in malignancies of the oesophagus, stomach, pancreas, colon, rectum and anus. In oesophageal cancer, recent studies from Germany and France indicate tha

BACKGROUND: The benefit of adjuvant chemoradiotherapy (CRT) for pancreatic adenocarcinoma (PAAD) is debated. This study aimed to elucidate the impact of adjuvant CRT on survival outcomes.
METHODS: We searched major medical databases for clinical and retrospective studies comparing postoperative CRT vs. no CRT in PAAD. Data were synthesized using RevMan 5.4 and Stata 17.0 software. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated for overall survival (OS), local recurrence, and distant metastasis.
RESULTS: Our analysis included 19 studies involving 4,581 patients.

While the clinical benefits of adjuvant chemotherapy are well documented in resected pancreatic ductal adenocarcinoma regardless of surgical margin status, the role of adding chemoradiation to adjuvant chemotherapy is an ongoing debate due to the paucity of clinical studies that concurrently incorporated contemporary chemotherapy regimens such as mFOLFIRINOX or gemcitabine/capecitabine with modern day radiation dose fractionation and techniques. Adjuvant chemoradiation should be tailored to individual patients and supported by multidisciplinary discussion. In R1 patients with good performance

OBJECTIVES: This study aimed to evaluate clinical outcomes after chemoradiation (CRT) for the definitive (nonsurgical) treatment of vulvar cancer.
MATERIALS AND METHODS: Women with vulvar cancer treated with definitive CRT at a single academic institution between 1994 and 2015 were retrospectively identified. Overall survival (OS), freedom from local recurrence, freedom from distant recurrence, and late toxicities were estimated using the Kaplan-Meier method at 3 years after radiotherapy completion. Univariate Cox regression models were used to estimate the effects of risk factors on these cl

BACKGROUND: The combination of high-dose radiotherapy with intensive chemotherapy may improve the prognosis of patients with inoperable, locally advanced non-small-cell lung cancer. In the design of suitable clinical protocols, potential interactions of drugs and radiation in the tumour, the lung and other critical normal tissues have to be considered.
METHODS: From experimental data on chemotherapy radiotherapy interactions and based on knowledge about the biology of non-small cell lung cancer and the pathogenesis of radiation pneumopathy, the principles which should be considered in the des

PURPOSE: The goal of this work was to analyze the outcome of adjuvant chemoradiotherapy for patients with gallbladder cancer who underwent surgical resection and to identify the prognostic factors for these patients.
PATIENTS AND METHODS: Between August 1989 and November 2006, 47 patients with gallbladder cancer underwent surgical resection followed by adjuvant radiotherapy. There were 21 males and 26 females, and median age was 60 years (range 44-75 years). Postoperative radiotherapy was delivered to the tumor bed and regional lymph nodes up to 40-50 Gy at 2 Gy/fraction; 41 patients also rec

Salvage surgery is the sole curative-intent treatment option for patients with esophageal cancer after definitive chemoradiotherapy. The most significant factor associated with long-term survival appears to be RO resection. Patients who undergo salvage esophagectomy have high morbidity and mortality rates. Extended three-field lymphadenectomy should be limited in salvage surgery. Ischemic tracheobronchial lesions are serious complications of salvage esophagectomy. The right posterior bronchial artery should be preserved, and neck dissection should be avoided to preserve the blood supply from t

BACKGROUND: Gallbladder cancer is generally associated with a poor prognosis, being local recurrence the main pattern of failure.
AIM: To evaluate neoadjuvant chemoradiation as a means to improve the prognosis in gallbladder cancer.
PATIENTS AND METHODS: Twenty three gallbladder cancer patients were prospectively treated between June 1993 and September 1999 in the Temuco Regional Hospital. Eighteen (82%) patients had subserosal infiltration, while three (13%) had serosal and two (9%) adipose tissue infiltration. Chemotherapy was done with 5-fluorouracil in continuous infusion during 5 days a

Radiation therapy with concomitant chemotherapy is the standard treatment for non resectable esophageal carcinoma. For patients with operable tumors, surgery is the traditional treatment. However several data have suggested that preoperative chemo- and radiotherapy could improve therapeutic results. At the present time, no randomized trial has demonstrated, except for adenocarcinoma of the cardia, the benefit of preoperative treatment. Other randomized trials are needed to determine the role and the optimal modalities of these treatments. This is a review of the literature data in concomitant

Radiotherapy may be used in the treatment of vulval cancer as an alternative to surgery in unfit patients, as an adjuvant to surgery in patients with poor prognosis tumours and for the treatment of inoperable, recurrent and metastatic disease. High-energy X-rays, electrons and both superficial mould and interstitial brachytherapy may be integrated in the regimen to produce the maximum tumour control and minimum morbidity. Concomitant chemoradiotherapy has a high response rate and may be used before surgery to reduce the morbidity of otherwise sphincter-sacrificing procedures. This chapter pres

Radiotherapy is a fundamental part of the treatment of pelvic neoplasms. Up to 90% of patients develop gastrointestinal symptoms as a result of acute injury to the small and large intestine, particularly in the mucosa. Radiotherapy leads to atrophy of the intestinal epithelium, acute crypt inflammation, inflammatory infiltration of the epithelium, malabsorption of lactose, and biliary salts as well as alterations in pancreatic enzymes and biliary salts, resulting in the malabsorption syndrome and dysbiosis. The most commonly reported symptoms of pelvic radiation disease include changes in bowe

AIMS: Chemoradiotherapy is the standard treatment for advanced Oropharyngeal squamous cell carcinoma (OPSCC). Upcoming hypofractionation has led to better compliance and non-inferior results in various sites such as breast and prostate cancer etc. This study prospectively compared a dose-intensified schedule in advanced OPSCC with standard hypofractionation.
MATERIALS AND METHODS: Patients with advanced stage III and IV OPSCC suitable for radical chemoradiotherapy were eligible. Patients were alternatively allocated to both the treatment arms. Arm A planned to receive 64 Gy in 25 fractions (

BACKGROUND: Glioblastoma multiforme (GBM) is an aggressive brain tumor with poor survival rates despite current treatments. The standard of care (SOC) includes surgery, followed by radiotherapy plus concurrent and adjuvant chemotherapy with temozolomide (TMZ). This phase II trial assessed the safety and efficacy of neoadjuvant TMZ (nTMZ) before and during chemoradiotherapy in newly diagnosed GBM patients.
METHODS: Newly diagnosed GBM patients who underwent maximal safe resection were randomized into 2 groups. One received nTMZ before standard chemoradiotherapy and adjuvant TMZ (intervention).

Invasive tracheobronchial aspergillosis is a rare disease with most reported cases in the literature occurring in immunocompromised hosts. We report an unusual case of a patient with persistent cough and dyspnoea in the context of prior chemoradiotherapy for primary lung cancer. Chest computed tomography (CT) demonstrated an abnormal soft tissue mass surrounding the trachea and carina, with focal moderate fluorodeoxyglucose (FDG) activity on positron emission tomography (PET) scan. Bronchoscopic biopsy revealed fungal hyphae associated with necrosis and cartilage invasion, subsequently confirm

BACKGROUND: Carotid body tumors (CBTs) are rare paragangliomas with an incidence of 1-2 per 100,000 individuals. While surgical resection remains the standard treatment, postoperative recurrence poses a clinical challenge.
CASE PRESENTATION: We describe a 49-year-old female with recurrent CBT involving the jugular foramen, managed with IGRT (60 Gy/30 fractions) and 4 cycles of CVD chemotherapy. Post-treatment RECIST evaluation revealed stable disease, supporting chemoradiotherapy as a salvage option for anatomically complex recurrences.
CONCLUSION: This case underscores the necessity for sta

In the guidelines on American National Comprehensive Cancer Network, local excision with adequate margin is recommended as a primary treatment for patients with T1, N0, and well-differentiated anal margin cancers. Otherwise, concurrent chemotherapy using mitomycin C(10mg/m², day 1 and 29)and 5-FU(1,000mg/m2/day, continuous intravenous infusion, day 1-4 and 29-32)with radiation(total dose of 45-59 Gy)is the recommended primary treatment for all other stages of nonmetastatic anal margin and anal canal cancer. Abdominoperineal resection is performed for patients with local recurrent diseases or r

PURPOSE: Extrahepatic cholangiocarcinoma is a rare malignancy. Despite radical resection, survival remains poor, with high rates of local and distant failure. To clarify the role of radiotherapy with chemotherapy, we performed a retrospective analysis of resected patients who had undergone chemoradiotherapy.
METHODS AND MATERIALS: A total of 45 patients (13 with proximal and 32 with distal disease) underwent resection plus radiotherapy (median dose, 50.4 Gy). All but 1 patient received concurrent fluoropyrimidine-based chemotherapy. The median follow-up was 30 months for all patients and 40 m

BACKGROUND: Thymic carcinoma is a rare anterior mediastinal neoplasm. It is more invasive and has a poorer prognosis than ordinary thymoma. Complete curative resection is frequently impossible to achieve because of extensive invasion or metastasis at diagnosis. The role of systemic chemotherapy and the optimal regimen in thymic carcinoma remain uncertain. We report our experience with 16 patients with unresectable thymic carcinoma who underwent concurrent chemoradiotherapy.
METHODS: Between July 1989 and July 2003, 29 patients were diagnosed with unresectable thymic carcinoma at our hospital.
