Adjuvant Chemoradiotherapy for Pancreatic Adenocarcinoma.

While the clinical benefits of adjuvant chemotherapy are well documented in resected pancreatic ductal adenocarcinoma regardless of surgical margin status, the role of adding chemoradiation to adjuvant chemotherapy is an ongoing debate due to the paucity of clinical studies that concurrently incorporated contemporary chemotherapy regimens such as mFOLFIRINOX or gemcitabine/capecitabine with modern day radiation dose fractionation and techniques. Adjuvant chemoradiation should be tailored to individual patients and supported by multidisciplinary discussion. In R1 patients with good performance

Adjuvant Chemoradiotherapy for Non-Pretreated Gastric Cancer.

BACKGROUND: While the curative approach to gastric cancer includes perioperative regimens in several countries, a substantial proportion of patients may not receive treatment prior to surgery. This study examines the adjuvant provision of chemoradiotherapy (CRT) for non-pretreated patients with cancer of the stomach including the gastric cardia. METHODS: All surgically treated patients with primary adenocarcinoma of the stomach and gastric cardia diagnosed between January 2004-December 2013 were selected from the Netherlands Cancer Registry. Patients who did not receive neoadjuvant treatment

Comparative efficacy of postoperative adjuvant chemoradiotherapy versus no chemoradiotherapy in the treatment of pancreatic adenocarcinoma.

BACKGROUND: The benefit of adjuvant chemoradiotherapy (CRT) for pancreatic adenocarcinoma (PAAD) is debated. This study aimed to elucidate the impact of adjuvant CRT on survival outcomes. METHODS: We searched major medical databases for clinical and retrospective studies comparing postoperative CRT vs. no CRT in PAAD. Data were synthesized using RevMan 5.4 and Stata 17.0 software. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were calculated for overall survival (OS), local recurrence, and distant metastasis. RESULTS: Our analysis included 19 studies involving 4,581 patients.

Role of chemoradiotherapy in oesophageal cancer -- adjuvant and neoadjuvant therapy.

Despite low postoperative mortality rates, the long-term outcomes from surgical-based treatment for oesophageal cancer remain poor. Chemoradiotherapy (CRT), either given before surgical resection as neoadjuvant therapy or after resection as adjuvant therapy, has been postulated to improve these outcomes. This systematic review examines the evidence for these approaches. The evidence for postoperative radiotherapy is limited and conclusions are difficult, but it may have a role in patients at high risk of local relapse (positive margins). The addition of chemotherapy is recommended when possibl

Restaging after neo-adjuvant chemoradiotherapy for N2 non-small cell lung cancer.

Recent studies have shown that patients who are down-staged via neoadjuvant therapy and undergo resection have a significant increased 5-year survival rate (as high as 40%-50%) when compared with patients who have residual N2 disease. The identification of patients who are N2 negative after the completion of their neoadjuvant therapy is a critical component of proper patient selection for thoracotomy. Some may even argue that it is a necessary step before resection. In this article we review the best ways to restage patients with N2 disease after they have completed their neoadjuvant therapy.

De-intensified adjuvant (chemo)radiotherapy versus standard adjuvant chemoradiotherapy post transoral minimally invasive surgery for resectable HPV-positive oropharyngeal carcinoma.

BACKGROUND: More than 400,000 cases of oropharyngeal squamous cell cancer (OPSCC) are diagnosed every year worldwide and this is rising. Much of the increase has been attributed to human papillomavirus (HPV). HPV-positive OPSCC patients are often younger and have significantly improved survival relative to HPV-negative patients. Traditional management of OPSCC has been with radiotherapy with or without chemotherapy, as this was shown to have similar survival to open surgery but with significantly lower morbidity. Techniques have evolved, however, with the development of computerised planning a
