Avoidance strategies in an exceptional child during unsuccessful reading performances.

Employing an interactional analysis and focusing on the complexity of meaning making, this study investigates the uses of several types of avoidance strategies generated by a language-disordered boy who is struggling with literacy. The results suggest that these avoidance strategies may function as compensatory adaptations that assist him in overcoming his literacy limitations so that he can still sustain effective social action even within contexts where his literacy difficulties are highlighted. There are both theoretical and practical implications for these findings.

Exasperating or exceptional? Parents' interpretations of their child's ADHD behavior.

OBJECTIVE: Attention Deficit/Hyperactivity Disorder (ADHD) is a commonly diagnosed childhood disorder associated with parent-child conflict and parental stress. This investigation explored whether parents' interpretation of symptomatic behavior predicted negative interactions with and perceptions of their child. METHOD: We recruited parents of 7-12 year old children who were diagnosed with ADHD (n = 41), were diagnosed with ADHD but whose parents construed symptomatic behavior positively (Indigo children; n = 36), and had no diagnosis (n = 26). Parents completed a questionnaire about their ex

Exceptional verbal intelligence after hemispherotomy in a child with Rasmussen encephalitis.

We report a longitudinal case study of a left-handed girl who underwent left hemispherotomy at 7 years for Rasmussen encephalitis (RE). Presurgical evaluation showed mild hemiparesis, no visual defect, and light neuropsychological impairment with short-term memory weakness. Language fMRI showed a right hemispheric dominance. Postoperatively, the patient exhibited right hemiplegia and hemianopsia but preserved intellectual capacities. She became seizure-free, and antiepileptic medication was discontinued. Long-term follow-up showed very high verbal intelligence at 11 years of age (VCI of 155) a

For Infants With Fractures: Involvement of a Child Protection Team Is Mandatory With Few Exceptions.

OBJECTIVE: The objective of this study was to compare the frequency at which abuse is detected in institutions with mandatory skeletal surveys for infants with fractures to that in institutions with discretionary referral to child protection teams (CPTs). METHODS: A retrospective chart review of all infants with fractures diagnosed at an emergency department from 2014 to 2018 was conducted to analyze factors leading to a discretionary referral to CPTs and to identify the frequency of nonaccidental trauma. RESULTS: Seventy-two infants with a median age of 6 months were included in this study.

Neuropsychological functioning of an Asperger child with exceptional skill in arranging picture stories.

A striking special ability in arranging picture stories was reported in an Asperger child (C.M.) showing an exceptional performance on Wechsler picture arrangement subtest. Neuropsychological examination did not disclose visuoperceptual and spatial defects, or working memory, attention and executive disorders, but revealed an attentional bias towards local details of complex structures. A specific assessment of C.M.'s understanding of picture stories demonstrated that, with respect to normal controls, he showed an enhanced ability to detect causal links among elements of a story. These finding

Leprosy in an Eight-Year-Old Child - An Exceptional Case with Unusual Oral Manifestation.

Leprosy is a contagious and granulomatous disease which is caused by Mycobacterium leprae. The incubation period for leprosy is five to seven years and it can take as long as about 30 years before signs and symptoms of leprosy develop in some patients. It is a chronic systemic disease which mostly affects the skin and peripheral nerves. It has been seen that indeterminate leprosy is the most common type seen in childhood followed by tuberculoid variant. Borderline lepromatous and lepromatous leprosy are only occasionally encountered in children and rarely affects the oral cavity. Here, we repo

[Lumbar spinal cord injury without radiological abnormality in a child: an exceptional form of presentation].

SCIWORA is an uncommon syndrome affecting mainly children and is defined as the occurrence of acute spinal cord injury despite normal plain radiography and normal computed tomography (CT). Lumbar SCIWORA is very rare in children, and to our knowledge, there is only one report of lumbar SCIWORA in the literature. We present the case of a 5 year-old boy who suffered acute bilateral lower limbs paralysis, associated to urinary and bowel incontinence following a 1.5 meter fall. Lumbar cord contusion could be demonstrated on MRI without other radiologic abnormalities, which confirmed SCIWORA diagno

An exceptional case of urogenital sinus associated with a scrotal pouch and duplicate urethra in a female child.

We report the case of an unusual genitourinary anomaly in a 5-year-old patient. The malformation consisted of a para-anal persistent urogenital sinus in a 46XX patient associated with a scrotum-like structure in the presumed clitoral location. A duplicate urethra arising from the anterior aspect of the bladder neck drained into the scrotal pouch. Two hemivaginas drained into the urogenital sinus. The patient was totally continent through the urogenital sinus but the duplicate urethra was incontinent. No hormonal problems were encountered. The girl underwent total urogenital sinus mobilization,

[Pulmonary and cutaneous lesions and terminal ileitis in a 11-year-old child: an exceptional case (author's transl)].

A 11-year-old child presented with terminal ileitis associated with pulmonary lesions showing periodic progression. The initial thoracic disorder developed two years before radiological evidence of the ileitis, the second episode coinciding with the diagnosis of the ileal lesion. Pulmonary lesions progressed simultaneously with the clinical signs of digestive tract disturbances, which are now stabilized. The exceptional cases of respiratory affections in Crohn's disease reported in the published literature are reviewed. The diagnosis of these little known associations is still further complica

Primary Hodgkin's Lymphoma of bone in 7-year-old- an exceptional case report of youngest child in literature.

Hodgkin lymphoma (HL) is mainly considered a nodal disease but extra nodal involvement can also be seen with variable frequency. Solitary bone involvement in HL known as Primary osseous hodgkin lymphoma (POHL) is very rare. It is defined as a lymphoma that is restricted to the bone without any simultaneous organ or lymph node involvement at the time of initial diagnosis. Primary Hodgkin lymphoma of the bone can be very challenging to diagnose because of its rarity especially in children and its variable presentation. Here we report the youngest case of POHL in a girl 7 years of age. She presen

Managing fetal alcohol spectrum disorder in the public school system: a needs assessment pilot.

BACKGROUND: Published data and Canadian population reports suggest that approximately 1% of students in Toronto may have learning problems related to Fetal Alcohol Spectrum Disorders (FASDs). It is therefore imperative to understand how the needs of affected students are being met by various practitioners in their school environment. To date no comprehensive follow-up studies on FASD-affected children, families and educators in Toronto public schools are available. Documentation of school experiences associated with FASDs is needed to aid in developing appropriate and efficient intervention mo

Care coordination for children and youth with special health care needs: a descriptive, multisite study of activities, personnel costs, and outcomes.

OBJECTIVES: Objectives included testing use of the care-coordination measurement tool in pediatric primary care practices; describing care-coordination activities for children and youth that occur in primary care practices; assessing the relationship of care-coordination activities in the medical home with outcomes related to resource use; and measuring the direct personnel costs of care-coordination activities. METHODS: Six general pediatric practices were selected, representing a diverse range of sizes, locations, patient demographics, and care-coordination activity model types. The care-co

Neurobiological findings in 20 relatively gifted children with Kanner-type autism or Asperger syndrome.

Seventeen children with classical Kanner autism and three children with Asperger syndrome, all of whom had Full-scale IQs above 65, were subjected to a neurobiological assessment, including CAT scan, auditory brainstem response, EEG, chromosomal cultures, cerebrospinal fluid, blood and urine examinations and a thorough physical examination. 15 of the 20 children had 'definite abnormalities' on at least one of these examinations. It is concluded that the number of cases of 'non-organic' autism, even among children with relatively higher intelligence, dwindles rapidly as our neurobiological asse

Necrotic carpal tunnel syndrome in a child.

Median nerve entrapment at the wrist level causes carpal tunnel syndrome (CTS). Although frequent in adults, CTS is a rare entity in children. Bouvier described an exceptional necrotic variant in 1979 in which skin, nail, and bone lesions are typical. We report the case of a 10-year-old child with necrotic CTS secondary to trauma. To our knowledge, this is the first case reported in a child.
