Cholinesterase reactivators: the fate and effects in the organism poisoned with organophosphates/nerve agents.

Understanding the mechanism of action of organophosphates (OP)/nerve agents -- irreversible acetylcholinesterase (AChE, EC 3.1.1.7) inhibition at the cholinergic synapses followed by metabolic dysbalance of the organism -- two therapeutic principles for antidotal treatment are derived. The main drugs are anticholinergics that antagonize the effects of accumulated acetylcholine at the cholinergic synapses and cholinesterase reactivators (oximes) reactivating inhibited AChE. Anticonvulsants such as diazepam are also used to treat convulsions. Though there are experimental data on a good therapeu

No-observed-adverse-effect-level (NOAEL) assessment as an optimized dose of cholinesterase reactivators for the treatment of exposure to warfare nerve agents in mice.

Despite the international convention on the prohibition of chemical weapons ratified in 1997, the threat of conflicts and terrorist attacks involving such weapons still exists. Among these, organophosphorus-nerve agents (OPs) inhibit cholinesterases (ChE) causing cholinergic syndrome. The reactivation of these enzymes is therefore essential to protect the poisoned people. However, these reactivating molecules, mainly named oximes, have major drawbacks with limited efficacy against some OPs and a non-negligible ChE inhibitor potential if administered at an inadequate dose, an effect that they a

Potent inhibition and limited reactivation of human cholinesterases by cholesterol- and quinoline-based oximes.

Current oxime antidotes exhibit poor blood-brain barrier (BBB) penetration due to their permanent positive charge, limiting the reactivation of organophosphorus (OP)-inhibited cholinesterases in the central nervous system. To address this limitation, the present study investigates nine lipophilic cholesterol- and quinoline-based oximes as potential reactivators of human acetylcholinesterase (hAChE) and butyrylcholinesterase (hBChE) inhibited by OP compounds. Several oximes exhibited strong reversible inhibition with pronounced potency toward hBChE (Ki as low as 4.6 nM). However, despite the fa

Halogenated monopyridinium oximes are less effective in reactivation of phosphylated cholinesterases than bisquaternary oximes.

Mono-quaternary pyridinium oximes derived from K-oximes K027, K048 and K203 were designed, synthesized and evaluated for the reactivation of organophosphate-inhibited cholinesterases. The incorporation of the halogen atoms to the structure decreased the pKa value of the oxime group resulting in an increased formation of oximate necessary for reactivation. The stability and pKa values were found to be similar to analogous bis-quaternary compounds. Some mono-quaternary oximes resulted as relatively strong inhibitors of human acetylcholinesterase. Nevertheless, the reactivation ability of mono-qu

Imidazole as a Pendant Reactivation Ligand Increases Efficacy Scope for Reactivation and Resurrection of Organophosphorus-Inhibited/Aged Cholinesterases by Quinone Methide Precursors.

Organophosphorus (OP) inhibition of acetylcholinesterase (AChE) continues to pose a deadly risk to human health. Despite decades of research on oximes, improved therapeutics are still needed as most oximes fail to cross the blood-brain barrier, none exhibit efficacy against the OP-aged form of AChE, and reactivation by oximes results in a toxic byproduct. However, despite efforts to replace oxime therapeutics, many of the new candidates fall short in broad-scope activity or sufficient efficacy relative to the oxime therapeutics. Previously, researchers have used imidazole or Mannich phenol moi

Phosphine-induced human erythrocyte cholinesterase inhibition is reactivable by oximes: A step towards antidote development for phosphine.

Phosphine-releasing metal phosphides are used to protect stored grains. Phosphine gas can be fatal if inhaled. However, the easy availability of metal phosphides (like Aluminium phosphide) causes increased misuse to commit suicide. Phosphine is a mitochondrial poison and affects the body's major organs. Phosphine poisoning has no specific diagnostic biomarker or remedy/antidote. It is reported that phosphine inhibits erythrocyte cholinesterase activity. However, contradictory views regarding this issue exist, and no consensus has been reached yet. Further, it is completely unknown that even if

Next-generation broad-spectrum reactivators for effective countermeasure against organophosphorus poisoning.

Sustained effort has been dedicated to the development of novel cholinesterase reactivators-the only causal antidotes-to counter organophosphorus (OP) intoxication. As more lethal nerve agents-such as A-agents-continue to emerge, the existing arsenal of causal antidotes remains unchanged. Approved oxime reactivators-2-PAM, HI-6, and LüH-6-are restricted by their limited efficacy spectrum, poor blood-brain barrier permeability, and suboptimal pharmacokinetics. The objective of this study is to design, synthesize, and characterize a new class of asymmetric monoquaternary bisoxime reactivators wi

Some pharmacological correlations of hypothermia induced by anticholinesterasics.

The investigations have been performed on Wistar rats intoxicated with paraoxon in toxic sublethal doses. There have been measured the variations of rectal temperature at various time periods following the anticholinesterase agent. The authors established the pharmacodynamic correlations of paraoxon-induced hypothermia with cholinesterase reactivators (toxogonin, isonitrosine), anticholinergic substances (atropine, butylscopolamine), carbamic anticholinesterase (eserine, neoeserine) and chlorpromazine. The efficiency of atropine and cholinesterase reactivators in antagonization of hypothermia

A nomogram for predicting progression-free survival in stage III-IV gastric cancer based on post-treatment cholinesterase and C-reactive protein.

BACKGROUND: Serum cholinesterase (CHE) and C-reactive protein (CRP) are established prognostic markers in gastric cancer, but their levels after two cycles of therapy are rarely incorporated into predictive models. This study aimed to develop a nomogram integrating post-treatment CHE and CRP levels after two cycles of first-line therapy to predict progression-free survival (PFS) in patients with stage III-IV gastric adenocarcinoma. METHODS: This retrospective study included 104 treatment-naïve patients with stage III-IV gastric adenocarcinoma who received first-line therapy. Serum CHE and CRP

Unwashed grapes as a cause of organophosphate poisoning in a child in rural India.

This report describes the symptoms of pesticide poisoning in a previously healthy teenage girl. After consuming unwashed grapes for several days, the girl developed the following symptoms: drowsiness, vomiting, truncal weakness and fasciculations in the tongue and hands. Blood tests confirmed exposure to a small amount of the organophosphate (OP) compound, a type of chemical found in certain pesticides. The girl was treated with supportive care and cholinesterase reactivators, which minimised the damage caused by OP poisoning. Within 48 hours, the girl's symptoms improved and she made a full r

[Prevention of abnormalities induced by 2 organophosphate insecticides (parathion and bidrin) in quail embryos].

When Quail embryos, injected with bidrin at the unincubated stage, are treated with nicotinamide, beak and leg abnormalities are prevented, but niacin has no beneficial effect on the axial deformities caused by both parathion and bidrin. In contrast, vertebral defects are greatly reduced by giving pralidoxim, an antidote known and used in organophosphorus intoxications. But this compound has no effect on beak and leg damage caused by bidrin. Two other cholinesterase reactivators, diacetylmonoxime and monoisonitrosoacetone have any effect on organophosphorus abnormalities and have no antiterato

Efficacy Assessment of an Uncharged Reactivator of NOP-Inhibited Acetylcholinesterase Based on Tetrahydroacridine Pyridine-Aldoxime Hybrid in Mouse Compared to Pralidoxime.

(1) Background: Human exposure to organophosphorus compounds employed as pesticides or as chemical warfare agents induces deleterious effects due to cholinesterase inhibition. One therapeutic approach is the reactivation of inhibited acetylcholinesterase by oximes. While currently available oximes are unable to reach the central nervous system to reactivate cholinesterases or to display a wide spectrum of action against the variety of organophosphorus compounds, we aim to identify new reactivators without such drawbacks. (2) Methods: This study gathers an exhaustive work to assess in vitro and

Clinical characteristics and treatment of mixed-pesticide poisoning in a patient: reflections on a particular case.

Patients who commit suicide often deliberately hide their medical history. Given that taking pesticides is one of the most common methods of suicide, other forms of poisoning may be neglected in clinical practice. We report here a case of mixed-pesticide poisoning. The patient was poisoned by oral administration of a coumarin rodenticide in combination with an intramuscular injection of organophosphorus (OP) pesticide. The patient was treated with vitamin K1, cholinesterase reactivators, atropine, ventilator-assisted ventilation, and bedside debridement. Her condition gradually stabilized and

Therapy against organophosphate poisoning: the importance of anticholinergic drugs with antiglutamatergic properties.

Potent cholinesterase inhibitors (e.g., soman, sarin), induce a wide range of deleterious effects including convulsions, behavioral impairments and ultimately, death. Due to the likelihood of various scenarios of military or terrorist attacks by these and other chemical weapons, research has to be aimed at finding optimal therapies. Early accumulation of acetylcholine in synaptic clefts was suggested to trigger an array of toxic events including an excessive release of glutamate, culminating in the activation of its receptors. Stimulation of the N-Methyl-D-Aspartate (NMDA) subtype of these rec

Toxic Injury to Muscle Tissue of Rats Following Acute Oximes Exposure.

Therapeutic application of newly developed oximes is limited due to their adverse effects on different tissues. Within this article, it has been investigated which morphological changes could be observed in Wistar rats after the treatment with increasing doses of selected acetyl cholinesterase reactivators - asoxime, obidoxime, K027, K048, and K075. Subsequently, heart, diaphragm and musculus popliteus were obtained for pathohistological and semiquantitative analysis 24 hrs and 7 days after im administration of a single dose of 0.1 LD50, 0.5 LD50, and 1.0 LD50 of each oxime. Pralidoxime as an insignificant reactivator in severe anticholinesterase (organophosphate insecticide) poisoning.

In acute severe anticholinesterase poisoning by organophosphate compounds, pralidoxime (P-2-AM, pyridine-2-aldoxime methiodide) used in the recommended doses, intravenously, has not been shown to reactivate the inhibited cholinesterase, as evidenced both clinically and biochemically. In vitro studies using pralidoxime iodide up to ten times the recommended concentrations, produced insignificant reactivation of cholinesterases inhibited by the organophosphate insecticide Bidrin (di-methyl-3-hydroxyl-N, N-dimethyl-crotonamide phosphate). This was even so despite prolonged exposure of the inhibit

Freeze-drying of HI-6-loaded recombinant human serum albumin nanoparticles for improved storage stability.

Severe intoxications with organophosphates require the immediate administration of atropine in combination with acetyl cholinesterase (AChE) reactivators such as HI-6. Although this therapy regimen enables the treatment of peripheral symptoms, the blood-brain barrier (BBB) restricts the access of the hydrophilic antidotes to the central nervous system which could lead to a fatal respiratory arrest. Therefore, HI-6-loaded albumin nanoparticles were previously developed to enhance the transport across this barrier and were able to reactivate organophosphate-(OP)-inhibited AChE in an in vitro BBB

Molecular Modeling Studies on the Multistep Reactivation Process of Organophosphate-Inhibited Acetylcholinesterase and Butyrylcholinesterase.

Poisoning with organophosphorus compounds used as pesticides or misused as chemical weapons remains a serious threat to human health and life. Their toxic effects result from irreversible blockade of the enzymes acetylcholinesterase and butyrylcholinesterase, which causes overstimulation of the cholinergic system and often leads to serious injury or death. Treatment of organophosphorus poisoning involves, among other strategies, the administration of oxime compounds. Oximes reactivate cholinesterases by breaking the covalent bond between the serine residue from the enzyme active site and the p
