Chordoma.

Four cases of intracranial chordomas are reported. Intracranial chordomas initially tend to occur with sixth nerve paresis and subsequently with visual disturbance and optic disc changes. Computed tomography demonstrated an isodense mass in one case, a low-density mass in another case, and both an isodense and a partial high-density in two cases. Histopathologically, isodense areas were coincident with regions of compact tumor cells, low-density areas coincided with abundant mucinous intercellular matrices, and high-density areas were coincident with cartilaginous matrices or calcifications.

[Pharyngeal chordoma].

Chordoma is a neoplasm arising from embryonal notochord remnants. It is infrequent and rarely located at the cervical level. The diagnosis is histological and immunohistochemical tests are required to differentiate it from other neoplasms. We report a case of pharyngeal chordoma treated with a transmandibular approach. The diagnosis, treatment, and prognosis of these tumors are reviewed.

Sarcomatoid chordoma: chordoma with a massive malignant spindle-cell component.

We report a case of chordoma containing a spindle cell sarcomatoid component with a gradual transition from conventional chordoma. Immunohistochemically, many tumor cells in both conventional chordoma and sarcomatoid components were positive for cytokeratins (AE1/AE3, CAM5.2) and epithelial membrane antigen as well as vimentin. This report provides a rare example of sarcomatoid chordoma. Familiarity with this type of bone tumor should help to avoid confusion with dedifferentiated chordoma and other spindle cell sarcomas or carcinomas.

[Chordoma cutis, an unusual clinical presentation of a rare neoplasm: Chordoma].

Chordoma cutis represents an unusual clinical presentation of a rare neoplasm. The involvement of skin or sub-cutaneous soft tissues can be the consequence of local infiltration or metastasis; the latter may occur several years following the initial diagnosis of chordoma and therefore, may pose a diagnosis challenge when the clinical history of the patient is unknown. The clinical forms, morphology, immuno-histochemical profile and the main differential diagnoses of chordoma cutis are presented here through an anatomoclinical case.

Chordoma of the Oropharynx.

BACKGROUND: Chordoma is a rare malignant neoplasm that predominantly arises from the axial skeleton, but can also develop in unusual locations. However, there are also rare cases of "NOS" chordoma involving the oropharyx and epithelial-myoepithelial carcinoma of the parotid gland in the same patient. According to contemporary research, chordoma is a rare malignant neoplasm that arises from the embryonic remnants of the notochord. and typically involves the clivus, sacrococcygeal bones or vertebrae. Studies have shown that the incidence of chordoma has been estimated to be one per one million p

Chordoma of the thorax.

Chordomata are malignant tumours which arise from foetal notochordal rests. They occur alongside the vertebral column, mostly at the cranial or caudal extremities, but they are seldom found in the thorax. A description is given of a thoracic chordoma in a 70-year-old male patient who presented with difficulty in swallowing and breathing. Emphasis is placed on the role of computer-assisted tomography in reaching the diagnosis.

Spinal chordomas.

A series of 54 patients with spinal chordomas were treated at Memorial Sloan-Kettering Cancer Center between 1949 and 1976. Thirty-six lesions were located in the sacrococcygeal region and 18 involved the vertebral column at a higher level. The male to female ratio was 35:19. Vertebral chordomas generally occurred in a younger age group. Our radiological findings suggest that there is marked soft-tissue extension anterior to the vertebral column at the time of initial diagnosis. Eleven of 18 vertebral chordomas and 10 of 36 sacral chordomas were found to have disseminated metastases during the

Metastatic saccrococcygeal chordoma.

We report a 46-year-old man who presented with a 2week history of worsening headaches and acute onset left sided hemiplegia. He had undergone a surgical resection of a sacral chordoma 13years prior, followed by adjuvant radiotherapy and chemotherapy. MRI revealed multiple enhancing lesions in the brain, and the two largest were resected. The histopathology was consistent with chordoma. Sacrococcygeal chordomas are locally invasive notochord-related sarcomas. They rarely metastasize to the brain, and only eight patients have been reported. While currently available adjuvant radiotherapy and sys

Intradural cranial chordoma.

BACKGROUND: Intradural chordomas are rare and have been considered benign owing to the feasibility of complete resection and the display of lesser aggressive biologic behavior than typical chordomas. CASE DESCRIPTION: We herein reported 2 cases of intradural cranial chordoma with aggressive biologic behavior. A tumor (anti-Ki-67 monoclonal antibody [MIB-1], 13.9%) in a 59-year-old woman was strongly adherent to the brainstem and involved the basilar artery and its branches. After subtotal removal, the remnant tumor was treated with stereotactic radiotherapy. A tumor (MIB-1, 6.2%) in a 75-year

Paediatric Chordomas.

Paediatric chordomas are rare malignant tumours arising from primitive notochordal remnants with a high rate of recurrence. Only 5 % of them occur in the first two decades such less than 300 paediatric cases have been reported so far in the literature. In children, the average age at diagnosis is 10 years with a male-to-female ratio closed to 1. On the opposite to adults, the majority of paediatric chordomas are intracranial, characteristically centered on the sphenooccipital synchondrosis. Metastatic spread seems to be the prerogative of the under 5-year-old children with more frequent sacro-

Craniocervical chordomas.

In a 20 year period seven cases of craniocervical chordoma were seen and treated. Since it is not possible to completely resect these lesions, the aim of treatment has been to debulk the tumor as much as feasible and employ postoperative irridation. Palliation is the most that can be hoped for, and minimal success in delaying the natural history of the tumor has been achieved. Surgery for relief of symptoms secondary to bulky recurrence is indicated for palliation, as is a repeat course of x-ray therapy. Isolated cases of long-term useful survival have followed doses in the range of 10,000 rad

Giant Recurrent Sacral Chordoma.

A 65-year-old man presented with a giant recurrent sacral chordoma after undergoing 7 surgical resections. Neurologic examination revealed urinary incontinence. Magnetic resonance imaging showed a huge mass lesion in the bilateral gluteal regions and multiple metastatic chordomas. Colostomy and wide en bloc excision of the giant recurrent chordoma were performed in April 2017. The tumor weighed 13.15 kg, and histopathologic examination revealed a chordoma. Magnetic resonance imaging performed at the 12-month follow-up showed slight recurrence. In addition, multiple metastatic chordomas localiz

Cutaneous metastasis of chordoma.

BACKGROUND: Chordomas are rare neoplasms that arise from the notochord remnant. They develop in the sacrococcygeal (50%) or cervical (15%) region and are generally regarded as a locally aggressive tumor with a slow progressive growth rate and a metastatic incidence ranging from 3 to 48%. Skin involvement by chordoma is rare, but can occur by direct extension, by local recurrence and by metastases. OBJECTIVE: To illustrate by a case report the clinical presentation and management of this disease. METHODS: We present a case of sacral chordoma with metastases over a 10-year period to the lungs,

[Intrasellar chordoma (author's transl)].

The authors present two cases of intrasellar chordoma and review these tumor's most important characters. This intrasellar localization seems to be very rare. Endocrinological disorders, when observed, leads only to insfufficiency and never to hypersecretion. Final diagnosis is established by histological studies. Their volume is important, so that the recurrence after surgery is common and the middle term prognosis is bad.

Extraosseous chordoma of the nasopharynx.

BACKGROUND AND PURPOSE: Chordoma is a relatively rare tumor of the skull base and sacrum thought to originate from embryonic remnants of the notochord. Chordomas arising from the skull base/clivus are typically locally aggressive with lytic bone destruction. When chordomas occur in an extraosseous location, they may mimic other lesions of the nasopharynx. We present 5 cases of primarily extraosseous chordoma involving the nasopharynx in an effort to improve the preoperative diagnosis of this rare tumor. In addition, we review regional notochordal embryology to explain this variant tumor locati

Imaging of cranial chordomas.

The clinical, pathologic, and imaging characteristics of clival chordomas in 14 patients who underwent magnetic resonance examinations were evaluated. Magnetic resonance imaging (MRI) was compared with skull series, tomography, computed tomography (CT), and magnification angiography in the diagnosis of clival chordomas. Although all examinations were highly sensitive for the detection of clival chordomas, MRI was the best single study because of its ability to image in orthogonal planes, its excellent soft-tissue contrast, and its demonstration of the relationship between the neoplasm and regi
