The conservation of skin in Cialit was suggested by SANCHIS-OLMOS in 1954. The process was introduced into Switzerland at the Balgrist Hospital in 1957; it has found many applications in traumatological and orthopaedic surgery. Due to economical reasons some ophthalmologists are using an irrigating solution made by the hospital pharmacy instead of the commercially available solutions. These irrigating solutions come in bottles which are identical to the ones used for other solutions. During the last three years bottles were accidentally mixed up five times. In 20 patients with recurrent perforations of the tympanic membrane after tympanoplasties the membrane defect was closed by using cialit-conserved temporal fascia and human fibrin tissue adhesive. The defects differed in size from 2 mm up to a subtotal defect. In 19 cases there was increasing vascularisation from the perforation margins towards the transplant centre after 4 weeks. In seven three-month old piglets, tracheal defects were created experimentally and Cialit-fixed human tracheal transplants were grafted at the site of the defect. The face of a child with Down's syndrome is stigmatised by the large tongue, open mouth, saddle nose and epicanthal folds. Operative reduction of macroglossia facilitates nose-breathing, comprehensible speech and eating. Raising the nasal bridge with Cialit-preserved homograft cartilage obliterates the epicanthal folds and conceals the oblique slant of the eyelids. The child should be operated on from the third year onwards.

It is reported about the intraseptal implantation of allogeneic cartilago quadrangularis for the correction of saddle nose in the case of eleven patients. The indication for the implantation were non sufficient residuals of the behind septum for a substitution plastic. The sagging of the noses was caused by iatrogenic in 6 and traumatic influences in 5 cases. The conservation of the septal cartilage was carried out in Cialit 1:5000. All the septa healed in without any reaction and obtained satisfactory functional and cosmetic results.

In reconstruction of the orbital floor, homograft lyophilised dura or cialit-stord rib cartilage are suitable, but the best materials are autologous cartilage or silastic or teflon. Autograft bone from the maxilla or rib is rarely used. The surgery of traumatic hypertelorism and the reconstruction of the medial angle of the eye after orbito-nasal fractures are described and illustrated. With complete destruction of the floor of the orbit, a fronto-temporal pedicled flap combined with auricular cartilage is recommended. The aim of our study was to assess the behaviour of a despecified collagen tissue laid on desepidermized human skin (taking area of skin graft). The collagen tissue was prepared according to Bell's method. The collagen was latticed, contracted by fibroblasts, chemically treated (formaldehyde or glutaraldehyde) to stabilize the fibrils and then despecified by cialit treatment. This tissue laid on superficial, non infected wounds very rapidly took a necrotic appearance and was totally lysed after 10 to 12 days without any modification of the healing course. No bacterial infection was observed. H

We report the case history of a 54-year-old man who developed a fatal neurological disorder 4 years after a successful tympanoplasty with homograft pericardium. The final diagnosis of this case was Creutzfeldt-Jakob disease. This infectious spongiform encephalopathy is probably caused by a slow virus that can be transmitted by transplantation materials. The possible accidental transmission of Creutzfeldt-Jacob disease by the use of homograft materials in otologic surgery is discussed.
