Responding to clinical alarms in unfolding simulated clinical scenarios: auditory icons perform better than tonal alarms.

BACKGROUND: The international medical device safety standard IEC 60601-1-8 now recommends use of auditory icon alarms. Auditory icon alarms are alarms that act as metaphors for the problems that they signal. These are compared with traditional tonal alarms. METHODS: Two sets of three auditory alarms were compared, one consisting of auditory icons and one consisting of variants of older tonal alarms. Volunteer participants were required to monitor three clinical scenarios each lasting 4 min with different problems (cardiac, oxygenation, and technical) occurring during each of these scenarios t

False Crisis Alarms in Cardiopulmonary Monitoring:: Identification, Causes, and Clinical Implications.

The systematic annotation of crisis alarms reveals a high number of false alarms for both ventricular tachycardia and asystole, which are best identified by inspecting simultaneous multilead electrocardiographs. Among the few true crisis alarms, 11 were categorized as ventricular tachycardia, while the remaining 11 were classified as asystole. These false crisis alarms were primarily attributed to artifacts and low QRS voltage. Future research should use artificial intelligence that mimics critical care professionals to identify and evaluate false crisis alarms. This could enhance the algorith

When alarms are ignored: clinical reflections on ignorance culture in eating disorder care.

In his analysis of 'ignorance culture' in eating disorder services, Downs describes how repeated alarms raised by patients, carers and clinicians are routinely ignored, deflected or reframed as individual pathology. In this Opinion piece, I reflect on the clinical implications of that analysis, arguing that ignorance culture is enacted through everyday treatment structures that misread multi-layered presentations, invalidate advocacy and displace responsibility. Drawing on dialectical theory and biosocial frameworks, and using multidiagnostic eating disorder-dialectical behaviour therapy as an

Decreasing inconsistent alarms notifications: a pragmatic clinical trial in a post-anesthesia care unit.

BACKGROUND: Alarms alert healthcare professionals of deviations from normal/physiologic status. However, alarm fatigue may occur when their high pitch and diversity overwhelm clinicians, possibly leading to alarms being disabled, paused, and/or ignored. We aimed to determine whether a staff educational program on customizing alarm settings of bedside monitors may decrease inconsistent alarms in the Post-Anesthesia Care Unit (PACU). METHODS: This is a prospective, analytic, quantitative, pragmatic, open-label, single-arm study. The outcome was evaluated on PACU admission before (P1) and after

Glucose alarms approach with flash glucose monitoring system: a narrative review of clinical benefits.

Attaining an adequate glycemic control has been associated with a better prognosis and with a reduction in the risk of developing long-term microvascular and macrovascular diabetic complications. Continuous glucose monitoring (CGM) has been shown to improve glycemic control and reduce blood glucose variability. Furthermore, CGM is associated with greater treatment adherence and higher satisfaction. Hypoglycemia is the most frequent acute complication in individuals with insulin treated diabetes and may limit the achievement of glycemic control. Furthermore, repeated episodes of hypoglycemia, p

Clinical value of alarm features for colorectal cancer: a meta-analysis.

BACKGROUND: Colorectal cancer (CRC) is a common neoplasm in Western countries. Prioritizing access to colonoscopy appears of critical relevance. Alarm features are considered to increase the likelihood of CRC. Our aim was to assess the diagnostic performance of alarm features for CRC diagnosis. METHODS: We performed a systematic review and meta-analysis of studies reporting the diagnostic accuracy of alarm features (rectal bleeding, anemia, change in bowel habit, and weight loss) for CRC, published up to September 2021. Colonoscopy was required as the reference diagnostic test. Diagnostic acc

Proactive Prediction of Alarms in Hemodialysis Machines: A Comparative Evaluation of AI Models for Clinical Prognostic Support.

Background/Objectives: Hemodialysis is a treatment for patients with kidney disease, yet it remains risk-prone due to potential technical or physiological instabilities. Dialysis machines rely on reactive safety mechanisms that trigger alarms only when predefined thresholds are reached. This often alerts clinical staff only after a complication has already occurred, contributing to potential delays in interventions. This study aims to develop a proactive prognostic framework capable of anticipating system instabilities before they transform into clinical adverse events. Methods: This work prop

Fear of ventricular tachycardia in Syncope: Clinical reasoning versus reflex alarmism.

Despite widespread concern, ventricular tachycardia (VT) is rarely the actual cause of syncope, even in patients with structural heart disease. Indeed sustained VT occurs, it typically results in cardiac arrest rather than transient loss of consciousness, with the exception of torsades de pointes. On the other hand when syncope is arrhythmic, it is more commonly due to atrioventricular (AV) block or sinus node dysfunction, age-related conditions due to conduction system fibrosis. We argue that current guideline-based risk stratification may overestimate arrhythmic and sudden death risk in pati

Critical Care Nurses' Perceptions of Clinical Alarm Management on Nursing Practice.

The alarm management of physiological monitoring systems is a key responsibility of critical care nurses. However, the high numbers of false and nonactionable (true but clinically irrelevant) alarms cause distractions to healthcare professionals, interruptions to nursing workflow, and ignoring of crucial tasks. Therefore, understanding how nurses manage large amounts of alarms in their daily work could provide a direction to design interventions to prevent adverse patient care effects. A qualitative design with focus group interviews was conducted with 37 nurses in Taiwan. Content analysis was

Alarm Management Competence in Clinical Nursing Practice: An Evolutionary Concept Analysis.

AIMS: To clarify the defining attributes, antecedents, and consequences of alarm management competence in clinical nursing practice and to develop a conceptual definition and framework. BACKGROUND: Clinical alarms are integral to patient monitoring, yet effective alarm management requires more than technical device operation. Alarm-related knowledge, skills, judgment, behaviors, and fatigue have been examined across the literature, but the concept of alarm management competence remains insufficiently defined. EVALUATION: Rodgers' evolutionary concept analysis was conducted. PubMed, CINAHL, E

Monkeypox 2022 Outbreak: How Alarming Is the Situation? Epidemiological and Clinical Review.

Monkeypox is a disease caused by Orthopoxvirus, which also includes the smallpox virus. Several endemics have been reported on the African continent, typically in the western and central regions. However, since 13 May 2022, there have been several cases reported from different member states; the number of confirmed cases in 1 month exceeded the total number of cases reported outside the African continent since the first case in 1970. The World Health Organization (WHO) and Centers for Disease Control (CDC) consider monkeypox as an important disease for global public health. The clinical manife
