Telehealth clinical learning: understanding pre-clerkship medical student experiences.

BACKGROUND: With the onset of the COVID-19 pandemic, a reliance upon telehealth patient visits emerged. Many medical schools use early clinical experiences in the pre-clerkship years to provide opportunities to practice evolving clinical skills and broaden classroom learning. However, little is known about the value of telehealth visits during the pre-clerkship years. Therefore, the purpose of the current study was to determine what student learning experiences were with telehealth patient encounters during early clinical experiences. METHODS: In this qualitative study, we used a descriptive

Implementation of self-directed learning within clinical clerkships.

Background: The main aim of medical curricula is to prepare students for the first day at the work place. While teaching clinical competence is pivotal, clinical clerkships are often the last chance to close knowledge gaps with the help of clinical teachers. Self-directed learning is a dynamic field for research within medical education, though its curricular implementation is rare. This study focuses on the needs assessment of clinical clerkships using the concept of self-directed learning. Methods: The study comprised an educational experience at the Ludwig-Maximilians Universität (LMU) Muni

How medical students' trauma histories affect their clinical clerkship experiences.

BACKGROUND: As the gender demographics of medical students have evolved over the past decades, it is important to understand potential stressors and challenges that may affect clinical learning experiences. This study investigated the prevalence of prior sexual assault (SA) and interpersonal violence (IPV) in medical students and how these affect their clinical clerkship experiences. METHODS: A survey was distributed to third- and fourth-year medical students at a single institution in August 2022 querying respondents on demographics and prior experiences with SA/IPV at any point in their liv

Medical students' clerkship experiences and self-perceived competence in clinical skills.

INTRODUCTION: In a traditional curriculum, medical students are expected to acquire clinical competence through the apprenticeship model using the Halstedian "see one, do one, and teach one, approach". The University of Zambia School of Medicine used a traditional curriculum model from 1966 until 2011 when a competence-based curriculum was implemented. OBJECTIVE: To explore medical students' clerkships experiences and self-perceived competence in clinical skills. METHODS: A cross-sectional survey was conducted on 5th, 6th, and 7th year medical students of the University of Zambia, School of

A comparison of students' clinical experience in family medicine and traditional clerkships.

The clinical experience of 40 Duke University medical students during their required two-month clinical clerkships is analyzed to compare experience on the traditional internal medicine, surgery, pediatrics, obstetrics-gynecology, and psychiatry clerkships with experience on a new family medicine clerkship. On the traditional clerkships, the students saw mostly hospital inpatients for an average of 36 different patients for 142 encounters and 300 problem contacts during a two-month rotation. On the family medicine clerkship, they saw mostly office ambulatory patients for an average of 184 pati

Education of medical students in clinical breast examination during surgical clerkship.

BACKGROUND: Medical student training in clinical breast examination is deficient at most medical schools. The use of silicone breast models may allow the education of abnormal and normal findings. This study examines the efficacy of silicone breast models to educate medical students in clinical breast examinations during their third-year surgical rotation. METHODS: Medical students were randomly selected to participate in formalized training sessions in clinical breast examination or as a control group. Presession and postsession testing with silicone breast models were performed. True positi

Effect of clinical teaching on student performance during a medicine clerkship.

PURPOSE: To measure what proportion of student clerkship performance can be attributed to teachers' educational skills as reported by students. SUBJECTS AND METHODS: From August 1992 to June 1994, we collected critiques of teacher skills from 314 third-year students at the end of a 12-week medicine clerkship. Interns, residents, attending physicians, and student preceptors were rated (on a 1 to 5 scale) on teaching behaviors from the 7 categories of the Stanford Faculty Development Program framework. A linear regression model was used to determine the relative contributions of the rated teach

The outcomes of a telemedicine-based introductory clinical clerkship: an innovative educational model.

BACKGROUND: Telemedicine has long been recognized as an effective means of providing healthcare, yet it remained underutilized for decades. However, with the onset of the COVID-19 pandemic, telemedicine began to influence not only medical practice but also medical education. A remote telemedicine-based clerkship introduced to third-year medical students as a part of their introductory clinical education. The program aimed to enhance students’ experience with telemedicine as a tool for healthcare delivery. This study explores the outcomes of students’ experience with telemedicine as a tool for

Effectiveness of education on radiation exposure for medical students prior to clinical clerkship.

PURPOSE: The widespread use of radiation in diagnostic imaging, interventional procedures, and radiotherapy requires robust radiation safety education. Despite its importance, medical students in Japan and other countries have limited awareness of radiation protection. In Japan, radiation safety education is essential due to recent legislative changes that position medical students as "student doctors" and expand their clinical responsibilities. The purpose of this study was to evaluate the knowledge, concerns, and anxieties of 5th-year medical students regarding radiation exposure before clin

Planting the Seeds of Collaboration: A Pilot for School/Clinic Partnerships During Pediatric Clerkship.

Although pediatricians and family physicians often refer children to early intervention (EI) and provide support and information to families, medical school training that provides information about special education policy and procedures is often limited. We piloted a program whereby medical students, during their pediatric clerkship, observed school classrooms that included young children with disabilities. Visit impact was measured through assessments of perceived competency and a written reflection. Students showed perceived competency growth across all areas measured. Written reflections d

How Do Clinical Electives during the Clerkship Year Influence Career Exploration? A Qualitative Study.

Problem: Although many students begin medical school with some idea of their specialty interest, up to 80% of these students choose a different specialty by their final year. This pivot tends to happen in the clerkship year, when students are immersed in the clinical environment, gaining a practical understanding of the day-to-day work in different fields. Yet, in this year students have limited experiences with specialties. Clinical electives during the clerkship year may aid students in career development. The authors examined student career exploration through the lens of social cognitive c

Exploring Medical Students' Learning Through Interprofessional Interactions in Clinical Clerkships: A Qualitative Analysis.

PURPOSE: Medical students need help navigating clinical environments to find ways to engage in and learn from clinical work. Previous explorations focused on how physicians help students, largely neglecting the potential contributions of other health professionals (OHPs). This study explores how students learn to participate in clinical work through interactions with OHPs. METHOD: Using a constructivist grounded theory approach, researchers conducted 17 semistructured interviews with Harvard Medical School students completing clinical clerkships between 2023 and 2024. Students drew rich pictu

Offshore Medical Schools Are Buying Clinical Clerkships in U.S. Hospitals: The Problem and Potential Solutions.

U.S. medical education faces a threat from for-profit Caribbean medical schools which purchase clinical rotation slots for their students at U.S. hospitals. These offshore schools are monetizing a system that was previously characterized as a duty-the duty of the current generation of physicians to educate their successors. Offshore schools purchase clinical rotation slots using funds largely derived from federally subsidized student loans. This leads to pressure on U.S. schools to pay for clinical clerkships and is forcing some of them to find new clinical training sites.For-profit Caribbean

Lessons learned from the David Geffen school of medicine-UCLA clinical clerkship psychology of bias seminar.

Despite the documented benefits of a diverse physician workforce, significant barriers continue to limit some students' success within the clinical clerkship phase of their medical education. These students may experience unintended bias in grading and assessment that can limit their access to future opportunities. Anti-bias training is one strategy used to address these shortcomings in the assessment process. However, anti-bias training has been shown to be ineffective, resented by participants, and can even result in the opposite of its intended effects. Innovative training strategies to sup
