Pharmacometrics of clobazam in pediatrics: Prediction of effective clobazam doses for Dravet syndrome.

OBJECTIVE: To describe the use of a population pharmacokinetic (PopPK) model incorporating weight and ontogeny to identify effective clobazam (CLB) dosing for use in a clinical trial in pediatric patients with Dravet syndrome. METHODS: Pharmacokinetic data were combined from 3 CLB trials (OV-1012, OV-1017, and study 301) and a simulated study (study 401) for a total of 1306 CLB and 1305 N-desmethyl clobazam (N-CLB) samples from 193 Lennox-Gastaut syndrome patients and healthy subjects aged 6 months to 45 years. A structured approach based on US Food and Drug Administration guidance and pharma

Blood concentrations of clobazam and norclobazam in a lethal case involving clobazam, meprobamate and clorazepate.

Clobazam is a benzodiazepine with anti-anxiety and anticonvulsant properties marketed in several countries. Norclobazam, a metabolite of clobazam, has similar pharmacological activity but weaker sedative and tranquilizing effect. The two drugs were detected by GC-MS and determined by HPLC-DAD in the samples from a postmortem case. The femoral blood concentrations of clobazam and norclobazam were 0.72 and 36 μg/mL, respectively. The concentration of the active norclobazam was very high. The sum of both clobazam and norclobazam blood concentration (36.72 μg/mL) was clearly toxic, but was not nec

Clobazam as an adjunctive treatment for infantile spasms.

Infantile spasms constitute a catastrophic epileptic condition. Seizures in approximately half of children with infantile spasms fail to improve with initial treatment attempts; at present, data regarding alternative treatments are limited. We assessed the efficacy of clobazam as an adjunctive therapy in patients whose seizures failed to respond to initial regimens of standard treatment for infantile spasms. All patients from Severance Children's Hospital who received clobazam as adjunctive therapy for infantile spasms were selected for the study. The efficacy of clobazam was evaluated by asse

Clobazam monotherapy for focal or generalized seizures.

BACKGROUND: This is an updated version of the original Cochrane Review published in Issue 10, 2014. There is a need to expand monotherapy options available to a clinician for the treatment of new focal or generalized seizures. A Cochrane systematic review for clobazam monotherapy is expected to define its place in the treatment of new-onset or untreated seizures and highlight gaps in evidence. OBJECTIVES: To evaluate the efficacy, effectiveness, tolerability and safety of clobazam as monotherapy in people with new-onset focal or generalized seizures. SEARCH METHODS: For the latest update we

Clobazam : in patients with Lennox-Gastaut syndrome.

Clobazam, as with other benzodiazepines, has a long history of use in the treatment of epilepsy. More recently, it was approved in the USA as adjunctive therapy for the treatment of seizures associated with Lennox-Gastaut syndrome in patients aged ≥2 years. In the pivotal, placebo-controlled CONTAIN trial in paediatric and adult patients with Lennox-Gastaut syndrome (n = 217 evaluable), adjunctive therapy with clobazam 5-40 mg/day for 12 weeks significantly reduced mean weekly drop seizure rates from baseline compared with adjunctive placebo (primary endpoint), with a significant dosage-depend

Kinetics and metabolism of clobazam in animals and man.

1 The pharmacokinetic behaviour of the psychotropic drug clobazam, a 1,5 benzodiazepine, and its metabolism were studied with the 14C-labelled compound in rats, dogs, monkeys and man. The absorption was practically complete in all three animal species. Clobazam was not excreted in the unchanged form by all species. The main metabolite in plasma of monkeys, dogs and man was N-demethylclobazam. The metabolites were partially in the conjugated form. 2 The binding to serum proteins (concentration range 0.05-10 μg/ml serum) amounted to between 66% (in rats) and 85% (in man). The maximal levels of t

Clobazam for the treatment of intractable childhood epilepsy.

OBJECTIVE: Clobazam is a newer 1,5-benzodiazepine used for the treatment of epilepsy. It is better tolerated and less sedating than other benzodiazepines. It has yet to gain wide use for epilepsy in the Middle East. Our objective is to report our experience with clobazam for the treatment of childhood epilepsy. METHODS: A cohort of children with intractable epilepsy, defined as recurrent seizures after at least 3 anti-epileptic medication trials, were included prospectively. Clobazam was added to a maximum dose of 2 mg/kg/day. Follow-up by two pediatric neurologists was performed. Therapeutic

Clobazam: uncontrolled and standard controlled clinical trials.

1 In an uncontrolled clinical trial, carried out in 11 psychiatric patients with the clinical diagnoses of anxiety neurosis and depressive neurosis, clobazam, a new benzodiazepine preparation, in the dosage range 10-60 mg daily produced statistically significant improvement in the total and both factor scores of the Hamilton Anxiety Scale (HAM-A). The lowest mean total HAM-A scores occurred with a mean clobazam dosage of 48 mg daily. 2 Results of the uncontrolled clinical trial were further substantiated in a standard-controlled clinical study in which no statistically significant difference b

Photo-induced toxic epidermal necrolysis caused by clobazam.

Toxic epidermal necrolysis (TEN) is a life-threatening disease, the pathogenesis of which remains largely unknown. We describe a 23-year-old woman under treatment with clobazam who developed lesions of TEN in light-exposed areas. Patch and photopatch tests with clobazam were negative. The cellular phenotype and cytokines were studied in blister fluid. The cellular infiltrate was composed mainly of T lymphocytes with a predominant cytotoxic phenotype. There was an increase in the level of tumour necrosis factor (TNF)-alpha in blister fluid compared with the control (a patient with bullous pemph

Change in clobazam pharmacokinetics after introduction of cenobamate.

Cenobamate is a new antiseizure medication with an unusually high seizure-free rate, resulting in frequent use in patients with drug-resistant epilepsy. Many of these patients are taking clobazam (CLB) at baseline. Cenobamate has important interactions with CLB, requiring CLB dose adjustment in conjunction with cenobamate titration. There is limited guidance on the optimal CLB dose adjustment after cenobamate introduction and titration. We measured levels of CLB and its active metabolite N-desmethylclobazam (N-CLB) before and after cenobamate titration in 10 subjects with drug-resistant focal

The Amorphous State of the Antiepileptic Clobazam: Preparation and Characterization.

The aim of this study was to prepare and characterize amorphous clobazam (CLOB) and investigate its devitrification under various stressors (temperature/humidity, compaction and mechanical/thermal stresses). Amorphous CLOB was prepared by melt-quenching in liquid nitrogen. The quench-cooled sample (CLOB-q) was characterized via polarized light and hot-stage microscopies (PLM and HSM), X-ray powder diffraction (XRPD), attenuated total reflectance Fourier transform infrared spectroscopy (ATR-FTIR), conventional and modulated DSC (DSC-c and MDSC®), thermogravimetry (TG), dynamic mechanical analys

Clobazam exerts an anticonvulsant action in immature rats.

The anticonvulsant action of 1,5-benzodiazepine clobazam was studied in 12-, 18-, and 25-day-old rats. Cortical epileptic afterdischarges (ADs) elicited by rhythmic electrical stimulation of the sensorimotor cortical area were used as a model in animals with implanted electrodes. As far as the duration of ADs is concerned, clobazam in doses of 1 or 5 mg/kg i.p. blocked the progressive increase with repeated stimulations in all age groups and the higher dose significantly shortened ADs in 25-day-old rats. The intensity of movements accompanying stimulation was decreased only by the 5 mg/kg dose

Metabolic profiling of clobazam, a 1,5-benzodiazepine, in rats.

The metabolism of the 1,5-benzodiazepine clobazam (CLBZ) was investigated in the rat and in vitro by GC/MS using stable isotope techniques. Coadministration of CLBZ and pentadeuteriophenyl CLBZ to rats facilitated the identification of 4'-hydroxy CLBZ 7,4'-hydroxy N-desmethylclobazam (4'-hydroxy DMC) 5, 3',4'-dihydroxy CLBZ 13, 4'-hydroxy-3'-methoxy CLBZ 14, 3'-hydroxy-4'-methoxy CLBZ 15, and 4'-hydroxy-3'-methoxy DMC 16 in bile as both glucuronide and sulfate conjugates. Metabolites 7, 13, and 14 were present in urine as sulfate conjugates. 4'-Hydroxy CLBZ and 4'-hydroxy-3'-methoxy CLBZ were

[Usefulness of clobazam in the treatment of refractory epilepsy in children].

Below is report on our experience using a relatively new benzodiazepine as treatment for refractory epilepsy in 30 children. All of the patients had a severely fixed encephalopathy; 17 presented mixed epilepsy, 9 had Lennox-Gastaut syndrome and 4 others, West Syndrome. The seizures were totally controlled in 20% of the children; in a proportion superior to 75%, seizures were decreased in 46% of the patients. There were positive results in 6% of the cases and side-effects were seen in 13%. The usefulness of the clobazam as a co-adjuvant medication in the control of refractory epileptic seizures

Clobazam therapy of refractory epilepsy in tuberous sclerosis complex.

Clobazam (CLB) was recently approved by the FDA, but has not been evaluated in tuberous sclerosis complex (TSC). We retrospectively reviewed a cohort of patients with TSC and refractory epilepsy who started CLB over a 5-year period. Clinical characteristics and number of tubers on MRI were assessed. Duration of therapy, therapeutic response and adverse events were recorded. CLB was prescribed in 29 adults and children of whom 72% were cognitively impaired, with a median age at seizure onset of 5 months. Mean duration of CLB therapy was 17.3 months with a 12 and 24-month estimated retention rat

Carbamazepine intoxication with negative myoclonus after the addition of clobazam.

We report a case a carbamazepine (CBZ) intoxication with negative myoclonus that occurred 4 weeks after clobazam (CLB) had been added to a stable regimen of CBZ and topiramate (TPM). Both CBZ and CBZ-epoxide (CBZ-E) blood levels were elevated, and the symptoms resolved quickly when CBZ dosage was reduced and CLB discontinued. CLB was reintroduced a year later with the patient's consent, and the time course of the interaction was studied: CBZ and CBZ-E levels increased slowly over 12 days. The interaction is thus probably related to the progressive increase in Nor-CLB.

Some aspects of the effects of clobazam on human psychomotor performance.

1 Three studies are described, the first being a comparison of the effects of acute night-time doses of clobazam 20 mg, amylobarbitone sodium 100 mg, nitrazepam 5 mg and placebo, on choice reaction time, critical flicker fusion (CFF) and stabilometer performance. Clobazam improved early morning performance on a choice reaction test, in contrast to the other two active drugs. 2 Repeated doses of clobazam 10 mg three times daily, chlordiazepoxide 10 mg three times daily and diazepam 5 mg three times daily were given for 5 days. Again clobazam did not produce any impairment of psychomotor perform
