Free-base cocaine smoking.

Six healthy male, paid subjects smoked 50 mg of free-base cocaine in a specially designed glass pipe under a rigidly controlled smoking protocol. The method of heating the pipe and the temperature that produced the most efficient and consistent vaporization of the drug had been determined experimentally. The psychological and cardiovascular effects of smoking free-base cocaine were recorded. Approximately 26% of th original material was recovered from the pipe after smoking. Simulated smoking experiments in vitro indicated that only 44% of the material not trapped in the pipe was cocaine and t

Binge cocaine self-administration by humans: smoked cocaine.

Tolerance develops to the cardiovascular and subjective effects of intravenous cocaine during single self-administration sessions, but diminishes within 3 h after the session ends. To examine whether a similar pattern of tolerance occurs to smoked cocaine, seven adult 'crack' cocaine users completed a protocol investigating changes in behavior during the repeated self-administration of smoked cocaine. During sessions, participants could self-administer up to 6 doses of smoked cocaine (50 mg per dose), one every 14 min. Both two- and three-day binge conditions were tested. During the two-day bi

Achieving smoking abstinence is associated with decreased cocaine use in cocaine-dependent patients receiving smoking-cessation treatment.

BACKGROUND: Past research suggests that a significant relationship exists between cigarette smoking and illicit-stimulant abuse. The present study evaluated the association between achieving smoking abstinence in response to smoking-cessation treatment (SCT) and illicit-stimulant abstinence in cocaine- and/or methamphetamine-dependent participants. METHODS: Secondary analysis of a randomized, 10-week trial conducted at 12 substance use disorder (SUD) treatment programs. Two hundred and sixty seven adults, meeting DSM-IV-TR criteria for cocaine and/or methamphetamine-dependence and interested

Women who use cocaine too much: smoking crack vs. snorting cocaine.

Monique (an African-American 19-year-old) and Becky (a white 21-year-old) were interviewed as part of a NIDA-funded study of women and cocaine. Although they were not necessarily typical, they do illustrate some of the differences among women who use cocaine. Despite the fact that they were close in age and both used cocaine, their scenarios and outcomes were very different, as one was a white middle-class woman and the other was an underclass woman of Color. The argument presented in this article is that it is not simply substance use that frames the experiences of women who use cocaine too m

Adult supraglottitis subsequent to smoking crack cocaine.

Supraglottitis is one possible complication of smoking crack cocaine. From 1992 through 2001, our institution treated nine patients for thermal supraglottitis secondary to crack cocaine inhalation. In this article, we describe two of these cases, and we briefly review what is known about this entity and the mechanism of injury. We also provide our recommendations for management.

Esophageal foreign body after smoking crack cocaine.

Injuries from cocaine abuse are often seen and have been reported more often with the introduction of crack cocaine. Burns of the upper aerodigestive tract have been reported but no reports of esophageal obstruction have been located. We report ingestion of a foreign body from smoking crack cocaine and our management. A review of other possible aspiration and ingestion injuries is included.

Perforated ulcers related to smoking "crack" cocaine.

This article describes four patients with perforated gastroduodenal ulcers related to smoking "crack" cocaine. All patients came to the hospital with acute abdominal pain; only one patient had a history of ulcer disease. Only one patient had an elevated white blood cell count, and two patients had depressed white blood cell counts. In three patients, the x ray showed that pneumoperitoneum was present. Upon surgical exploration of the abdomen, all patients were found to have extensive peritoneal contamination. Operative repair consisted of omental patching with or without primary closure of the

Bilateral internuclear ophthalmoplegia after smoking "crack" cocaine.

A previously healthy 35-year-old man developed sudden diplopia after smoking "crack" cocaine. Neuroophthalmologic examination revealed findings consistent with a tegmental mesencephalic lesion which correlated closely with the computed tomographic documentation of a small midbrain hemorrhage. Cocaine abuse is a recognized cause of intracranial hemorrhage. In most patients, however, the bleeding is localized in the subarachnoid space or the subcortical white matter. To our knowledge, this is the first case of isolated midbrain hemorrhage secondary to cocaine abuse.

Smoked cocaine self-administration in females.

Although approximately 32% of all smoked cocaine ("crack") users are women, most studies investigating cocaine use have recruited only male subjects. Therefore, this study examined the smoked cocaine self-administration patterns of female crack abusers. A work requirement, where up to five tokens were earned by subjects, was followed by the administration of a sample delivery of one of three dose sizes [5.0 mg (placebo), 0.2 mg/kg, 0.4 mg/kg] of cocaine. The three dose sizes of cocaine were administered in counterbalanced order across subjects, with each subject receiving one dose size within

Relationship of cessation of cocaine use to cigarette smoking in cocaine-dependent outpatients.

Cocaine abusers (N=43) from an outpatient drug rehabilitation program were interviewed regarding the relationship of cessation of cocaine use to cigarette smoking. Patients reported significant reductions in cigarette consumption after cessation of cocaine use (37.3+/-26.4 cigarettes per day when using cocaine vs. 18.1+/-7.0 cigarettes per day after cessation of cocaine use; P < .0001, paired t test, t=4.79 with 42 degrees of freedom). Reduction of cigarette smoking after cessation of cocaine use was reported primarily by heavy smokers. However, patients with longer clinic enrollments reported

Butyrylcholinesterase levels and subjective effects of smoked cocaine in healthy cocaine users.

BACKGROUND: Butyrylcholinesterase (BChE) is beginning to attract attention as a possible target for cocaine abuse treatment because of its role in metabolizing cocaine. OBJECTIVE: The purpose of this analysis was to assess whether endogenous BChE levels are associated with the subjective effects of cocaine. METHODS: Data from 28 participants in five inpatient cocaine self-administration studies were included in the present analysis. Four minutes after each smoked cocaine dose, participants rated their drug-related effects from 0-100 using a computerized self-report Visual Analogue Scale (VAS

Cocaine-specific antibodies blunt the subjective effects of smoked cocaine in humans.

BACKGROUND: Rates of relapse among cocaine-dependent patients are high, and new treatment approaches are needed. Clinical data demonstrate that a cocaine vaccine (TA-CD) produces selective anticocaine antibodies, yet the impact of these antibodies on cocaine's direct effects is unknown. The objective of this human laboratory study was to measure the relationship between antibody titers and the effects of smoked cocaine on ratings of intoxication, craving, and cardiovascular effects. METHODS: Ten cocaine-dependent men not seeking drug treatment spent 2 nights per week for 13 weeks inpatient wh

Self-administration of cocaine by humans: choice between smoked and intravenous cocaine.

Ten healthy adult male research volunteers reporting smoked and i.v. cocaine use resided on a Clinical Research Center for 2 weeks and participated in nine daily sessions. A session consisted of seven choice trials. The first two trials were sampling trials in which subjects received one dose each of i.v. cocaine hydrochloride (0, 16 and 32 mg) and smoked cocaine base (0, 25 and 50 mg). Each of the remaining five trials was a choice trial in which subjects could choose to self-administer either of the doses from the initial two trials. Subjects 1) reliably chose active doses of cocaine compare

Subjective and physiological effects of intravenous nicotine and cocaine in cigarette smoking cocaine abusers.

The subjective and physiological effects of intravenously administered cocaine and nicotine were compared in 10 cigarette-smoking cocaine abusers. Subjects abstained from smoking at least 8 h before each session. Under double blind conditions, subjects received placebo, cocaine (10, 20, and 40 mg/70 mg), or nicotine (0.75, 1.5, 3.0 mg/70 kg) in mixed order. Physiological and subjective data were collected before and repeatedly after each intravenous drug administration. Cardiovascular and subjective effects of repeated smoked cocaine administration in experienced cocaine users.

Studies using rodents have shown that behavioral responses to a stimulant are enhanced when the stimulant is given within the same context as previous stimulant administrations; this increase in effect related to context is often referred to as sensitization. We examined the role of environmental stimuli in modulating the subjective and cardiovascular effects of cocaine in humans (1) within a daily "binge" and (2) after cocaine abstinence. Ten non-treatment seeking users of smoked cocaine were admitted to the hospital for 17 consecutive days. Participants smoked cocaine (25mg/dose) under two c

Effects of carbamazepine on acute responses to smoked cocaine-base in human cocaine users.

A double-blind, placebo-controlled cross-over study was conducted to determine the effects of carbamazepine on the acute physiological and subjective responses to a single dose of smoked cocaine-base. Male cocaine users (N = 6) were given 400 mg carbamazepine or placebo, each for a period of 5 days. At the end of the 5-day period, a 40 mg dose of smoked cocaine was administered. The results showed a significantly higher heart rate, diastolic blood pressure elevation, and blood pressure-heart rate product under the carbamazepine compared to the placebo condition. There were no effects of carbam

Predictors of cardiovascular response to smoked cocaine in humans.

The purpose of this study was to examine the predictors of heart rate and blood pressure changes following cocaine administration. Sixty-two smoked cocaine users received a single 0.4 mg/kg dose of smoked cocaine. Male sex, African American race, higher body weight and current marijuana use predicted a greater cardiovascular response to cocaine. In contrast, higher baseline blood pressure, heart rate, amount and frequency of current cocaine use and presence of current cocaine snorting predicted a diminished cardiovascular response to cocaine. Whether these predictors of the cardiovascular resp

Smoked cocaine self-administration is decreased by modafinil.

Modafinil has been reported to reduce cocaine use in a clinical sample of infrequent users (2 days/week), but the effects of modafinil on cocaine self-administration in the laboratory have not been studied. The present study investigated the effects of modafinil maintenance on cocaine self-administration by frequent users (4 days/week) under controlled laboratory conditions. During this 48-day double-blind, crossover design study, the effects of modafinil maintenance (0, 200, and 400 mg/day) on response to smoked cocaine (0, 12, 25, and 50 mg) were examined in nontreatment-seeking cocaine-depe
