Cochlear Implantation.

BACKGROUND: Hearing impairment that is too severe to be adequately treated with conventional hearing aids can lead, in children, to severe developmental disturbances of hearing and language, and, in adults, to communicative and social deprivation. Recent advances in medical device technology and in microsurgical techniques have led to an expansion of the indications for cochlear implantation (CI) for adults with progressive hearing loss in older age, and to a restructuring of the process of care for these patients in Germany. METHODS: This review is based on pertinent publications retrieved b

[Cochlear implantation].

Since the Israeli cochlear implant program was started in June 1989, 74 candidates have been evaluated for implantation. The main causes for hearing loss were meningitis (16 patients), aminoglycoside toxicity (9), congenital (6), viral (e.g. mumps) (6), and middle ear infection. Candidates were rejected mainly due to psychosocial factors (40%) and audiological parameters (30%). 16 postlingual deaf were implanted. In 7 of them (43.7%) the results were excellent, in 4 (25.0%) good, in 3 (18.7%) fair; 1 case is currently in a training program and 1 is a failure. There were 2 major complications:

Cochlear implantation in cochlear otosclerosis.

OBJECTIVES/HYPOTHESIS: The objective was to correlate implant performance in cochlear otosclerosis to 1) matched control samples, 2) severity of otic capsule involvement, 3) prior ipsilateral surgery, and 4) programming issues. STUDY DESIGN: Retrospective case controlled study. METHODS: Study cohort comprised 30 individuals. Diagnosis was based on prior ear surgery (stapedectomy [n = 18] or fenestration [n = 2]) and/or pathognomonic radiological findings. High-resolution computed tomography images of the temporal bones were assessed by two radiologists and graded (range, 0-3) for the extent

Revision cochlear implantation.

Reoperation on a patient with an indwelling cochlear implant is uncommon. When necessary, surgery is performed for explantation of an existing device with immediate or delayed reimplantation, or for scalp flap revision and receiver-stimulator repositioning in the case of infection or device migration. Rarely, revision surgery is performed to reintroduce intracochlear electrodes that may have partly or entirely extruded from the cochlea or were placed inappropriately. Successful revision cochlear implant surgery requires attention to certain surgical principles. Good outcomes, asa measured by s

Cochlear implantation in children with cochlear aplasia.

CONCLUSION: Children with cochlear aplasia can achieve favorable speech perception abilities from cochlear implantation (CI) by inserting an electrode array into the vestibule if an audiological response is reliably demonstrated before surgery by behavioral audiometry and objective measures. OBJECTIVES: The purpose of this study was to evaluate the surgical outcome of CI in children with cochlear aplasia. METHODS: The medical records of 454 children who underwent CI at the Dong-A University Hospital between January 1994 and September 2011 were reviewed. The prevalence of cochlear aplasia and

[Cochlear implantation for adults].

A cochlear implant is an electrode, which is implanted in the inner ear to establish or re-establish hearing. Based on estimates on the prevalence of undertreated bilateral profound hearing loss, the number of adult people receiving a cochlear implant is low. In addition, many people with residual acoustic hearing or people with single-sided deafness could benefit from a cochlear implant. Due to the recent expansions in the indications for cochlear implantation and the unmet need for hearing improvement among the adult population, more focus on referring these patients to audiological cochlear

Endoscope-Assisted Cochlear Implantation.

OBJECTIVES: Our aim was to present our endoscope-assisted cochlear implantation (CI) technique, in which the middle ear landmarks were identified through the facial recess exposure by using an endoscopic view without elevating the tympanic annulus. The secondary goal was to assess whether the situation of difficult surgical exposure could be predicted by evaluating preoperative axial computed tomography (CT) examinations. METHODS: CT examinations and surgical outcomes of endoscope-assisted CI surgeries were analyzed. RESULTS: A total of 179 CI operations performed in 27 adults (15.1%) and 15

Cochlear Implantation in Neurobrucellosis.

BACKGROUND: Neurobrucellosis is a disease consisting of a wide spectrum of complications such as peripheral neuropathy, cranial nerve involvement, ataxia, meningeal irritation, paraplegia, seizures, coma, and even death. The vestibulocochlear nerve seems to be the most commonly affected cranial nerve (10%). We present a patient with neurobrucellosis whose auditory perception and speech intelligibility skill performances improved after cochlear implantation. CASE REPORT: A 35 year-old woman was admitted to another hospital 2 years ago with the symptoms of headache, nausea, and altered consciou

[Cochlear implantation in children].

Children with severe hearing impairment or deafness are preferably recognised and treated with a cochlear implant (CI) before the age of one year, as early stimulation of the auditory sense is essential for the development of spoken language. Today, children with deafness are offered a bilateral CI, as it allows for improved speech perception in noise and sound localisation. However, the indications for cochlear implantation have been extended to include children with asymmetric hearing loss, in order to avoid the development of aural preference syndrome, which may limit the effect of a CI lat

Contralateral recovery of cochlear function after cochlear implantation.

Significant recovery of sensorineural hearing loss in either ear after cochlear implantation (CI) is rare. We present the case of a 57-year-old lady with medically treated depression and a background of chronic suppurative otitis media (CSOM) in the non-implanted ear, who clearly fulfilled audiological criteria for CI. Two years post-implantation her CSOM in the non-implanted ear was addressed with blind sac closure of the ear. Post-operatively the hearing thresholds in this ear had improved to the extent that a bone-anchored hearing aid became a viable option. The literature is carefully revi
