The Korea Cohort Consortium: The Future of Pooling Cohort Studies.

OBJECTIVES: We introduced the cohort studies included in the Korea Cohort Consortium (KCC), focusing on large-scale cohort studies established in Korea with a prolonged follow-up period. Moreover, we also provided projections of the follow-up and estimates of the sample size that would be necessary for big-data analyses based on pooling established cohort studies, including population-based genomic studies. METHODS: We mainly focused on the characteristics of individual cohort studies from the KCC. We developed "PROFAN", a Shiny application for projecting the follow-up period to achieve a cer

What are we learning from genetic cohort studies?

Although genetic component of asthma has been well recognised, genetics alone cannot explain the rise in asthma prevalence. This increase is likely a consequence of environmental factors increasing the risk in genetically susceptible individuals. As wheezing illness usually begins within months of birth, prospective birth cohorts with detailed measures of environmental exposures and objective measures of disease are essential to study gene-environment interactions in the development of different wheeze phenotypes. Such studies will enable identification of children at increased risk of disease

Observational epidemiologic studies of endemic waterborne risks: cohort, case-control, time-series, and ecologic studies.

Observational studies have assessed endemic waterborne risks in a number of countries. Time-series analyses associated increased water turbidity with increased gastroenteritis risks in several public water systems. Several cohort studies reported an increased risk of gastroenteritis in populations using certain public or individual water systems. Although several case-control studies found increased waterborne risks, they also found increased risks associated with other exposures. An increased risk of campylobacteriosis was associated with drinking untreated water from non-urban areas and some

Estimating direct effects in cohort and case-control studies.

Estimating the effect of an exposure on an outcome, other than through some given mediator, requires adjustment for all risk factors of the mediator that are also associated with the outcome. When these risk factors are themselves affected by the exposure, then standard regression methods do not apply. In this article, I review methods for accommodating this and discuss their limitations for estimating the controlled direct effect (ie, the exposure effect when controlling the mediator at a specified level uniformly in the population). In addition, I propose a powerful and easy-to-apply alterna

Estimating the hazard rate difference from case-cohort studies.

The case-cohort design, among many two-phase sampling designs, substantially reduces the cost of an epidemiological study by selecting more informative participants within the full cohort for expensive variable measurements. Despite their benefits, additive hazards models, which estimate hazard differences, have rarely been used for the analysis of case-cohort studies due to the lack of software and application examples. In this paper, we describe a newly developed estimation method that fits the additive hazards models to general two-phase sampling studies along with the R package addhazard t

Lifetime Risk of Venous Thromboembolism in Two Cohort Studies.

BACKGROUND: Greater public awareness of venous thromboembolism may be an important next step for optimizing venous thromboembolism prevention and treatment. "Lifetime risk" is an easily interpretable way of presenting risk information. Therefore, we sought to calculate the lifetime risk of venous thromboembolism (deep vein thrombosis or pulmonary embolism) using data from 2 large, prospective cohort studies: the Cardiovascular Health Study (CHS) and the Atherosclerosis Risk in Communities (ARIC) study. METHODS: We followed participants aged 45-64 years in ARIC (n = 14,185) and ≥65 in CHS (n =

Prospective cohort studies of shigellosis during military field training.

Epidemiological and clinical features of shigellosis occurring among cohorts of Israeli recruits followed-up for 3-6 months during the summer field training of years 1993-1997 were studied. The incidence rate of culture-proven shigellosis was the highest (78 cases per 1,000 recruits) in 1996 and the lowest (13 cases per 1,000 recruits) in 1995. Shigella sonnei (152 isolates) and Shigella flexneri (151 isolates) were the most common species. Fifty percent of the patients with shigellosis had fever (>37.5 degrees C), compared to only 18% of the subjects with other diarrheal diseases (P < 0.001).

Immortal Time Bias in Cohort Studies: A Concept Simply Explained.

Immortal time bias is one of many types of bias that may flaw cohort studies. This bias occurs when formal observation of patients starts before they experience the exposure of interest, and when, by design, those who experience the outcome of interest before they experience the exposure are ineligible to be included in the exposure arm of the study. Consequently, patients in the exposure arm, by design, are immortal to the outcome between the start of observation and point of the exposure. If this period of immortality is included in analysis, the result is immortal time bias. This concept is

Physical activity and constipation: A systematic review of cohort studies.

BACKGROUND: Constipation significantly impacts quality of life and is a common public health issue. For affected individuals, especially those who are inactive and experience constipation symptoms, it is recommended to engage in physical activity (PA) to improve their condition. However, the relationship between PA and improvement in constipation remains unclear. We performed this systematic review of cohort studies to evaluate this potential association. METHODS: We systematically searched the Embase, Cochrane Library, PubMed, and CINAHL databases for all cohort studies examining the relatio

Approaches to Optimize Medication Data Analysis in Clinical Cohort Studies.

OBJECTIVES: Methods for pharmacoepidemiologic studies of large-scale data repositories are established. Although clinical cohorts of older adults often contain critical information to advance our understanding of medication risk and benefit, the methods best suited to manage medication data in these samples are sometimes unclear and their degree of validation unknown. We sought to provide researchers, in the context of a clinical cohort study of delirium in older adults, with guidance on the methodological tools to use data from clinical cohorts to better understand medication risk factors and

The importance of cohort studies in the post-GWAS era.

The past decade has seen enormous success of wide-scale genetic studies in identifying genetic variants that modify individuals' predisposition to common diseases. However, the interpretation and functional understanding of these variants lag far behind. In this Perspective, we discuss opportunities for using large-scale cohort studies to investigate the downstream molecular effects of SNPs at different 'omics' data levels. We point to the pivotal role of population cohorts in establishing causality and advancing drug discovery. In particular, we focus on the breadth-versus-depth concepts of p

Lessons learned from birth cohort studies conducted in diverse environments.

Childhood asthma develops from a complex interaction among host and environmental factors in early life. Birth cohort studies have provided valuable insight into asthma risk factors and the natural history of wheezing and asthma through childhood and beyond. Early life aeroallergen sensitization and wheezing illnesses associated with virus and bacterial infections have been identified as pivotal risk factors for asthma inception. Recently, focus has turned toward protective factors that promote lung health in children. Studies in a variety of environments, including farms and urban communities

Classifying Chronic Lower Respiratory Disease Events in Epidemiologic Cohort Studies.

RATIONALE: One in 12 adults has chronic obstructive pulmonary disease or asthma. Acute exacerbations of these chronic lower respiratory diseases (CLRDs) are a major cause of morbidity and mortality. Valid approaches to classifying cases and exacerbations in the general population are needed to facilitate prevention research. OBJECTIVES: To assess the feasibility, reproducibility, and performance of a protocol to identify CLRD cases and exacerbations triggering emergency department (ED) visits or hospitalizations in cohorts of patients derived from general populations of adults.
