Open colectomy versus laparoscopic colectomy: are there differences?

Laparoscopic colectomy has been increasingly reported as an option for the treatment of colonic pathology. However, there is very little information regarding perioperative morbidity and the cost effectiveness of this technique. The purpose of this study is to review our first year of experience with laparoscopic colon resection. Data collected includes: age, technique (open laparotomy, laparoscopic, laparoscopic/converted open), Karnofsky score, complications, specimen size/nodes, OR time, hospital stay, and cost. This is a consecutive series of 140 elective colonic resections including 102 o

Laparoscopic colectomy vs. open colectomy for sigmoid diverticular disease.

PURPOSE: The feasibility of laparoscopic colectomy for colon surgery has now been well established. Most of the studies on laparoscopic colectomies include all types of colonic pathologies without discrimination. Our goal was to compare laparoscopic sigmoid colectomy open sigmoid colectomy for simple sigmoid diverticular disease, to assess whether it can be done safely and whether the proposed advantages could be realized. METHODS: We evaluated the differences in outcomes of 66 laparoscopic sigmoid colectomy patients and 88 open sigmoid colectomy patients. We report a five-year outcomes analy

[Surgical treatment for slow transit constipation: total colectomy and subtotal colectomy].

Although the surgical treatment of slow transit constipation (STC) has been around for over a century, the choice of surgical method is still controversial. Specifically, the two most widely used operations in the world are total colectomy ileorectostomy (TC-IRA) and subtotal colectomy. Subtotal colectomy offers many methods for anastomosis, including ileocecal and cecorectal anastomoses and so on. Cecorectal anastomosis can be divided into two types: isoperistaltic anastomosis and antiperistaltic anastomosis. It is generally believed that total colectomy provides definitive effects but result

Operative Method for Transverse Colon Carcinoma: Transverse Colectomy Versus Extended Colectomy.

BACKGROUND: The type of surgery performed for primary transverse colon cancer varies based on tumor characteristics and surgeon perspective. The optimal oncological outcome following different surgical options has not been clearly established, and transverse colectomy has shown oncological equivalence only in small cohort studies. OBJECTIVE: Our aim was to compare long-term oncological outcomes after transverse colectomy versus extended resection for transverse colon cancer. DESIGN: This study is a retrospective review of prospectively collected data. SETTING: This study was conducted at a

Extended right colectomy, left colectomy, or segmental left colectomy for splenic flexure carcinomas: a European multicenter propensity score matching analysis.

BACKGROUND: The surgical resection of the splenic flexure carcinoma (SFC) is challenging and the optimal surgical procedure for SFCs remains a matter of debate. The present study aimed to compare in a multicenter European sample of patients the short- and long-term outcomes of extended right (ERC) vs. left (LC) vs. segmental left colectomy (SLC) for SFCs. METHODS: This retrospective multicenter study analyzed the surgical and oncological outcomes of SFC patients undergoing elective curative intent surgery between 2000 and 2018. Descriptive and exploratory analyses were first conducted on the

Prospective comparison of gastric emptying after laparoscopic-aided colectomy versus open colectomy.

Laparoscopic colectomy has been associated with a shorter postoperative ileus when compared to open colectomy, although the mechanism is unclear. This study is designed to evaluate gastric emptying following open colectomies (OC) versus laparoscopic-aided colectomies (LAC) using serial serum acetaminophen levels (ACE), which correlate with gastric emptying. The study groups were limited to patients undergoing either right or left colectomy who received general anesthetic. Patients with diabetes mellitus or other colon resections were excluded. Postoperative analgesia was provided with intramus

Laparoscopic colectomy compares favorably with colectomy by laparotomy for reduction of postoperative ileus.

PURPOSE: The aim of this study was to compare the length of postoperative ileus in patients undergoing colectomy by either laparotomy or laparoscopy. METHODS: A total of 166 patients were studied. These patients were divided into two groups: Group 1, in which colectomy was done laparoscopically, and Group 2, consisting of patients undergoing laparotomy. Both groups contained 83 patients who were matched for disease severity, indications for surgery, and procedure. Indications for surgery included sigmoid diverticulitis in 12 (14 percent) patients, polyps in 22 (27 percent), Crohn's disease in

Colectomy in patients with previous colectomy or occlusive vascular diseases: Pitfalls and precautions.

Two principal branches from the aorta provide the colonic blood supply: the superior and inferior mesenteric arteries. There are numerous anatomical variations, which the surgeon must fully understand before embarking on any colonic surgery. A good knowledge of these variations is particularly important when the patient has already undergone colectomy or presents with occlusive vascular disease. The aim of this review is to summarize the standard anatomy and the main variations of the colonic blood supply as they apply to colorectal surgery in this setting.

Colectomy for sigmoid volvulus.

PURPOSE: Volvulus of the sigmoid carries a high mortality. This minimally invasive technique is designed for poor-risk patients. TECHNIQUE: Aided by laparoscopy, the mobile sigmoid is exteriorized through a mini-incision without preliminary dissection. The entire resection and anastomosis is performed on the surface of the abdomen. The options of end colostomy, primary anastomosis with or without fecal diversion, can all be used with this method. RESULTS: No mortality or leakage occurred in four patients. CONCLUSION: This rapid operation does not require advanced laparoscopic skills. Prelim

The functional consequences of colectomy.

The colon plays a decisive role in salt and water conservation in the intact human, normally removing from the terminal intestine approximately one liter of isotonic fluid that escapes small bowel absorption. The primary purpose of this colon function is probably to prevent extracellular fluid volume depletion and only incidentally to produce a normal solid stool. The patient with an ileostomy can partly adapt to replace the absorptive capacity lost after colectomy but is still vulnerable if salt and water intake ceases completely. In contrast, patients with ileorectostomies and some patients

Totally robotic left colectomy.

Laparoscopic colectomy has been a challenge for surgeons due to steep learning curves, limited dexterity of instruments and non-depth perception with visualization. Robotics for left colectomy has been described in the past years using mostly single docking or hybrid techniques. With the advantages of the robotic technology including the use of 3D visualization, increase dexterity with 360-degree motion of the instruments, surgeons may see an increase in the adoption of minimally invasive surgery for colectomies. Surgeons know that multi-quadrant dissection can be difficult at best with the cu

[Colectomy in emergency surgery].

Unlike the standardised surgery of the right sided colic emergencies there is still a matter of debate on the emergency approach of the left colon and rectum. Between 1998 - 2007 on 32 patients (15 males, 17 females) we performed the single stage radical procedure total or subtotal colectomy. In the same period we performed 372 emergency operations for low intestinal occlusion. The patients had ages between 24 - 86 years, the admittance diagnosis was intestinal occlusion. The postoperative diagnosis was left colic carcinoma (n=23), strangulated hernia (n=2) strangulated incisional hernia (n=2)

Colonoscopic-assisted laparoscopic colectomy.

One of the technical difficulties during laparoscopic and laparoscopic-assisted resection of the right, transverse, and left colon is the mobilization of the splenic and hepatic flexures. We present a simple technique of colonoscopic traction of the splenic or hepatic flexure. This technique enables good exposure and facilitates dissection while laparoscopic mobilization of these segments of the colon is performed.

Hand-assisted laparoscopic colectomy compared with open colectomy in a nontertiary care setting.

BACKGROUND: Laparoscopic colectomy allows oncologic resection equivalent to open colectomy while reducing postoperative morbidity, at the cost of longer operating time. Hand-assisted laparoscopy might yield the benefits of laparoscopy while reducing operating time. PATIENTS AND METHODS: We compared the intraoperative and postoperative experience of patients undergoing hand-assisted laparoscopic colectomy (HALC) to those who had open colectomy. In this retrospective case review of consecutive patients undergoing HALC for colon tumors from April 2003 to September 2004 compared with patients who

Novel procedure, SILSOID colectomy, is a bridge between conventional and single-incisional laparoscopic colectomy.

INTRODUCTION: Laparoscopic colectomy (LC) is a widely accepted treatment for various diseases of the colon. Transumbilical single-incisional laparoscopic surgery (SILS) offers excellent cosmetic results compared with standard multi-port laparoscopic surgery. We describe a new hybrid laparoscopic procedure, SILSOID colectomy, which combines conventional LC with SILS. METHODS: We performed SILSOID colectomy to treat four patients with colorectal disease. Three ports were inserted through the single transumbilical incision, and an additional port was inserted in the flank at a site that depended
