[Collagenous sprue].

HISTORY AND FINDINGS: A 45-year-old man with type I diabetes mellitus was admitted to hospital because of colicky abdominal pain and 5-6 watery stools daily. Upper gastrointestinal endoscopy showed nearly total atrophy of the villi in the duodenum and jejunum suggesting coeliac disease. However, gluten-free diet for 2 weeks brought no improvement. Another examination of the biopsy 6 weeks after the first endoscopy revealed extensive collagen deposition in the lamina propria of the small intestine, giving the diagnosis of collagenous sprue. TREATMENT AND COURSE: Parenteral nutrition, lactulose

[Collagenous sprue (author's transl)].

Malabsorption is the main functional abnormality in gluten-sensitive enteropathy or endemic sprue. The malabsorption is related to villous flattening, which is the main histological feature. The diagnosis is also based on a good response to gluten-free diet. On the other hand, collagenous sprue is not an independent disease but a variant of gluten-sensitive enteropathy, as documented in three personal cases. Its course is characterised by a malabsorption syndrome unresponsive to gluten withdrawal, with massive subepithelial collagen formation. The pathogenesis remains unknown, prognosis poor,

Atypical Presentations of Collagenous Gastritis Mimicking Celiac Sprue.

Collagenous gastritis has been reported as a rare cause of nausea, diarrhea, weight changes, and early satiety in female patients. Here, we describe two women aged 43 and 71 years who presented with similar symptoms. Gastric biopsies from both individuals showed thickened, irregular subepithelial collagen bands (>10 μm). The pathogenesis of collagenous gastritis is poorly understood, but it may be the presenting symptom for many underlying autoimmune conditions. In particular, there is a well-established connection between collagenous disorders of the gastrointestinal tract and celiac sprue, S

Concomitant presentation of collagenous sprue and HFE hemochromatosis.

Collagenous sprue (CS) is a progressive malabsorptive disorder characterized by collagen deposition beneath the basement membrane of small bowel epithelium in refractory celiac sprue. CS is a pathologically distinct entity from celiac disease, despite a similar clinical presentation. The etiology of CS is unclear, although there are speculations that CS and celiac disease may share similar pathogenetic pathways. On the other hand, HFE hemochromatosis (HH) is a distinct disease entity. Celiac disease and HH are common HLA-associated genetic disorders in Northern European populations. There are

Collagenous sprue: a clinicopathologic study of 12 cases.

Collagenous sprue is a rare form of small bowel enteropathy characterized by chronic diarrhea and progressive malabsorption with little data available on its natural history. The pathologic lesion consists of subepithelial collagen deposition associated with variable alterations in villous architecture. The small bowel biopsies of 12 cases were reviewed. Clinical details, celiac serology, and T-cell receptor gene rearrangement study results, when available, were collated. There were 8 females and 4 males (age ranged from 41 to 84 y) who presented with chronic diarrhea and weight loss. Small in

Collagenous sprue cross-sectional imaging: a comparative blinded study.

PURPOSE: Collagenous sprue (CS) is a rare enteropathy characterized by villous atrophy and a thickened subepithelial collagen band. The aim of this study is to describe the cross-sectional imaging findings of CS. METHODS: A case-control, retrospective study with cases of all CS patients from January 2000 to 2015 was performed. Inclusion criteria were (1) Histopathologic diagnosis and (2) Imaging with computed tomography abdomen/pelvis (CT A/P), CT enterography (CTE), or magnetic resonance enterography within 6 months of small bowel (SB) biopsy. Control subjects were irritable bowel syndrome (

Increased immunoglobulin G4-positive plasma cells in collagenous sprue.

Collagenous sprue is a rare enteropathy whose etiology is unknown, but immune-mediated mechanisms are one of several possibilities. However, the role of immunoglobulin G4 (IgG4)-positive plasma cells has not been studied in collagenous sprue. Endoscopic biopsies from the duodenum with a histologic diagnosis of collagenous sprue (n = 40 from 35 patients), celiac disease (n = 25), peptic duodenitis (n = 15) and normal duodenum (n = 25) were immunohistochemically stained with IgG4 and CD138 antibodies. For each case, the quantities of IgG4- and CD138-positive plasma cells in the lamina propria we

Hyposplenism, antiendomysial antibodies and lymphocytic colitis in collagenous sprue.

A 66-year-old woman was seen repeatedly over a decade to remove recurrent colonic adenomas and investigate episodes of watery diarrhea. Although the diarrhea was believed to be due to lymphocytic colitis, she developed weight loss, hypoproteinemia and hyposplenism that resulted in further studies, specifically to exclude celiac disease. Small intestinal biopsies, however, showed severely 'flattened' villous architecture with trichrome-positive subepithelial collagenous deposits, characteristic of collagenous sprue. Antiendomysial antibodies, known serological markers of celiac disease, were al

Comparison of clinical features, treatment, and outcomes of collagenous sprue, celiac disease, and collagenous colitis.

BACKGROUND: Collagenous sprue (CS) is a rare form of enteropathy that had been reported to be associated with celiac disease (CD) and collagenous colitis (CC). The aim of our study was to compare the clinical features, treatments, and outcomes of CS, CD, and CC. METHODS: All patients with histologic diagnosis of CS, CD, or CC with complete clinical data were extracted from our pathology database between 1990 and 2015. Demographic and clinical features were recorded along with treatments and outcomes. RESULTS: A total of 21 patients with CS were included. Overall CS patients were more symptom

Collagenous sprue: a rare, severe small-bowel malabsorptive disorder.

Collagenous sprue is a severe malabsorptive disorder, histologically characterized by small intestinal villous and crypt atrophy, and a subepithelial collagen deposit, thicker than 12 µm, that entraps lamina propria cellular elements. Collagenous sprue is a rare disease entity, with only about 60 sporadic cases reported worldwide since it was first described in 1947. Its exact etiology is still under investigation, and its relationship with classic celiac disease and other refractory, spruelike intestinal disorders remains controversial. Two larger-scale studies, in 2009, brought new insights

[Collagenous Sprue, Collagenous Gastritis, and an Uncommon Association with Inflammatory Bowel Disease: A Case Report].

UNLABELLED: Celiac disease is the most common cause of intestinal villous atrophy. It may present with a clinical course characterized by chronic diarrhea, malabsorption, and weight loss. Diagnosis is based on the presence of positive specific antibodies in serum, characteristic enteropathy, and the clinical and histological response to a gluten-free diet. However, in some cases, patients with villous atrophy who do not respond to the exclusion of gluten from the diet present a diagnostic and therapeutic challenge. It is essential to perform a differential diagnosis, as there are different pat

[Collagenous sprue--a rare form of celiac disease in adulthood].

The authors report a case of collagenous sprue. This condition is characterized by coeliac type small bowel malabsorption, resistant to gluten free diet and other therapeutic efforts, associated with poor prognosis. The diagnosis depends on the histological demonstration of extensive collagenization of the lamina propria in the flat jejunal mucosa. This disease must be kept in mind at differential diagnosis of chronic diarrhea with progressive malabsorption, especially if it is resistant to gluten withdrawal in contrast to conventional coeliac disease.

Collagenous sprue across five decades (1970-2025): a systematic review.

BACKGROUND: Collagenous sprue (CS) is a rare sprue-like enteropathy with villous atrophy and a subepithelial collagen band. Published evidence is fragmented. METHODS: We conducted a systematic scoping review with a pooled, descriptive patient-level analysis to summarize presentation, evaluation, management and outcomes in reported adult CS. We searched PubMed/MEDLINE, Embase, Web of Science, CINAHL and Scopus from inception through 1 December 2025. We included adult cases with biopsy-confirmed CS, defined as small-bowel villous atrophy with a subepithelial collagen band. RESULTS: Of 777 reco

The first cases of collagenous sprue successfully treated with thioguanine.

OBJECTIVE: Collagenous sprue (CS) is a rare form of small bowel enteropathy characterised by a thickened basement membrane and is, in most of the literature, reported as part of coeliac disease. Multiple treatment strategies are suggested in CS, but there is no standardised therapy. The aim of this series is to describe 4 cases of CS and to propose thioguanine (6-TG) treatment. DESIGN: We reviewed 4 cases of CS. Data were obtained from our prospective database of patients referred to our coeliac centre. Evaluation of small bowel biopsies was performed by an expert pathologist. RESULTS: None

Collagenous sprue: a coeliac disease look-alike with different treatment strategy.

Collagenous sprue is a rare clinicopathological condition of the small bowel. It is characterised by abnormal subepithelial collagen deposition and is typically associated with malabsorption, diarrhoea and weight loss. The clinical features of collagenous sprue often resemble those of coeliac disease and together with frequent histological findings like mucosal thinning and intraepithelial lymphocytosis the diagnosis may be hard to reach without awareness of this condition. While coeliac disease is treated using gluten restriction, collagenous sprue is, however, not improved by this interventi

Collagenous sprue: a rare cause of watery diarrhea and villous atrophy - case report.

Collagenous sprue is a rare and unrecognized cause of diarrhea and weight loss, mainly affecting the duodenum and small bowel. The clinical picture often resembles that of coeliac sprue, the main differential diagnosis, albeit, being refractory to a gluten-free diet. The histological features are fundamentally characterized by the deposition of collagen beneath the basement membrane of gut mucosa. Treatment should be initiated as soon as the diagnosis is established, so as to prevent the progression of fibrosis. We will describe the case of a 76-year-old woman with collagenous sprue, her diagn

Angiotensin-II inhibitor (olmesartan)-induced collagenous sprue with resolution following discontinuation of drug.

Collagenous sprue (CS) is a pattern of small-bowel injury characterized histologically by marked villous blunting, intraepithelial lymphocytes, and thickened sub-epithelial collagen table. Clinically, patients present with diarrhea, abdominal pain, malabsorption, and weight loss. Gluten intolerance is the most common cause of villous blunting in the duodenum; however, in a recent case series by the Mayo Clinic, it has been reported that olmesartan can have a similar effect. In this case report, a 62-year-old female with a history of hypothyroidism and hypertension managed for several years wit

Successful treatment of collagenous sprue with combination of prednisolone and gluten-free diet.

A case is described of adult coeliac disease with subepithelial collagen in a jejunal biopsy (‘collagenous sprue’) which responded well to treatment with prednisolone in addition to a gluten-free diet. The indications for corticosteroid treatment in adult coeliac disease are discussed in the light of the findings. It is suggested that collagenous sprue may represent the most severe form of adult coeliac disease, and the presence of sub-epithelial collagen in a jejunal biopsy may be the strongest indication for treatment with the combination of a gluten-free diet and corticosteroids.
