Thumb metacarpophalangeal ulnar and radial collateral ligament injuries occur frequently in the competitive athlete. Collateral ligament integrity is essential to joint stability, pinch strength, and pain-free motion. Acute rupture of the ulnar collateral ligament is due to a sudden radial deviation force on the abducted thumb and is referred to as skier's thumb. An ulnar-directed force causes injury to the radial collateral ligament. The degree of joint instability on clinical examination allows classification of these injuries and guides management. Surgical repair of acute, complete tears r

Recent advances in the diagnosis and treatment of the overhead athlete's elbow has led the medical community to understand that the ulnar collateral ligament (UCL) of the elbow is more commonly injured than originally thought. Injury can result in secondary symptoms and problems in other regions of the elbow. Sports requiring an overhead motion, such as throwing a ball, hitting a ball overhead, or serving a tennis ball, imparts a valgus stress on the elbow that is resisted by the UCL. Throwing sidearm or hitting a forehand in tennis, squash, or racquetball may also impart a valgus stress to th

BACKGROUND: The ulnar collateral ligament (UCL) plays a vital role in maintaining the stability of the elbow joint during overhead throwing activities. Stress to the UCL may lead to high-grade or full-thickness tears that require reconstruction or repair and lengthy rehabilitation in athletes. Workup of these injuries can sometimes reveal ossification in the ligament. It is unknown if ossification in the UCL affects postoperative outcomes.
PURPOSE/HYPOTHESIS: The purpose of this study was to investigate whether different preoperative ossification conditions of the UCL might influence various

The medial ulnar collateral ligament (MUCL) complex is integral for valgus elbow stability, especially in individuals engaged in repetitive overhead activities such as throwing. MUCL injuries often necessitate surgical intervention to restore elbow stability. Early studies reporting outcomes after MUCL repair demonstrated suboptimal return to play compared with ulnar collateral ligament reconstruction, prompting a shift toward reconstruction techniques. The first widely popular MUCL reconstruction technique was the Jobe technique. Many reconstruction techniques have since been described. Despi

BACKGROUND: Ulnar collateral ligament (UCL) repair with single-strand suture augmentation has been introduced as a viable surgical option for throwers with acute UCL tears. For the original single-strand suture augmentation construct, revision UCL reconstructions can be challenging owing to the bone loss at the site of anchor insertion in the center of the sublime tubercle. This biomechanical study assessed a small-diameter (1.5-mm) ulnar bone tunnel technique for double-strand suture-augmented UCL repair that may be more easily converted to salvage UCL reconstruction if necessary, as well as

BACKGROUND: The increasing prevalence of ulnar collateral ligament (UCL) injuries, particularly in young athletes, necessitates optimization of treatment options. The introduction of UCL repair with internal bracing offers an exciting alternative to traditional UCL reconstruction.
PURPOSE: To compare midterm outcomes between UCL repair with internal bracing and UCL reconstruction in competitive athletes.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: The authors identified competitive athletes who underwent primary UCL repair with internal bracing or UCL reconstruction between 2

Reconstruction of the ulnar collateral ligament using a free tendon graft was performed on sixteen athletes. All participated in sports that involved throwing (mostly professional baseball), and all had valgus instability of the elbow. After reconstruction and rehabilitation, ten of the sixteen patients returned to their previous level of participation in sports, one returned to a lower level of participation, and five retired from professional athletics. Despite precautions, there was a high incidence of complications related to the ulnar nerve. Two patients had postoperative ulnar neuropathy

BACKGROUND: Ulnar collateral ligament (UCL) reconstruction has become an increasingly common procedure in overhead-throwing athletes because of overuse and repetitive valgus stress placed on the inside of the elbow or valgus stress during an acute traumatic event2. Athletes with a deficient UCL often report a decrease in pitch velocity and accuracy in addition to an increase in fatigue, pain, and instability4. As such, UCL reconstructive procedures have been increasing in prevalence in throwing athletes to address their symptoms and regain their throwing capabilities, particularly in young bas

BACKGROUND: Ulnar collateral ligament (UCL) reconstructions have become increasingly common, particularly in youth overhead throwing athletes. These injuries are most commonly due to overuse and repetitive trauma on the inner elbow. Throwers with a deficient UCL often report decreased pitching speeds in addition to elbow pain and instability.
INDICATION: The indications for this procedure include symptomatic valgus elbow instability during overhead throwing motions and a verified rupture of the UCL on advanced imaging.
TECHNIQUE DESCRIPTION: The ipsilateral gracilis tendon is harvested and p

Valgus extension overload is well described in the throwing athlete. The surgical treatment involves posterior olecranon osteophyte resection. It is not known how much of the posterior olecranon can be resected before increased stress is placed on the ulnar collateral ligament. Ten cadaveric arms underwent posterior olecranon resection in 4-mm increments. After each bone resection, three different valgus loads were applied to the elbow at 70 degrees and 90 degrees of flexion. Ulnar collateral ligament strain significantly increased with each load. Significantly more strain occurred on the ulna

BACKGROUND: No model exists to predict which patients with elbow ulnar collateral ligament (UCL) injuries will successfully return to play (RTP) after nonoperative treatment. The reported rates for successful RTP after the nonoperative management of UCL injuries are limited and vary widely. Furthermore, patient and injury characteristics that influence the failure of nonoperative treatment have not been established.
PURPOSE: To identify patient- and injury-specific factors predictive of successful RTP after the nonoperative management of UCL injuries in baseball players.

BACKGROUND: The ulnar collateral ligament (UCL) is the primary static stabilizer of the medial elbow between 30° and 130° of elbow flexion. Athletes participating in overhead throwing sports, such as baseball, have the highest rate of UCL injury. High-grade injuries often require surgical intervention.
INDICATIONS: Low-grade UCL injuries are typically treated nonoperatively; however, high-grade injuries can require surgical intervention for full healing and function to be achieved.
TECHNIQUE DESCRIPTION: A 5-cm incision is made from the medial epicondyle to the sublime tubercle to allow for

Repetitive valgus forces on the throwing elbow place significant stress on that joint. This stress can cause structural damage and injury to the ulnar collateral ligament. Many acute injuries of the throwing elbow are caused by repetitive chronic wear. Although much work has been done on injury prevention in youth who are pitchers, overuse injury in throwing sports constitutes an epidemic. Failing nonsurgical management, ulnar collateral ligament reconstruction is a viable option to return the throwing athlete to competition.

The ulnar collateral ligament (UCL), particularly the anterior portion of the anterior oblique ligament, is the primary static contributor to elbow valgus stability. UCL injuries are most common in athletes participating in overhead sports. Acute and chronic injuries to the UCL result in valgus instability, which may predispose the athlete to the development of disabling secondary elbow conditions. Provocative physical examination maneuvers include the valgus abduction test, the modified milking maneuver, and the moving valgus stress test. Plain radiographs and magnetic resonance imaging are t

The functional thumb is a necessity for successful athletic participation. It not only allows the athlete to manipulate athletic equipment but also allows the precise manipulation of objects in the athlete's hand. Injury to the thumb often negates power grip and finesse of the athletic hand. Injuries can range from fractures, to dislocations, to ligamentous injuries. The relatively unconstrained thumb metacarpophalangeal joint is particularly vulnerable to injury from an abduction moment to its distal segment. Such injuries occur from direct contact and with falls on equipment such as racquet

PURPOSE: Ulnar collateral ligament (UCL) injuries may result in disabling valgus instability in throwing athletes. We evaluated the docking technique for UCL reconstruction and describe a modification to the technique.
METHODS: UCL surgery was indicated in 20 high-level baseball players (13 professional and 7 collegiate) based on medial elbow pain preventing effective throwing, clinically apparent medial elbow laxity, and magnetic resonance arthrogram consistent with UCL injury. The mean age was 21.7 years (range, 17.9 to 25.3 years). One patient had previous UCL reconstruction. One had previ

Suture augmentation is frequently being used for medial ulnar collateral ligament (UCL) repair and many surgeons now augment UCL reconstructions with a high strength suture. This article and video describe the technique of suture augmented UCL reconstruction. The technique uses an autograft from palmaris or hamstring that is supported by a suture across the reconstruction. The graft is further modified using multiple points of suture augmentation to strengthen the reconstruction including a 1.6 mm FiberTak at the humerus to bolster the proximal aspect of the reconstruction. The graft is also i
