Determinants of colonic barrier function in inflammatory bowel disease and potential therapeutics.

Intestinal barrier dysfunction is a main feature of the inflammatory bowel diseases (IBD), Crohn's disease and ulcerative colitis. Leak flux diarrhoea and a facilitated uptake of noxious antigens are the two consequences resulting from an impaired epithelial barrier. Barrier perturbations in IBD comprise alterations in epithelial tight junctions (TJ), i.e. a reduced number of horizontal TJ strands and an altered TJ protein expression and subcellular distribution. Moreover, increased incidence of apoptotic events as well as erosions and ulcerations can add to that leakiness. These barrier defec

[Study on function of aganglionic colon musculature of Hirschsprung's disease murine model].

This study examined the function in vitro of aganglionic colon musculature in mice with hereditary aganglionosis--a strain of animals used as a model of Hirschsprung's disease. Double sucrose gap recordings from the muscle strips of both normal and aganglionic colon showed bursts of spike potentials with muscle contraction. Intracellular recordings of the membrane potentials of the circular muscle cells of normal, aganglionic and oligo-ganglionic colon had no statistical difference. Microelectrode recordings from the circular muscle cells of normal siblings, in the presence of nifedipine, irre

Laser Doppler flowmetry as a measure of extrinsic colonic innervation in functional bowel disease.

BACKGROUND: In functional disorders it is unknown whether disturbed function is due to an intrinsic gut abnormality or altered extrinsic innervation. AIMS: To study whether measurement of mucosal blood flow could be used as a quantitative direct measure of gut extrinsic nerve autonomic activity in patients with idiopathic constipation. METHODS: Seventy two patients with idiopathic constipation and 26 healthy volunteers had rectal mucosal blood flow measurements by a laser Doppler flowmetry probe applied 10 cm from the anus. Measurements were made at rest and after inhaled placebo and ipratro

Are there any functional differences of nitric oxide between the transitional segment in Hirschsprung's disease and the diseased colon in hypoganglionosis?

BACKGROUND/AIMS: In histological studies, there are no significant differences between the transitional segment (TS) of Hirschsprung's disease (HD) and the diseased segment in patients with hypoganglionosis (Hypo). In contrast, there are no reports whether or not TS show impaired motility like Hypo. Nitric oxide (NO) has recently been shown to be a neurotransmitter in the non-adrenergic noncholinergic (NANC) inhibitory nerves in the human gut. To clarify the significance of NO in TS and Hypo, enteric nervous responses in colonic tissue obtained from TS and Hypo were investigated. METHODOLOGY:

Quantitative imaging approaches to capture structural and functional dynamics of colonic mucus in health and disease in situ.

Mucins are highly glycosylated proteins, a subset of which form the structural basis of mucus barriers at mucosal surfaces. In the gastrointestinal tract, the secreted gel-forming mucin MUC2 is the principal component of the intestinal mucus layer and plays a critical role in maintaining host-microbiota homeostasis. MUC2 undergoes extensive post-translational glycosylation mediated by numerous glycosyltransferases, producing a dense network of O- and N-linked glycans that regulate mucus structure, microbial interactions, and barrier function. Disruption of mucus integrity has been implicated i

Colonization by Pneumocystis jirovecii in patients with chronic obstructive pulmonary disease: association with exacerbations and lung function status.

Exposure to Pneumocystis jirovecii (P. jirovecii) can lead to a wide variety of presenting features ranging from colonization in immunocompetent patients with lung disease, to invasive infections in immunocompromised hosts. Colonization by this fungus in patients with chronic obstructive pulmonary disease (COPD) could be associated with higher rates of exacerbations and impaired lung function in these patients. Our objective was to determine whether colonization by P. jirovecii in patients with COPD is associated with increased exacerbations and deterioration of lung function. This was a prosp

Tachykinin NK2 receptor and functional mechanisms in human colon: changes with indomethacin and in diverticular disease and ulcerative colitis.

Neurokinin A (NKA) is an important spasmogen in human colon. We examined inflammatory disease-related changes in the tachykinin NK(2) receptor system in human sigmoid colon circular muscle, using functional, radioligand binding, and quantitative reverse transcription-polymerase chain reaction methods. In circular muscle strips, indomethacin enhanced contractile responses to NKA (p < 0.01) and to the NK(2) receptor-selective agonist [Lys(5),MeLeu(9),Nle(10)]-NKA(4-10) (p < 0.05) in both normal and acute diverticular disease (DD) specimens, indicating NK(2) receptor-mediated release of relaxant

Overexpression of mitochondrial STAT3 protein improves colonic inflammation and fibrosis in inflammatory bowel disease by enhancing mitochondrial function.

INTRODUCTION: The STAT3 protein is involved in mitochondrial functions such as the respiratory electron transport chain, regulation of cellular metabolism, and scavenging of reactive oxygen species. Inflammatory bowel disease (IBD) is associated with damaged intestinal cells and mitochondrial dysfunction due to the inflammatory environment of the intestine. Here, we studied the potential use of the Stat3 gene to induce STAT3 expression in mitochondria to help treat IBD. METHODS: We transferred the Stat3 gene and examined its effects on the expression of proinflammatory cytokines and fibrosis

Early functional outcomes of the Swenson ileoanal pull-through for total colonic Hirschsprung disease at a high-volume referral center.

INTRODUCTION: There is no consensus for the optimal surgical approach for total colonic Hirschsprung disease (TCHD) and few studies report functional outcomes for the ileoanal Swenson pull-through (IAPT). METHODS: A single-center retrospective review identified patients with TCHD who underwent IAPT between 1/2020 and12/2023. Complications and functional outcomes were reviewed descriptively. RESULTS: 25 patients (64 % male) underwent IAPT at a median age of 1.6 years (range 0.6, - 15.6; IQR, 0.9-7.0) with a median follow-up of 16 months (range, 1-39; IQR, 3-26). Functional CD4+CD25high regulatory T cells are enriched in the colonic mucosa of patients with active ulcerative colitis and increase with disease activity.

BACKGROUND: Factors determining the extension and degree of inflammation in the colonic mucosa of patients with ulcerative colitis (UC) are largely unknown, but CD4+CD25high regulatory T cells (Tregs) have been implicated to play an important role in suppressing inflammation. Therefore, the aims of this study were to determine whether colonic Tregs have suppressive effects on colonic effector T cells in UC and to analyze the association between segmental colonic Treg distribution and disease activity. MATERIALS AND METHODS: The suppressive activity of colonic CD4+CD25high Tregs from patients

Glial Cell-Derived Neurotrophic Factor Induces Enteric Neurogenesis and Improves Colon Structure and Function in Mouse Models of Hirschsprung Disease.

BACKGROUND & AIMS: Hirschsprung disease (HSCR) is a life-threatening birth defect in which the distal colon is devoid of enteric neural ganglia. HSCR is treated by surgical removal of aganglionic bowel, but many children continue to have severe problems after surgery. We studied whether administration of glial cell derived neurotrophic factor (GDNF) induces enteric nervous system regeneration in mouse models of HSCR. METHODS: We performed studies with four mouse models of HSCR: Holstein (HolTg/Tg, a model for trisomy 21-associated HSCR), TashT (TashTTg/Tg, a model for male-biased HSCR), Pieba

Abnormal Scn1b and Fxyd1 gene expression in the pulled-through ganglionic colon may influence functional outcome in patients with Hirschsprung's disease.

PURPOSE: Smooth muscle cells are electrically coupled to ICC and PDGFRα+ cells, to regulate smooth muscle contraction. Recent studies have reported that the voltage-gated sodium channel type 1β (Scn1b), and the chloride channel subunit, Fxyd1, are highly expressed by both ICC and PDGFRα+ cells in the mouse colon. We designed this study to investigate the expression of the Scn1b and Fxyd1 genes in the normal human colon and in HSCR. METHODS: HSCR tissue specimens (n = 6) were collected at the time of pull-through surgery, while control samples were obtained at the time of colostomy closure in
