Computed tomographic colonography for colorectal screening.

Computed tomographic colonography (CTC) is a specialized computed tomographic examination that has been optimized for the detection of colorectal polyps. The technology has undergone major advances in recent years and there is emerging consensus that state-of-the-art CTC results in performance characteristics comparable to those of optical colonoscopy for polyps > or = 8 mm in size. Effective polyp detection rests on the quality of several components of the examination, which must all be optimized in order to maintain appropriate sensitivity and specificity, including adequate bowel preparatio

The future of colorectal imaging: computed tomographic colonography.

The development of colorectal cancer screening alternatives that are attractive to patients and effective for screening is critical for reducing this too-common and deadly disease. CTC is an exciting technique that is the only imaging alternative developed since the barium enema for colon screening. In the past few years, many obstacles to clinical implementation of CTC have been eliminated. For example, there is no longer any post-processing time, which was previously as long as 8 to 10 hours, and the interpretation time has been drastically reduced from 4 hours to 15 to 20 minutes. The major

Computed tomographic colonography in the diagnosis of colorectal cancer.

BACKGROUND: This study aimed to determine the sensitivity of computed tomographic colonography (CTC) in diagnosing colorectal cancer and to explore the reasons why these cancers are missed on CTC. METHODS: Patients who underwent CTC in the 56-month period from 1 January 2004 to 1 September 2008, and all cases of colorectal cancer recorded in the National Cancer Registry database from 1 January 2004 to 1 December 2008, were identified. Cases from the two data sets were then matched to identify all patients in whom CTC had been performed more than 6 weeks before a histological report was availa

Computed tomographic and magnetic resonance colonography: challenge for colonoscopy.

BACKGROUND: Computed tomographic (CT) and magnetic resonance (MR) colonography are new noninvasive techniques to examine the colon. METHODS: A literature search was performed through 2/1/2012, and relevant studies included in the review. RESULTS: MR colonography is evolving, and holds the promise that it may achieve high-level sensitivity for adenomas and cancers without exposure to radiation. CT colonography is much more mature at this point in time, and has received great attention in the last decade as a colon imaging tool, including potentially as a primary colon cancer screening method.

Double colorectal cancer only diagnosed by computed tomographic colonography.

A 58-year-old woman presented to her physician with rectal bleeding and intermittent diarrhea. Optical colonoscopy revealed a bulky tumor which was diagnosed as rectal cancer. She was referred to our institution for further evaluation and treatment. Slim optical colonoscopy showed an obstructive cancer in the rectosigmoid junction and no information of the proximal side of the obstruction. The patient then underwent computed tomographic (CT) colonography for further evaluation of the proximal side. Three-dimensional endoluminal 'fly-through' images revealed another protruded lesion in the prox

Optimisation of colon cleansing prior to computed tomographic colonography.

Colon preparation technique is a major determinant factor for patient compliance and polyp detection in computed tomographic colonography (CTC). The purpose of this study is to compare three different colon cleansing techniques in terms of patient discomfort, sensitivity and specificity. The following colon cleansing methods were compared in 20 patients each: 1. standard colonoscopy cleansing (ScCl) the day of the examination, based on polyethylene glycol (PEG), 2. a slightly reduced cleansing (RcCI) the day prior to the examination, based on a combination of diet, bisacodyl and a reduced inta

Computed tomographic colonography for symptomatic patients: the diminutive polyp dilemma.

BACKGROUND: Computed tomographic colonography (CTC) is sensitive to polyp detection but is considered inaccurate for measuring diminutive polyps (<6 mm), with divergence between CTC and either colonoscopic or histopathological polyp measurements. Reporting diminutive polyps remains debatable. This study aims to compare outcomes of symptomatic patients with diminutive versus borderline polyps on CTC and to thereby examine the potential implication of reporting diminutive polyps. METHODS: A single-centre retrospective study of symptomatic patients who underwent CTC from October 2016 through Sep

Computed tomographic colonography (CTC) performance: one-year clinical follow-up.

AIM: Computed tomographic colonography (CTC) represents a valuable advance in imaging technology for patients with colonic symptoms who are unfit for or fail to complete investigation with conventional techniques of colonoscopy or barium enema. The aim of this study was to examine whether CTC was sufficient to exclude colorectal cancer in such a population. As our patients were unfit for or unable to complete conventional investigations, we used 1 year clinical follow-up to exclude colonic malignancy. MATERIALS AND METHODS: CTC examination was performed using multi-slice CT in patients fittin

Identification of Extracolonic Pathologies by Computed Tomographic Colonography in Colorectal Cancer Symptomatic Patients.

BACKGROUND & AIMS: Symptoms suggestive of colorectal cancer may originate outside the colorectum. Computed tomographic colonography (CTC) is used to examine the colorectum and abdominopelvic organs simultaneously. We performed a prospective randomized controlled trial to quantify the frequency, nature, and consequences of extracolonic findings. METHODS: We studied 5384 patients from 21 UK National Health Service hospitals referred by their family doctor for the investigation of colorectal cancer symptoms from March 2004 through December 2007. The patients were assigned randomly to groups that

Analysis of air contrast barium enema, computed tomographic colonography, and colonoscopy: prospective comparison.

BACKGROUND: The usefulness of currently available colon imaging tests, including air contrast barium enema (ACBE), computed tomographic colonography (CTC), and colonoscopy, to detect colon polyps and cancers is uncertain. We aimed to assess the sensitivity of these three imaging tests. METHODS: Patients with faecal occult blood, haematochezia, iron-deficiency anaemia, or a family history of colon cancer underwent three separate colon-imaging studies--ACBE, followed 7-14 days later by CTC and colonoscopy on the same day. The primary outcome was detection of colonic polyps and cancers. Outcomes
