[Innovations in colorectal surgery].

In this review, we summarize the new principles, new ideas and new techniques that have emerged in recent years in the surgical treatment of colon and rectal carcinomas. The paper is a narrative summary, providing a collection of the most recent relevant literature on thematically highlighted aspects of colorectal surgery, based on an independent selection of two surgeons experienced in colorectal surgery. Complete mesocolic excision (CME) and radical lymphadenectomy for colorectal carcinomas, biological differences between right and left colon tumors, and the options for neoadjuvant drug trea

Complications in colorectal surgery.

Colorectal pediatric surgery is a diverse field that encompasses many different procedures. The pullthrough for Hirschsprung disease, the posterior sagittal anorectoplasty for anorectal malformations including complex cloaca reconstructions and the ileal pouch anal anastomosis for ulcerative colitis and familial adenomatous polyposis present some of the most technically challenging procedures pediatric surgeons undertake. Many children prevail successfully following these surgical interventions, however, a small number of patients suffer from complications following these procedures. Anticipat

Pediatric colorectal robotic surgery.

Robotic colorectal surgery allows adult and pediatric surgeons to overcome the technical limitations of laparoscopic surgery. It also provides improved ergonomics in the field of surgery. Robotic surgery has several advantages in colorectal operations that require complex minimally invasive skills including anorectal malformations, Hirschsprung disease, and inflammatory bowel disease. In this section, we discuss the key aspects of colorectal surgery where robotic instrumentation seems ideal.

Laparoscopic colorectal surgery for colorectal polyps: single institution experience.

INTRODUCTION: Because of their difficult location or size, some polyps are impossible to remove with a flexible colonoscope and must be surgically removed. Laparoscopy is a great alternative. AIM: To assess outcomes of a laparoscopic approach for the management of difficult colorectal polyps. MATERIAL AND METHODS: From 2006 to 2014, patients with polyps that could not be treated by endoscopy were included. Demographic data, histology of the biopsy, type of surgery, length of postoperative stay, complications and final pathology were reviewed prospectively. RESULTS: Forty-two patients with a

Anastomotic Leak Following Colorectal Surgery.

Anastomotic leak (AL) following colorectal surgery is a devastating complication. It is associated with high mortality rates, increased length of stay, and-in the setting of cancer-higher disease recurrence and worse overall survival. Further, it is associated with significantly increased costs, both for the acute hospital setting and the higher-intensity postdischarge care requirements these patients often experience. Identification of factors that prognosticate AL is important, as it may enable earlier detection and even patient optimization prior to surgery.

Sexual Dysfunction after Colorectal Surgery.

Sexual health is a key component of well-being and quality of life. Following colorectal surgery, many patients experience sexual dysfunction in the form of difficulty with libido, arousal, pain, and fertility. Autonomic nerve damage can explain many of these symptoms and may be a result of surgical trauma, tumor invasion, chemotherapy and radiotherapy, or pelvic inflammation and infection. An understanding of pelvic neuroanatomy can help prevent direct nerve damage during surgical dissection, but the effects of postsurgical inflammation and ischemia may be unavoidable. Despite the availabilit

Ureteral complications during colorectal surgery.

Ureteral complications occur rarely during colorectal surgery (0.3 to 1.5%), are mainly diagnosed postoperatively (50-70%), and result in both short- and long-term morbidity. The objective of this update was to report on prevention, diagnosis, and treatment options for ureteral injuries that occur during colorectal surgery. For prevention, it is essential to identify at-risk patients preoperatively. Routine prophylactic insertion of a double-J catheter is not recommended. Intra-operatively, non-invasive techniques to aid in locating the ureters based on intravenous injection of methylene blue

Urological Injuries during Colorectal Surgery.

The proximity of the colon and rectum to the organs of the urologic system virtually ensures that iatrogenic urologic injuries become a distinct possibility during complex colorectal surgical procedures. An intimate knowledge of urogenital anatomy as well as strategies for identification and repair of potential injuries is of paramount importance. Attention is mandated when operating within the narrow confines of the pelvis, as this is where these structures are most at risk. The ureters are at highest risk of injury, followed by the bladder and urethra. The nature of these injuries encompasse

Antimicrobial prophylaxis for colorectal surgery.

BACKGROUND: Research shows that administration of prophylactic antibiotics before colorectal surgery prevents postoperative surgical wound infection (SWI). The best antibiotic choice, timing of administration and route of administration remain undetermined. OBJECTIVES: To establish the effectiveness of antimicrobial prophylaxis for the prevention of SWI in patients undergoing colorectal surgery: specifically to determine, 1 Whether it reduces risk of SWI. 2 The target spectrum/a of bacteria (aerobic and/or anaerobic). 3 The best timing and duration of antibiotic administration. 4 The most eff

Laparoscopic reintervention in colorectal surgery.

Laparoscopic colorectal surgery has developed in the 1990's and beginning of 2000. The favourable results and great progress in the development of laparoscopic techniques have expanded the indications of laparoscopic colorectal surgery. More and more complicated colorectal cases are treated laparoscopically, including those having had previous laparotomies. Surgical reinterventions after colorectal procedures are common. Reinterventions are either intended to treat complications of colorectal surgery or to treat colorectal disease after previous abdominal or pelvic surgery. Laparoscopic reinte

Infectious complications in colorectal surgery.

Postoperative infectious complications still represent a relevant problem in colorectal surgery. They always results in suffering for the patients and often prolonged hospitalization. Furthermore, they result in additional expenses to cover the cost of antibiotics, blood derivates, total parenteral nutrition, nursing and additional surgical procedures. Prevention and control of hospital infections start with surveillance, which is an important means to constantly evaluate the local bacterial epidemiology. An infection surveillance program finalized to minimize the incidence of postoperative in

[Anastomotic dehiscence in colorectal surgery].

Anastomotic leakage represents the most dangerous complication in digestive surgery and, particularly, in colorectal one. Indeed a careful evaluation and modification of all possible risk factors and a correct procedure aren't often enough to overcome a potential postoperative leakage or at least material filtration. The Authors review their series to analyse the role of all possible general and local risk factors and to verify what conditions can suggest an two or three stages operation.

[Antibiotic prophylaxis in colorectal surgery].

The literature of infections after colo-rectal surgery is reviewed, including the influence of mechanical preparation and the prophylactic use of various antibiotic combinations, orally or parenterally. Is is concluded that appropriate prophylaxis is mandatory. Third generation cephalosporins singly or in combination seem to give the best results in the majority of reports. They are economically justified as prophylaxis with antibiotics decreases hospital costs.

Modern trends in colorectal surgery.

In 39 patients, the 'classical' pretreatment for colorectal surgery (mechanical purgation, laxatives, irrigation and sulfaguanidine administration) was supplemented with the oral administration of 750 mg metronidazole daily for 3 days. This dose was later increased to 1 g daily. The 75 control cases received the same pretreatment as in the test group, with the exception of the metronidazole. In the test group, obligate anaerobic pathogens could not be cultivated from the wound discharge in the infected cases, and there were no surgical fatalities. In the control group, however, anerobic strain
