Surgical Site Infection and Colorectal Surgical Procedures: A Prospective Analysis of Risk Factors.

BACKGROUND: Colorectal surgical procedures (CRS) are associated with the highest surgical site infection (SSI) rate among elective operations. A wide range of patient and surgical characteristics have been identified as risk factors for SSI. Most studies are limited by reliance on retrospective data or subset analysis of data that includes CRS. This study reflects analysis of SSI risk factors using prospectively collected data in an elective CRS population. PATIENTS AND METHODS: We analyzed data prospectively collected as part of a randomized, blinded trial of skin anti-sepsis in elective CRS

Microbial sealants do not decrease surgical site infection for clean-contaminated colorectal procedures.

BACKGROUND: Surgical site infections (SSI) are costly complications that may cause significant morbidity and increase the cost of care, particularly in colorectal surgery. Microbial sealants (MS) are a new class of wound barriers aimed at decreasing SSI; however, there is only evidence of benefit in clean class 1 procedures. Based on its success in class 1 procedures, we hypothesized that a microbial sealant could reduce the rate of SSI by half for clean-contaminated colorectal procedures (class 2). METHODS: This was a single institution, multihospital, prospective, randomized study approved

Surgical procedure for sporadic colorectal cancer in patients with mild ulcerative colitis.

Restorative proctocolectomy is recognized as the standard procedure for colitic cancer in patients with ulcerative colitis (UC). However, whether this represents the optimal procedure for UC patients with sporadic cancer remains questionable, as functional quality of life differs substantially between patients with proctocolectomy and partial resection. This study considered possible problems associated with sporadic cancer in UC. Case 1 is a 55-year-old man with a 3-year history of UC who was treated with endoscopic resection for sporadic adenocarcinoma in the rectum. Low anterior resection w

Is It Necessary to Endoscopically Evaluate the Anastomosis in Robotic or Laparoscopic Surgical Procedures for Colorectal Cancer?

Background: In the surgical treatment of colorectal cancers, disease-free survival and life expectancy are inversely proportional to the increase in complications. We evaluated the superiority of colonoscopy and air and water tests in detecting anastomotic leaks in sigmoid and rectosigmoid junction colon cancers. Methods: Data of patients who underwent robotic/laparoscopic surgical procedures for sigmoid and rectosigmoid junctional colon cancers at a single center between January 2018 and February 24 were retrospectively evaluated. The anastomoses were evaluated by intraoperative colonoscopy (

[Palliative Surgery in Colorectal Cancer - Which Factors Should Influence the Choice of the Surgical Procedure?].

BACKGROUND: The surgical procedure for patients with colorectal cancer (CRC) in the palliative situation cannot be adequately standardised. The present study was initiated to identify criteria for the decision for resection of the malignancy with or without anastomosis. PATIENTS/MATERIAL AND METHODS: In a unicentric retrospective analysis, 103 patients after palliative resection with or without anastomosis due to CRC were examined. Using univariate and logistic regression analysis, the influence of a total of 40 factors on postoperative morbidity and mortality was assessed. RESULTS: In 46 ca

Impact of Surgical and Anesthetic Procedures after Colorectal Cancer Surgery: A Propensity Score-Matched Cohort Study (The PROCOL Study).

Background: Surgical inflammatory pain decreases the innate and adaptive immune antitumor response and favors residual circulating tumor cells. Objectives: This study investigated whether minimally invasive surgeries (laparoscopic and robotic procedures), which are less painful and inflammatory, improved oncological outcomes after colorectal resection compared to laparotomy. Methods: This research was a single-center propensity score-matched study involving patients who underwent colectomy and rectum resection from July 2017 to December 2019. Results: Seventy-four laparotomies and 211 minimall

A retrospective cohort study of the effect of sugammadex versus neostigmine on postoperative gastrointestinal motility in open colorectal surgical procedures.

INTRODUCTION: Neuromuscular blockers (NMB) are used in surgical procedures to facilitate muscle relaxation and intubation. NMBs are then reversed at the end of the surgery with either an acetylcholinesterase inhibitor, such as neostigmine, or a modified cyclodextrin compound, such as sugammadex. Neostigmine and glycopyrrolate elicit counteracting cholinergic effects, potentially impairing postoperative gastrointestinal motility. This may have higher significance in colorectal surgery procedures given the baseline risk of delayed postoperative motility associated with some operations. METHODS:

Novel surgical procedure for preventing anastomotic leakage following colorectal cancer surgery: A propensity score matching study.

Hypoperfusion is the main cause of anastomotic leakage (AL) following colorectal surgery. The conventional method for evaluating anastomotic perfusion is to observe color change and active bleeding of the resection margin of the intestine and the pulsation of mesenteric vessels. However, the accuracy of this method is low, which may be due to insufficient observation time. A novel surgical procedure that separates the mesentery in advance at the intended transection site can delay the observation of anastomotic perfusion, and can potentially detect more anastomotic sites with insufficient vasc

Comparative pregnancy rate after colorectal resection versus other surgical procedures for deep infiltrating rectal endometriosis: a systematic review and meta-analysis.

The debate around colorectal surgery for endometriosis has been ongoing, but to date no meta-analysis has investigated the impact of the different surgical approaches on the pregnancy rate. The aim of this meta-analysis study was to determine in women with deep infiltrating rectal endometriosis, how does colorectal resection surgery compare to other surgical techniques (e.g., rectal shaving, disc excision) in terms of pregnancy rates. We searched PubMed, Web of Science, Cochrane library and Clinical Trials for relevant studies published from inception to December 2024. We performed a systemati

Outcomes are Local: Patient, Disease, and Procedure-Specific Risk Factors for Colorectal Surgical Site Infections from a Single Institution.

BACKGROUND: Colorectal surgical site infections (SSIs) contribute to postoperative morbidity, mortality, and resource utilization. Risk factors associated with colorectal SSI are well-documented. However, quality improvement efforts are informed by national data, which may not identify institution-specific risk factors. METHOD: Retrospective cohort study of colorectal surgery patients uses institutional ACS-NSQIP data from 2006 through 2014. ACS-NSQIP data were enhanced with additional variables from medical records. Multivariable logistic regression identified factors associated with SSI dev

Perioperative management of colorectal surgical patients receiving a direct oral anticoagulant: a scoping review, particular emphasis on procedure-specific risks, and pharmacogenomics.

Perioperative management of patients on direct oral anticoagulants (DOACs) for preoperative deep vein thrombosis (DVT), pulmonary embolism (PE), or atrial fibrillation (AF), who subsequently undergo elective colorectal surgery, is a frequent clinical scenario with no clear consensus on best practices. Further complicating this issue, venous thromboembolism (VTE) and bleeding rates vary widely, ranging from 4.8% to 12.6% for VTE and 1.1% to 2.4% for bleeding, across different procedures (e.g., abdominoperineal resection, anterior resection, rectopexy, colectomy, and total proctocolectomy), as w

Assessing Colorectal Care Capacity at an Urban Tertiary Hospital in Ghana.

Background: The burden of colorectal diseases continues to rise in Ghana. However, building a surgical workforce to address these diseases has been hampered by the lack of a colorectal specialty training pathway. To address this gap, the first colorectal surgery fellowship in Ghana was established in July 2023. Objective: This study aims to identify strengths and gaps in colorectal care delivery prior to fellowship implementation by assessing relevant infrastructure, resources, and case volume at a Ghanaian teaching hospital. Methods: Data on surgical infrastructure and human resources were co

Beyond robotic platforms: artificial intelligence and the emergence of intelligent surgical ecosystems in colorectal surgery.

Robotic surgery has substantially advanced minimally invasive colorectal surgery through improvements in dexterity, visualization, and ergonomics. However, contemporary robotic platforms remain fundamentally mechanical systems that rely almost entirely on surgeons for perception, clinical judgment, and intraoperative decision-making. As colorectal surgery becomes increasingly complex and the surgical population grows older and frailer, further progress is unlikely to depend on hardware innovation alone. This narrative review and conceptual synthesis was informed by a purposive, non-systematic

Establishing a sustainable and high-utilization robot-assisted surgery platform in a regional cancer center: insights from a multidisciplinary, break-even-guided operational strategy.

Adoption of robot-assisted surgery (RAS) in regional non-urban cancer centers is constrained by high fixed costs and insufficient case volumes to achieve financial sustainability. Break-even thresholds vary widely (120-300 cases/year) across institution types; however, operational models for resource-constrained settings remain poorly characterized. To evaluate whether a financially sustainable, high-utilization RAS platform centered on colorectal cancer surgery can be established in a regional, non-urban cancer center through a four-phase, cross-departmental, volume-driven strategy guided by

Revisiting the lateral-to-medial approach in robotic surgery for left-sided colorectal cancer.

Robotic surgery has been widely adopted for left-sided colorectal cancer; however, mesenteric mobilization is predominantly performed using the medial-to-lateral approach. This technique can be technically demanding because dissection must be performed within a narrow working space dorsal to the mesenteric vessels, requiring the identification and preservation of the ureter and gonadal vessels. In robotic surgery, arm rigidity and the lack of tactile feedback may increase the risk of inadvertent pressure on the deep retroperitoneal structures. To address these challenges, we describe a lateral
