A systematic review of acquisition and mastery of skills taught using the Picture Exchange Communication System.

The Picture Exchange Communication System (PECS) is a popular augmentative and alternative communication intervention for individuals with developmental disabilities that includes six, sequential phases of instruction. We systematically reviewed published and unpublished single-case PECS studies for details about the percentage of participants mastering PECS phases, how long it took participants to master PECS phases (in trials to mastery), and what constituted mastery of PECS phases. We found most participants mastered only the first two phases of PECS, and Phases I, II, and IV showed the slo

Speech is exhausting.

This paper focuses on my meaningful participation in society as a semispeaking AAC user, the importance of self-determination and interdependence, and the critical role of participatory research. Having spent years in speech-centric services as a child, I pushed myself to use mouthwords for decades. But the cost was high. Such "expensive speech" is defined as that which may be effective, but has a significant cost in terms of energy, cognitive resources, or other internal resources, affecting what is available for other or later skills or tasks. Multimodal AAC has decreased my cognitive load,

Current state and conceptual framework of assistive technology provision in Saudi Arabia.

PURPOSE: Assistive Technology (AT) devices provide essential means of mobility, employment, communication, social engagement for older adults and people with different disabilities, if prescribed correctly to match users' needs and goals. Regardless of the setting or location, a successful AT service delivery model includes the multidisciplinary collaboration of the people with disabilities and the specialists who have knowledge and expertise in the design and application of AT. In Saudi Arabia, unfortunately, the availability of AT devices is mainly limited to basic mobility and daily living

How ableism impacts people who need and use AAC.

I was born with a neuromuscular disability. Ever since I was a child, I encountered and experienced ableism, but I did not have the word for it. It was not until I was in my twenties when I started to read disability studies and sociology that I had a better framework for understanding how the world operates. Learning about systemic ableism as a form of oppression was a turning point. I realized that not only was the physical world never built for me, but the entire sociopolitical apparatus is centered on normative bodies and behaviors that uphold white supremacy and hypercapitalism, deeming d

Overcoming barriers to literacy for individuals who need or use AAC: lived experiences, key research findings, and future directions.

Literacy supports participation in education, employment, healthcare, and community living, and enhances communication for individuals who need or use augmentative and alternative communication (AAC). Research has demonstrated that individuals with developmental disabilities who need or use AAC can successfully acquire a wide range of literacy skills when provided with effective tools and evidence-based, adapted instruction delivered by trained professionals. Unfortunately, individuals who need or use AAC typically face significant barriers to literacy learning: (1) They may be excluded from l

Optimizing Communication in Ataxia: A Multifaceted Approach to Alternative and Augmentative Communication (AAC).

The progression of multisystem neurodegenerative diseases such as ataxia significantly impacts speech and communication, necessitating adaptive clinical care strategies. With the deterioration of speech, Alternative and Augmentative Communication (AAC) can play an ever increasing role in daily life for individuals with ataxia. This review describes the spectrum of AAC resources available, ranging from unaided gestures and sign language to high-tech solutions like speech-generating devices (SGDs) and eye-tracking technology. Despite the availability of various AAC tools, their efficacy is often

Nothing about AAC users without AAC users: a call for meaningful inclusion in research, technology development, and professional training.

Historically, priorities for research, technology development, and professional training in the field of augmentative and alternative communication (AAC) have been identified largely by nondisabled professionals with limited, if any, involvement of people who need or use AAC. In this paper, we argue that people who need or use AAC must be leaders and co-creators in all activities that are about them or impact them. AAC users are the ones who have the best insights into their unmet needs; they also are the best positioned to identify priorities, appropriate methods, and potential solutions to t

Embracing change in AAC technology: the keyboard to my success.

My name is Tim Jin, and I use an augmentative and alternative communication (AAC) device by typing with my toes. My communication journey has spanned almost four decades. Communication was the key to my success, just as important as a good education. Over the years, I have used numerous AAC devices. My parents and therapists always knew my devices would need to change as technology advanced. Each system has its unique operating system so programming an AAC device can be challenging as requirements change across systems. Hopefully one day, we will have a universal interface, ensuring seamless f

A Scoping Review of Research Evaluating Physiological Information for Individuals With Developmental Disabilities: Augmentative and Alternative Communication Research Considerations.

PURPOSE: Today, an ever-increasing number of technological advancements are becoming mainstream. As the availability of technological innovations increases, so do opportunities for evaluating any benefits of those innovations in augmentative and alternative communication (AAC) research for individuals with developmental disabilities. One innovation that has become mainstream is the use of noninvasive wearable technology to provide physiological information (e.g., heart rate, skin conductance). Such information could be valuable in AAC research, but more information about its potential feasibil

A Synthesis of Relevant Literature on the Development of Emotional Competence: Implications for Design of Augmentative and Alternative Communication Systems.

PURPOSE: Emotional competence refers to the ability to identify, respond to, and manage one's own and others' emotions. Emotional competence is critical to many functional outcomes, including making and maintaining friends, academic success, and community integration. There appears to be a link between the development of language and the development of emotional competence in children who use speech. Little information is available about these issues in children who rely on augmentative and alternative communication (AAC). In this article, we consider how AAC systems can be designed to support

Demands Associated With an Augmentative and Alternative Communication System in Relation to Alternative Forms of Access for Individuals With Motor Impairments.

PURPOSE: Successful adoption and use of aided augmentative and alternative communication (AAC) must address how to promote fluid, efficient, and organized execution of the motor behavior needed to access the AAC system. A substantial number of people who use AAC have significant motor impairments and require "alternative" access options, such as eye gaze or switch use. Such individuals may be particularly vulnerable to interference from a poorly designed system. However, the inherent demands of alternative access methods have received little direct study. The goal of this tutorial is to offer

Updated systematic-narrative review on communication intervention in Rett Syndrome: 2010-2022.

Due to loss of spoken language and resulting complex communication needs, people with Rett syndrome are obvious candidates for communication intervention. To advance evidence-based practice and guide future research efforts, we identified and summarized 16 communication intervention studies published since a previous 2009 review on this topic. Studies were summarized in terms of (a) participants, (b) dependent variables related to communication, (c) intervention characteristics, (d) outcomes, and (e) certainty of evidence. Across the 16 studies, intervention was provided to a total of 100 part

Development of a Manually Operated Communication System (MOCS) for patients in intensive care units.

Nonvocal alert patients in the intensive care unit (ICU) setting often struggle to communicate due to inaccessible or unavailable tools for augmentative and alternative communication. Innovation of a hand-operated non-touchscreen communication system for nonvocal ICU patients was guided by design concepts including speech output, simplicity, and flexibility. A novel communication tool, the Manually Operated Communication System (MOCS), was developed for use in intensive care settings with patients unable to speak. MOCS is a speech-output technology designed for patients with manual dexterity i
